Flrida Internatinal University University Graduate Schl UNIVERSITY GRADUATE SCHOOL BULLETIN ANNOUNCEMENT Dctral Dissertatin Defense Abstract The Relatinship f Thymulin Activity, Immune Parameters f HIV Disease Prgressin and Substance Use in a Chrt f HIV Serpsitive Alchl and Drug Users by Carlin Rafie Zinc is essential fr the activity f thymulin, a thymic hrmne invlved in T-lymphcyte differentiatin and activatin. Zinc deficiency is widespread in ppulatins with HIV infectin, and HIV+ drug users are particularly susceptible t zinc deficiency and immune suppressin. This dissertatin explred the relatinship f zinc-bund active thymulin t plasma zinc, CD4+ and CD8+ cell cunt, the CD4+/CD8+ rati, and drug use in HIV-infected drug users. Zinc-bund active thymulin was assessed in plasma f HIV+ drug users wh were participating in a 30 mnth zinc supplementatin trial. Plasma frm 80 participants at the 12 mnth visit, and 40 f these same participants, randmly selected, at the baseline visit were assessed fr zinc-bund active thymulin levels using a mdificatin f the rsette inhibitin assay. Thymulin activity was directly assciated with CD4+ cell cunt (β =0.127, p = 0.002) and inversely assciated with ccaine use (β = - 0.908, p = 0.026; R 2 = 0.188, p = 0.019) independent f HIV viral lad, age, gender and antiretrviral use. An increase in thymulin activity was 1.4 times mre likely when the CD4+ cell cunt increased (OR = 1.402, 95%CI: 1.006 1.956), independent f change in viral lad, antiretrviral use, and age. Participants wh used ccaine cnsistently, were 7.6 times less likely t have an increase in thymulin activity (OR = 0.133, 95%CI: 0.017 1.061). There was a direct crrelatin between change in plasma zinc and change in zinc-bund active thymulin (r = 0.243, p = 0.13). Analysis f CD4+ cell cunt decline in 222 participants in the zinc supplementatin trial acrss the 30 mnths shwed that bth crack ccaine use and heavy alchl use accelerated CD4+ cell cunt decline. Thymulin activity is directly assciated with HIV disease prgressin, measured by CD4+ cell cunt, and is depressed with ccaine use independent f antiretrviral use and HIV viral lad. Ccaine and heavy alchl accelerate CD4+ cell cunt decline. The effect f ccaine n thymic utput requires further evaluatin as a mechanism fr the assciatin f ccaine use with faster HIV disease prgressin.
