STUDII CLINICE ªI EXPERIMENTALE CLINICAL AND EXPERIMENTAL STUDIES TRATAMENTUL CU DOZE MICI DE ISOTRETINOIN ÎN ACNEEA MODERATÃ TREATMENT WITH LOW DOSES OF ISOTRETINOIN IN MODERATE ACNE A. OANÞÃ*, M. IRIMIE*, SMARANDA OANÞÃ* Braºov Rezumat Background: Administrarea clasicã eficientã a isotretinoinului în acnee constã într-o dozã de 0,5-1 mg/kgc/zi pe o perioadã variind între 4 ºi 8 luni cu obþinerea unei doze cumulative de 120 mg/kgc. Uneori aceastã dozã conduce la apariþia de efecte secundare. Obiective: Scopul acestui studiu constã în stabilirea eficienþei dozelor mici de isotretinoin în tratamentul acneei moderate. Metodã: Acest studiu necomparativ a cuprins 102 pacienþi, cu vârsta cuprinsã între 15 ºi 30 de ani diagnosticaþi cu acnee moderatã. Tratamentul a fost efectuat cu o dozã de 20-30 mg/zi de isotretinoin, reprezentând aproximativ 0,3 mg/kgc/zi, timp de 8 luni, doza medie de isotretinoin administratã unui pacient fiind de aproximativ 72 mg/kgc. Evaluarea pacienþilor s-a fãcut prin examinare clinicã ºi de laborator la sfârºitul lunilor 1, 4, 6 ºi 8 de tratament. Pacienþii au fost urmãriþi timp de 1 an de la terminarea tratamentului. Rezultate: La sfârºitul tratamentului rezultatele au fost foarte bune ºi bune la 97,9% dintre pacienþi ºi nici un pacient nu a necesitat terapie adiþionalã. Din lipsa complianþei 7 pacienþi au pãrãsit studiul, un pacient a întrerupt tratamentul datoritã agravãrii unui sindrom depresiv preexistent, iar altul datoritã unei dislipidemii severe. Ca ºi efecte secundare cutaneo-mucoase cheilita a fost prezentã la 97,9% dintre pacienþi, o uºoarã xerozã la 62,1%, tulburãri de vedere la acomodarea nocturnã la 3,15%, cefalee la 2,1% ºi epistaxis la 5,26%. Creºterea lipidelor serice (cu peste 10% mai mare decât valoarea limitei superioare a normalului) a fost gãsitã la 9,47% Summary DermatoVenerol. (Buc.), 55: 181-186 Background: Efficient classical administration of isotretinoin in the treatment of acne consisted in 0.5 to 1.0 mg/kg per day with a duration of treatment between 4 and 8 months reaching a cumulative dose of 120 mg/kg. This regimen causes many side effects that are usually dose dependent. Objective: The purpose of this study was to determine the efficacy of low-dose isotretinoin in the treatment of moderate acne. Methods: In this prospective, noncomparative study, 102 patients, both male and female, between 15 and 30 years old, with moderate acne were enrolled and treated with isotretinoin at 20-30 mg/day (approximately 0.3 mg/kg per day) for 8 months, the mean total dosage being 72 mg/kg. The clinical and laboratory evaluations were done at the end of the first, 4-th, 6-th and 8-th months of treatment. The follow-up period was one year after the treatment was finished. Results: At the end of the treatment very good and good results were observed in 97.9% of the patients. Seven patients dropped out of the study because of lack of compliance, one patient stopped the treatment because of worsening depression and another patient discontinued participation because of a severe hyperlipidemia. The most common cutaneo-mucous side effect was cheilitis in 97.9% followed by mild xerosis in 62.1%, reduced night vision in 3.15%, headache in 2.1% and epistaxis in 5.26%. Elevated serum lipid levels (up to 10% higher than the upper limit of normal value) were found in 9.47% of the patients and mild and transient abnormal liver tests were observed in * Universitatea Transilvania Braºov. 181

dintre pacienþi iar a enzimelor hepatice uºoarã ºi tranzitorie la 3,15%. Timp de 1 an dupã terminarea tratamentului s-a constatat recidiva la un singur pacient. Concluzie: Tratamentul cu doze mici de isotretinoin de 0,3 mg/kgc/zi s-a dovedit eficient în acneea moderatã, efectele secundare fiind uºoare ºi cu incidenþã scãzutã faþã de cele apãrute în cazul dozelor clasice. Conclusion: Treatment with low-dose isotretinoin of 0,3 mg/kg/day was found to be effective in the treatment of moderate acne, with a low incidence of severe side effects and at a lower cost than higher doses. Key words: acne, isotretinoin. Introducere Introducerea isotretinoinului a revoluþionat tratamentul acneei, acesta dovedindu-se