Dear Member, Hospice Palliative Care Ontario values your membership and participation in the work of the association. Your membership with HPCO which includes Associate status with the Canadian Hospice Palliative Care Association (CHPCA) expires soon. I invite you to renew your membership. Attached is the renewal information package detailing member benefits and the renewal form. HPCO continues to work on your behalf as the trusted partner with government and provincial healthcare stakeholder. Your membership and participation helps us achieve our vision which is to be the unified voice of collaborative, quality hospice palliative care in Ontario. HPCO leads the hospice palliative care movement and supports our members to improve access to quality hospice palliative care by providing: Targeted effective public policy engagement, driving high level system change for building sustainable, high quality hospice palliative care Information, education and resources, enabling members to optimize and leverage resources to deliver high quality hospice palliative care A forum for the exchange of ideas and information, enabling members to learn from each other. This past year we have seen remarkable strides in the work of hospice palliative care. There were new funding announcements for visiting hospices, residential hospices, palliative care training in long-term care, and for Health Care Consent and Advance Care Planning training and resources. The HPCO team and many of our members, worked with the Ontario Palliative Care Network to create a three-year action plan for hospice palliative care. In 2017, HPCO s Annual Conference was another success, with over 800 people attending. With a vast array of workshops, keynotes and presentations, attendees spent three days networking, learning, and sharing resources. The 2018 Annual Conference, Striving for Equity in Hospice Palliative Care, has expanded to 70 workshops and feature a new fundraising and development series. The conference gala will, as always, have spectacular entertainment. There is a pre-conference on April 21, 2018 on Palliative Care for the Homeless and Vulnerably Housed Populations. The Biennial Provincial Symposium on Paediatric Palliative Care is back this year, hosted by HPCO and the SickKids Paediatric Advanced Care Team on April 25, 2018. Dignity Memorial has a network of more than 1,800 funeral, cremation and cemetery service providers is North America who understand the special challenges families face when dealing with loss and are committed to helping families before, during and after the funeral service. Should you ever need immediate care or decide to make funeral arrangements for yourself or others in advance, the Dignity Memorial team stands ready to provide service and support. This certificate provides a credit of up to $2,500 towards the cost of funeral or cremation services and merchandise. The Personal Planning Guide that allows you to document your final wishes, as well as, estate and financial information. One year unlimited access to the 24-Hour Compassion Helpline, a telephone grief support program staffed with trained counselors. Access to the Dignity Memorial Guidance Series, a collection of booklets, DVDs and CDs offering professional advice and compassionate insight. 100% Service Guarantee. Enrollment in the program is not required to take advantage of benefits offered. All you need to do is provide Dignity Memorial with your HPCO membership receipt for the current year. Savings apply to prearranged services or services at time of need. When a death occurs, lives are suddenly interrupted; friends and family have to be contacted quickly, and many stressful decisions have to be made in a short period of time. When you choose a provider within the Dignity Memorial network, you are entitled to valuable benefits that will help alleviate some of the financial and emotional burdens that often take place. 3 Individual Membership Application Individual membership is open to any individual with an interest in hospice palliative care and furthering the work of Hospice Palliative Care Ontario. Memberships are for a one-year period commencing on the date we process your application. Under an agreement between Hospice Palliative Care Ontario (HPCO) and the Canadian Hospice Palliative Care Association (CHPCA) all individual members of HPCO automatically become an Associate of CHPCA. CHPCA affiliation gives you the national perspective and access to a wide range of information, services, resources, and interest groups.
