The instructing party has requested an examination to be conducted with a report to include the nature and extent of the claimant s injuries, treatment received, effects on lifestyle and whether any further treatment is appropriate. The report is produced for Court purposes and prepared on the basis of the following information 1. Information provided by the Claimant 2. My examination, 3. Written and special instructions from the instructing party 4. My own professional medical opinion. History Accident details Road traffic accident He was involved in an accident. At the time of accident, he was a driver of a car. The car was proceeding correctly on a main road and he was wearing a seatbelt. Airbags were fitted but they did not deploy. The claimant's car was struck by an another car which failed to break in time. The impact was from the rear. He was jolted backwards and forwards. Due to the accident the vehicle was damaged and rendered a total loss. Visual Analogue Pain Scale The severity of the symptoms within this report has been done by reference to a visual analogue pain scale. Mr Harry Potter report dated 04/03/2016 3 / 13

Symptoms Bruising - Right Shoulder Current Status: resolved after 4 weeks (Claimant`s Statement) Sustained moderate bruising to the right shoulder within 24 hours after the accident. He stated that it resolved after 4 weeks. It was due to a soft tissue injury. Shock and shakiness Current Status: resolved after 1 day (Claimant`s Statement) Developed shock and shakiness after the accident. He stated that it resolved after 1 day. Pain - Lower Back Current Status: moderate(claimant`s Statement) Developed severe pain in lower back within 24 hours after the accident. It has improved and is currently moderate. They are worst (in the mornings and on lifting). It is associated with tingling and numbness. Pain - Neck Current Status: moderate(claimant`s Statement) Developed severe pain in neck within 24 hours after the accident. It has improved and is currently moderate. They are worst (in the mornings and on lifting). This is due to an exacerbation of the pre-existing condition. Symptom score - (Before the accident - 3/10; Following the accident - 9/10; Current level - 7/10). It is associated with tingling and numbness. Pain - Right Shoulder Current Status: moderate(claimant`s Statement) Developed severe pain in right shoulder within 24 hours after the accident. It has improved and is currently moderate. They are worst (in the mornings and on lifting). It is associated with tingling and numbness. Immediate treatment Immediate treatment - Yes Assistance at the scene - Yes Police and paramedics attended at the scene after the accident. Initial treatment: Advice. Following the accident, he was taken to casualty by an ambulance. Later treatment Casualty Attended casualty after the accident. Investigation: X-Ray - Neck Results: No bony injury Advice: Pain killers. GP Consulted GP after the accident. Advice: Whiplash associated injury and soft tissue injury. Prescriptions: Medications (Methocarbamol, Sleeping pills and Fentanyl Patches). Pain killers Started to self-medicate with Paracetamol and Ibuprofen after the accident. He continues to use them regularly. Physiotherapy No. of visits: 6 sessions Had an initial assessment. The treatment started after the accident and it is ongoing. He has attended 6 sessions. Has anxiety - when large vehicles come close by, loss of confidence and fear of being hit from behind. Effect on Daily Life Home restrictions Hobbies - dancing He was initially unable to do the activities (dancing) after the accident. It has improved and is currently moderate. Shopping, ironing, vacuum cleaning and house work Current severity: moderate Assistance: yes Shopping, ironing, vacuum cleaning and house work was initially severely restricted for 2 months. It has improved and is currently moderate. Received assistance from his family for 4 months. Sleep Current severity: mild to moderate Sleep was initially restricted moderately. It has improved and is currently mild to moderate. Sports restrictions Gym activities and running Current severity: moderate to severe He was initially unable to do gym activities and running. Section D Past Medical History Anxiety and depression Current status: well controlled Has a history of anxiety and depression started 6 years before the index accident and currently is well controlled. Road traffic accident Current status: recovered Had a road traffic accident 5 years ago and sustained injuries. He had fully recovered after 6 months. Neck symptoms Current status: mild to moderate Exacerbation yes Has a history of neck symptoms started approximately 6 years before the index accident and continues to suffer mild to moderate symptoms. It is exacerbated by the accident. On Examination General Physical Appearance General Health Well Dominant Hand Right General Movements Stiff Assistance None Depression None Anxiety None Limp None Noted Tearful No General Physical Examination Examination did not reveal cuts, marks, bruises, lumps and scars due to the index