eficient în mod particular în formele severe de acnee. Indicat iniþial doar în formele severe nodulochistice de acnee [1], isotretinoinul a fost prescris ulterior ºi în formele moderate de acnee care nu au rãspuns la terapia convenþionalã ºi chiar de primã intenþie în cazurile de acnee papulopustuloasã cu evoluþie cicatricialã [2]. Administrarea clasicã constã într-o dozã cuprinsã între 0,5 mg/kgc/zi ºi 1 mg/kgc/zi pe o perioadã de la 4 la 8 luni, cu obþinerea unei doze cumulative de 120 mg/kgc (cuprinsã între limitele de 100 mg/kgc ºi 150 mg/kgc) [3], aceastã dozã asigurând o vindecare completã în minimum 85% dintre cazuri. Administrarea dozei clasice poate conduce însã la apariþia de efecte secundare care de obicei sunt dependente de doza zilnicã. Aceastã situaþie a fãcut ca o serie de autori sã propunã administrarea în cazuri mai puþin severe a unor doze zilnice mai mici de isotretinoin [1, 4, 5, 6]. Metode S-a efectuat un studiu prospectiv necomparativ pe un numãr de 102 pacienþi cu acnee moderatã. Pacienþii au fost trataþi cu doze de isotretinoin de 20-30 mg/zi (Roaccutane, Hoffman- La Roche, Basel, Switzerland) corespunzând unei doze de 0,3 mg/kgc/zi, pe o perioadã de 8 luni, doza medie de isotretinoin administratã la un pacient fiind de aproximativ 72 mg/kgc. Evaluarea clinicã ºi de laborator s-a fãcut în lunile 1, 4, 6 ºi 8 de tratament. Aprecierea iniþialã ºi rezultatul tratamentului s-au efectuat dupã Investigator s Global Assesement Scale IGA versiunea 2007 [7]. Classical administration is a dose between 0.5 mg / kg / day and 1 mg / kg / day over a period of 4-8 months to obtain a cumulative dose of 120 mg / kg (within the boundaries of 100 mg / kg and 150 mg / kg) [3], this dose ensuring a complete cure in 85% of cases. Conventional dose but can lead to the appearance of secondary effects are usually dependent on daily dose. This has led some authors to propose the administration in less severe cases of lower doses of isotretinoin [1, 4, 5, 6]. Methods Non-comparative prospective study was conducted on a total of 102 patients with moderate acne. Clinical and laboratory evaluation was done at months 1, 4, 6 and 8 of treatment. Initial assessment and treatment outcome were performed after Assessment Investigator s Global Scale - 2007 IGA version [7]. Laboratory tests included blood count, determination of alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, cholesterol and 182

Testul de sarcinã a fost efectuat înaintea începerii tratamentului la toate pacientele cu potenþial de procreere. Pacienþii au fost urmãriþi timp de 1 an de la terminarea tratamentului. Rezultate Din cei 102 pacienþi înglobaþi în studiu 7 pacienþi au pãrãsit studiul din lipsa complianþei. Sex ratio M/F a fost de 1.63, iar vârsta pacienþilor a fost cuprinsã între 15 ºi 30 de ani. Acneea papulo-pustuloasã a fost prezentã la 93,68% din pacienþi, iar acneea papulo-chisticã la 6,3% pacienþi. Rezultate foarte bune ºi bune au fost obþinute la 97,9% dintre pacienþi (Fig. 1 ºi 2). Nici un pacient nu a necesitat tratament adiþional. Pe perioada de 1 an cât au fost urmãriþi s-a constatat recidiva la 1,05% din cazuri. Efecte secundare Cel mai important efect secundar cutaneomucos a fost cheilita prezentã la 97,9% dintre pacienþi, iar o uºoarã xerozã la 62,1%. Au mai fost prezente: cefalee la 2,1% dintre pacienþi, epistaxis la 5,26%, dureri musculo-scheletale la 2,1% ºi tulburãri ale vederii nocturne la 2,1%. Un pacient a întrerupt tratamentul datoritã agravãrii unui sindrom depresiv preexistent administrãrii isotretinoinului. Nici o pacientã nu a rãmas însãrcinatã pe durata tratamentului ºi douã luni dupã oprirea acestuia. Fig. 2. Results Out of 102 patients studied embedded the 7 study, patients had left the study for lack adherence. Sex ratio M / F was 1.63, and age of patients was between 15 and 30 years. Papulopustular acne was present in 93.68% of patients and papulo-cystic acne to 6.3% patients. Very good and good results were obtained in 97.9% of patients (Fig 1 and 2). No patient required additional treatment. Period of one year were followed as was found in 1.05% of cases recur. Secondary effects The most important