How can Physiotherapy help me after my operation? Recently you have had some surgery to reconstruct your breast using an implant placed under the muscle on your chest wall. You may have had a mastectomy at the same time, or you could have had one at an earlier date. This leaflet contains advice regarding the do s and don ts after this type of operation. You must follow this advice for the next few weeks. It is very important that you do not use your arm too much until the wounds have healed. It is also important that you do not let your shoulder become stiff. The exercises you do should be gentle. These should be started the day after your operation. Once you return home you should gradually increase the exercises to improve your posture and function. It is normal to feel tired after an operation. Accept and seek help as you require it. Take rest when you need it. Remember it is normal to feel anxious about arm movement. Please follow the instructions given in this advice leaflet. Which exercises should I start in hospital and when? Breathing exercises. These exercises should be started on the day of your operation. While you are awake, you should take 3-4 deep breaths followed by a cough, every hour until you are up and walking around. Deep breathing exercises. Take a deep breath in through your nose filling your lungs as much as you can, hold then slowly let the air out. Diaphragmatic breathing. Rest your hand on your stomach just under your rib cage. As you take a deep breath in feel your stomach rise and as you breathe out it should fall. Huffing or coughing. You can help to move any phlegm (secretions) that are in your chest by forcing out a breath (huffing). If you need to cough, support over the operated area with your hands. These techniques can help reduce any discomfort. 1

Circulatory Exercises. These exercises should be started on the day of your operation. Gentle leg exercises are a good way to improve your circulation while your mobility is limited. They can help to reduce the risk of blood clots in the deep veins. You can do these as often as you wish. Wiggling your toes. Pointing your toes to the ceiling, then away from you. One at a time, gently bend and straighten your knees. Tighten your thigh muscles by pushing your knee down into the bed. Hold for three seconds, then relax. Clench your buttocks together, hold for three seconds, then relax. Are there any movements I should avoid after my surgery? There are certain exercises and movements you should avoid until your wounds have healed. The tissue expander has been put under the muscle on your chest wall and it is important that you do not stretch or use this muscle until your wounds have healed. For the first 3-4 weeks after your surgery you should avoid:- Stretching your arm above your head (either forwards or sideways) or behind your back. Pushing open doors with the affected arm. Lifting or carrying anything heavy with the affected arm. Fastening your bra behind your back. Fasten it at the front then swivel it around behind you. Driving. If in doubt don t do it. Wait for help, or telephone the hospital for advice. 2

When should I start set 1 of my Shoulder Exercises? These should be started the day after your surgery. Do them about 4 times a day with 10 repetitions each time. Remember to work within your pain limits do not force any movements. Continue these exercises until you have your Physiotherapy review. 1. Shrug your shoulders up towards your ears, and then relax them. 2. Shrug your shoulders up towards your ears, then circle them back and down. Try taking some deep breaths while doing this. 3. Place hand on shoulder. Slowly raise your elbow forwards up to shoulder height. Then lower it down. 4. Place your hand on your shoulder. Slowly raise your elbow up to the side to shoulder height. Then lower it down. 5. Place both hands on shoulders. Slowly circle your elbows. Gradually aim to get your elbows to shoulder height. Exercise 6 can be done in sitting and progress to standing:- 6. Pendular exercise. Let your affected arm hang down by your side. Swing the arm in small, relaxed circles. Support the breast during the exercise. 3

When should I progress my home exercises? Once you get to 3 weeks after your surgery, and if the wounds are healing well, you can start the next set of exercises. If in doubt, check with your Physiotherapist or Surgeon. Shoulder Exercises Set 2. 1. Working within your pain limits, with your arm straight, slowly raise your arm forward as high as you can comfortably and then slowly lower it down. 2. Then with your arm straight, take it out to the side and then slowly lower it down to your side. 3. Lie on your back with your elbows bent or straight. With your hands clasped together or holding onto a stick, stretch your arms above your head. Your unaffected arm will be helping the affected one. Slowly aim to stretch your arms back towards the pillow. How often should I do these exercises? You should do these exercises 4 times a day, 10 repetitions each time. When can I start to use my arm more normally? You can begin to use your arm more normally once the process of filling the expander has started. This should be done gradually at first, keeping within your pain free range. 4

What should I do if I have problems with my arm movement? It is normal for your back and arm to feel tight for several months after the operation. It should improve with general use, and you should resume normal activities by 6 about weeks. If you go on to have the tissue expander removed and an implant inserted, then you should return to set 1 exercises for 1 week. You should start set 2 exercises the second week after your surgery, gradually increasing your range of movement towards full range. How can I reduce my risk of developing Lymphoedema? By achieving a good range of movement at your shoulder, keeping your skin in good condition, making sure that any cuts or scratches to the arm do not become infected, avoiding injections and blood pressure taking on the affected side, you can help to reduce your risk of developing Lymphoedema. If you have any problems with your arm movement, please contact the Physiotherapy Department for further advice. A Physiotherapy review will be arranged following your surgery.