accident. Musculoskeletal Examination Back Right straight leg raise, rotation, left lateral flexion, right lateral flexion, back extension and back flexion movements were 90-100% of normal. It appeared to cause discomfort. No swelling. No neurological deficit. Para lumbar region was tender. Neck Left lateral flexion, left rotation, right lateral flexion, right rotation, extension and forward flexion movements were 70-80% of normal. It appeared to cause discomfort. No swelling. Deformity noticed. No neurological deficit. Para cervical region, right trapezius and left trapezius were tender. Upper limb Right hand between shoulders and right hand on head movements were 90-100% of normal. It appeared to cause pain at extremes. Mr Harry Potter report dated 04/03/2016 7 / 13

Diagnosis Opinion and Opinion I was able to obtain a very good history. His injuries were mainly consistent with the accident. In my opinion on the balance of probabilities, his symptoms are due to the exacerbation of the pre-existing condition. His treatments were appropriate. Time off: The claimant's time off has been appropriate. Employment prospects: In my opinion, he is moderately restricted at work. His ability to work is likely to improve slowly. His long term employment prospects are likely to be unaffected. Home & Sports: His home and sports restrictions are mainly due to the accident. In my opinion on the balance of probabilities the restrictions are due to the exacerbation of the pre-existing condition. He is moderately restricted. It is likely to improve slowly. Bruising - Right Shoulder Recommendation: None : Resolved - 4 weeks(from the date of accident) Sustained moderate bruising to the right shoulder within 24 hours after the accident. He stated that it resolved after 4 weeks. It was due to a soft tissue injury. Pain - Lower Back Recommendation: Referral to a Physiotherapist : Resolve - 4 months (from the date of examination) Pain - Lower Back is due to a musculo-ligamentous sprain causally related to the accident. It is currently causing moderate disability. He has undertaken 6 sessions and I would recommend to continue with the treatment sessions, if further sessions are recommended to continue as advised by the therapist. On the balance of probabilities, I would anticipate that these symptoms will improve and resolve 4 months from the date of the examination. Long term complications: In my opinion, no long term complications are expected. If he does not recover in line with the stated prognosis following the recommended treatment then I would recommend further examination by an Orthopaedic specialist. Pain - Neck is It is due to a musculo-ligamentous sprain causally related to the accident. This is due to exacerbation of a pre-existing condition. It is currently causing moderate disability. He has undertaken 6 sessions and I would recommend to continue with the treatment sessions, if further sessions are recommended to continue as advised by the therapist. On the balance of probabilities, I would anticipate that these ongoing symptoms will resolve to pre accident levels 6 months from the date of examination. I would attribute * 70% to the index accident. * 30% to the pre-existing condition. Long term complications: In my opinion, no long term complications are expected. If he does not recover in line with the stated prognosis following the recommended treatment then I would recommend further examination by an Orthopaedic specialist. Pain - Right Shoulder Recommendation: Referral to a Physiotherapist : Resolve - 4 months (from the date of examination) Pain - Right Shoulder is due to a musculo-ligamentous sprain causally related to the accident. It is currently causing moderate disability. He has undertaken 6 sessions and I would recommend to continue with the treatment sessions, if further sessions are recommended to continue as advised by the therapist. On the balance of probabilities, I would anticipate that these symptoms will improve and resolve 4 months from the date of the examination. Long term complications: In my opinion, no long term complications are expected. If he does not recover in line with the stated prognosis following the recommended treatment then I would recommend further examination by an Orthopaedic specialist. Shock and Shakiness Recommendation: None : Resolved - 1 day(from the date of accident) Developed shock and shakiness after the accident. He stated that it resolved after 1 day. Travel Anxiety Recommendation: None : Resolve - 3 months (from the date of examination) Travel anxiety is due to the psychological trauma caused by the accident. It is currently causing moderate disability. I do not believe that further treatment will benefit the claimant. On the balance of probabilities, I would anticipate that these symptoms will improve and resolve 3 months from the date of the examination. Long term complications: In my opinion, no long term complications are expected. If he does not recover in line with the stated prognosis following the recommended treatment then I would recommend further examination by a Psychologist.