mucocutaneous side effect was the cheil present in 97.9% of patients and a slight xerosis to 62.1%. Un singur pacient a prezentat o creºtere marcatã a colesterolului ºi trigliceridelor (de 2,5 ori faþã de limita superioarã a valorilor normalului) impunându-se oprirea tratamentului. Creºterea uºoarã ºi tranzitorie a enzimelor hepatice a fost gãsitã la 3,15% dintre pacienþi. Discuþii Evitarea efectelor secundare cauzate de dozele zilnice mari de isotretinoin a fost încercatã fie prin prescrierea intermitentã a isotretinoinului o sãptãmânã pe lunã, fie prin prescrierea de doze zilnice mai mici ale acestuia. Un studiu efectuat de Amichai ºi colab. [1] pe 638 de pacienþi cu acnee moderatã a evaluat evoluþia acneei sub o dozã de 20 mg/zi de isotretinoin (0,3-0,4 mg/kgc/zi) timp de 6 luni. Pacienþii au fost împãrþiþi în douã grupe de vârstã: 12-20 ani ºi 21-35 ani. Succesul tratamentului, apreciat prin remiterea completã sau quasicompletã a acneei, a fost de 94,8% la pacienþii între 12 ºi 20 ani, ºi de 92,6% la pacienþii între 21 ºi 35 de ani. Mandekou- Lefaki ºi colab. [5] au obþinut rezultate excelente la 68% ºi rezultate medii pânã la bune la 31,2% din cazuri într-un grup de 32 pacienþi care au primit doze mici de isotretinoin cuprinse între 0,15 mg/kgc/zi ºi 0,4 mg/kgc/zi pe o duratã de 8 luni, doza totalã fiind de 78,9 mg/kgc. Hermes ºi colab. [6] au tratat un grup de 94 de pacienþi administrând isotretinoinul într-o dozã iniþialã de 10 mg/zi cu creºtere în funcþie de efectele secundare pânã la o dozã de 0,43 mg/kgc/zi raportând rezultate foarte bune la 62,8% ºi bune la 31,9% dintre pacienþi. Durata medie a tratamentului în acest studiu a fost de 8,3 luni. Administrarea în studiul nostru la pacienþi cu acnee moderatã a dozei de 20-30 mg/zi (0,3 mg/kgc/zi pe o perioadã de 8 luni a condus la rezultate foarte bune ºi bune la 97,9% dintre pacienþi. Aceste rezultate sunt similare celor raportate în literaturã la pacienþi cu acnee moderatã care au primit doze de 0,5-1 mg/kgc/zi de isotretinoin. Utilizarea unei doze cumulative inferioare de 100 mg/kgc de isotretinoin ar expune probabil la recidive mai frecvente. În studiul nostru urmãrind evoluþia pacienþilor timp de 1 an am constatat recidivã doar la un pacient. Este cunoscut cã severitatea efectelor secundare la isotretinoin sunt dependente de dozã. În studiul efectuat efectele secundare cutaneo-mucoase ºi anomaliile de laborator au fost apropiate celor din studiul lui Amichai ºi inferioare pacienþilor care au primit doze crescute. Astfel la doi pacienþi s-a constatat o uºoarã agravare a acneei în faza iniþialã a tratamentului, dar care nu au necesitat reducerea dozei. Efectele secundare cutaneo-mucoase au fost frecvente dar de intensitate redusã. Cheilita uºoarã pânã la medie a fost întâlnitã la 97,9% dintre pacienþi, xeroza la 62,1%, epistaxisul la 5,26%. La 2,1% dintre pacienþi s-au constatat tulburãri uºoare de vedere la acomodarea nocturnã. Hiperlipidemia a reprezentat una dintre cele mai frecvente anomalii de laborator apãrutã în cursul tratamentului cu isotretinoin, fiind prezentã la 35% dintre pacienþii care urmeazã terapia convenþionalã. În studiul nostru o creºtere cu 10% peste limita superioarã a normalului lipidelor serice a fost gãsitã la 9,47% dintre pacienþi, doar un singur pacient fiind nevoit sã întrerupã tratamentul din cauza hiperlipidemiei. O creºtere a enzimelor hepatice a fost prezentã la 3,15% dintre pacienþi faþã de 10% dintre pacienþii trataþi cu regimul clasic. Dupã 1980 se discutã despre rolul isotretinoinului în apariþia sau agravarea sindromului depresiv. Studii care au comparat incidenþa depresiei înainte ºi dupã tratament nu au arãtat o creºtere semnificativã a riscului de depresie. În studiul efectuat un pacient a întrerupt tratamentul afirmând accentuarea stãrii depresive preexistente începerii tratamentului. În cursul tratamentului ºi douã luni de la întreruperea acestuia nici o pacientã nu a fost însãrcinatã. Concluzie Tratamentul cu doze mici de isotretinoin (0,3 mg/kgc/zi) s-a dovedit eficient în acneea moderatã, efectele secundare fiind uºoare ºi cu incidenþã scãzutã faþã de cele apãrute în cazul administrãrii dozelor clasice.
