Dementia Care Specialist Training Overview The Dementia Care Manager Training is designed fr varius health plans thrughut the State f Califrnia wh are participating in Califrnia s Crdinated Care Initiative, Cal MediCnnect Dual Demnstratin. Care managers are selected by the health plans t participate in the Dementia Care Manager Training. The duratin f the training is apprximately seven hurs. The training can ccur in ne day; hwever, it is preferable t cnduct ver tw days, as it better facilitates the learning prcess. The Dementia Care Manager Training aims t imprve quality f care fr patients with Alzheimer s disease and related dementias (ADRD), and their caregivers. The training ultimately wrks twards ptimizing health system perfrmance by enhancing patient care, imprving verall health, and managing csts. The training, as part f the larger Dementia Cal MediCnnect Prject, imprves dementia care capacity within health plans by strengthening wrkfrce capabilities and cultural cmpetency. The Dementia Care Manager Training aligns with gals and recmmendatins utlined in Califrnia s State Plan fr Alzheimer s Disease: An Actin Plan fr 2011-2021. Dementia Care Manager Learning Objectives: Increase understanding f Alzheimer s disease and related dementias (ADRD) and their symptms Increase ability t manage care fr peple with ADRD Increase knwledge f effective management f cgnitive and behaviral symptms f ADRD Demnstrate ability t make apprpriate referrals t available hme and cmmunity based services (HCBS) Increase care manager self-efficacy in develping and implementing care plans fr patients and their caregivers

FACILITATED DISCUSSION Facilitated discussins help the trainer guide discussins s that participants actively engage in dialgue, relate cncepts t previus experiences, discuss immediate relevance, and determine future relevance. Facilitated discussins shift the fcus f the training frm being didactic t participatry. They als allw participants t prcess cncepts and exchange ideas in a respectful manner. During facilitated discussins, the trainer shuld encurage participatin frm multiple participants and set the tne fr respectful discussin. Make sure t allw different peple pprtunities t speak and prvide affirmatins. ACTIVITY Activities help participants apply what they are learning and build team learning. Activities encurage active participatin and prblem slving. Like facilitated discussins, activities shift the fcus f the training frm being didactic t participatry and interactive.
Dementia Care Manager Training Mdificatins/Cnsideratins The Dementia Care Manager Training has been designed as an apprximately sevenhur training [nte: time n utline is nly an estimate and shuld be used as a guide]; hwever, each health plan will determine the specific amunt f time allcated t the training. In accmmdating and respecting the time allcated t the training, flexibility must be used. If a plan allcates additinal time t the training, mre in-depth discussins can take place, mre thrugh review f activities can ccur, and the trainer will be able t prvide mre detailed examples thrughut the training. If a plan allcates less time t the training, the trainer will need t scale back activities and discussins. Each trainer shuld incrprate examples, stries, and vignettes int the training. This helps bring tpics t life, increases relevance, and prmtes applicatin f cncepts. Because the Dementia Care Manager Training is being delivered within varius health plans thrughut Califrnia, the training cntent will need t be mdified fr each health plan. Training cntent needs t reflect the different structures, systems, and needs f health plans, the variatins in hme and cmmunity-based services in different gegraphical lcatins thrughut the State, and different services available thrugh varius Alzheimer s rganizatins. Training cntent, particularly in Part IV: Resurces/Supprt Services, needs t be adapted t reflect the structure f the health plans and the available resurces and supprt services in the area. Althugh mdificatins will need t be made t the training, cmpnents f the training, and general cntent, need t be maintained s there is fidelity. It is imprtant that breaks are built int the training. Breaks are nt pre-determined because each health plan will structure the training differently. The trainer needs t allw time fr participants t take breaks. This will facilitate learning and make the verall training mre prductive. Thrughut the training, the trainer will need t assess grup dynamics t determine if activities are better cmpleted individually, with a partner, r in grups. Mst activities have been designed t allw fr flexibility. If the trainer sees that the larger grup enjys wrking cllabratively and uses time effectively, activities shuld be adapted t accmmdate fr this learning style. Cnversely, if the trainer sees that peple prefer wrking independently, mre individual activities can ccur.
The trainer shuld utilize cncepts f adult learning thrughut the training t maximize effectiveness. Principles f adult learning that shuld be integrated thrughut the training, include: Dialgue (facilitate sharing life experiences and insights; participants will benefit frm hearing frm their peers/clleagues) Respect (appreciate the cntributins and life experiences f participants; cnnect existing knwledge t learning bjectives) Relevance t previus experience (make cnnectins t what peple already knw r can d) Immediate relevance (participants shuld see hw they can immediately use and apply what they have learned; applicatin f knwledge t jbs is imprtant) Future relevance (participants need t realize the utility f what they are learning fr the future) Active participatin (prfessinals have a depth f knwledge and skills t cntribute t the training; they are nt passive recipients f knwledge)
