Important Notes: Examples of COMPLETED Immunization Forms - The form MUST be completed, signed and dated by the physician. - The form MUST also be signed and dated by the student. - Chest X-rays should be taken for students who have POSITIVE TB skin tests and have not been evaluated for the positive skin test. - The Immunization Form for Returning Students is mandatory if the results of last year s TB skin test was NEGATIVE. - Incomplete immunization forms will not be accepted. - Students must show they are fit for practicum. Incomplete submissions will hinder progress in the academic program for students, who have not fully satisfied the immunization requirements as outlined.
