Cataract Surgery Will Change the Way You See the World On behalf of all the doctors and staff, we would like to welcome you to The Eye Clinic. Whether you are a new patient or an established patient, we thank you for scheduling your cataract evaluation with us. We strive to make your cataract evaluation a pleasant and beneficial experience. In order to make your visit more productive, please take a moment to review the information enclosed in this packet. Our goal is to educate you and share with you some facts about cataracts. Our doctors offer you multifocal lens implant packages and toric lens implant packages. These advanced options can enable you to see things clearly at near, far and in between distances. At The Eye Clinic, we can customize your procedure to your eye s unique characteristics, resulting in better visual outcomes. Again, thank you for scheduling your cataract evaluation with us. We look forward to seeing you! Sincerely, Dr. Jon Yokubaitis, F.A.C.S. Dr. P. Mark Crawford Dr. Virgil Murray, IV Dr. Charles Thompson Dr. Margaret Carter Dr. Marcy Hanudel 2

Cataract Questions Why Do I Need A Lens Implant? When your cataract is removed, your eye will need a replacement lens. The intraocular (inside the eye) lens implant (IOL) is made to fit into the natural lens support system of the human eye and provides natural peripheral and distance vision. What Are My Options? Until recently, patients did not have options as to the lens implant type. The standard monofocal lens, designed to improve distance vision, was implanted in all cataract procedures. Patients had to wear glasses for closeup, arms length, and sometimes distance vision corrections. Today, with technologically advanced lenses, patients have several options. Once cataract surgery is recommended, you will be presented with different package options based on your visual needs and what type of IOL best suits your lifestyle. Your surgeon will recommend a lens implant package and specific lens implant type based on your exam, lifestyle needs, and your health. 3 www.theeyeclinic.net

Preparing for your Cataract Evaluation Appointment Premium Lens Self Evaluation Questionnaire and the Activities of Daily Living Please complete and bring your Premium Lens Self Evaluation Questionnaire and the Activities of Daily Living (found in the back of this folder) to your cataract evaluation. Your doctor will review, and utilize the results of testing and this questionnaire to select the lens best suited to your needs. If you Wear Contact Lenses If you wear hard contacts or sleep in soft contacts: do not wear your contacts for three (3) weeks before your cataract evaluation. If you wear soft contact lenses: do not wear your contact lenses for two (2) weeks before your cataract evaluation. If you wear your contact lenses to your cataract evaluation, we will not be able to complete necessary tests because we will not be able to get correct readings. Discussing Your Options and Scheduling Surgery After you meet with our doctors and have decided to proceed with cataract surgery, one of our experienced surgical coordinators will review your options and discuss your fees. The surgical coordinators will also schedule your surgery, review all instructions, eye drops for surgery and required paperwork. Cataract surgery is performed on one eye at a time. Surgery on the second eye is usually performed two weeks after the first eye. Appointment Schedule Date Time Reason 4
