Cllective Impact SHIP is a Cllective Impact prject t imprve the health and well-being f all Nebraskans. What is SHIP? The Nebraska State Health Imprvement Plan (SHIP) is a cllabrative, cmmunity driven prject, with many statewide partners wrking tgether t imprve the health and quality f life fr all individuals, families and cmmunities in Nebraska. The SHIP is develped t enhance the public health system t imprve utcmes, prmting ppulatin health and well-being f all Nebraskans. The SHIP is a data-driven plan that utlines health system and health status pririties based upn findings frm the State Health Assessment (SHA). The full Nebraska State Health Assessment 2016 reprt will sn be released. The current 2013-2016 SHIP has 5 pririties, and apprximately 82% f the activities are in prgress r are have been achieved. GENERAL OVERVIEW: Nebraska s health system is changing with an increased fcus n imprving patient and ppulatin health utcmes. It is recmmended that the public health system, the primary care system and ther health systems t establish cnnectins and interweave partnerships fr a slid fundatin f standards and practices fr individual and ppulatin health. This pririty is directed tward effrts that reduce in the inapprpriate use f emergency care, increase utilizatin f preventive services, standardize data measures and crdinate with integratin effrts particularly related t behaviral health. Changing the frame frm disparity t health equity and increasing the capacity f rganizatins t advance health equity is desired. The fundatin f this wrk must include data cllectin, crdinated delivery mdels, diversified wrkfrce and leadership.
