O1. Number f Medicatins (7-day lk back) Intent: Prcess: Cding: T determine the number f different medicatins (ver-the-cunter and prescriptin drugs) the resident has received in the past seven days. Cunt the number f different medicatins (nt the number f dses r different dsages) administered by any rute (e.g., ral, IV, injectins, patch) at any time during the last seven days. Include any rutine, prn, and stat dses given. Medicatins include tpical preparatins, intments, creams used in wund care (e.g., Elase), eyedrps, vitamins, and suppsitries. Tpical preparatins that are used fr preventative skin care (i.e. misturizers and misture barriers) shuld nt be cded here. Include any medicatin that the resident administers t self, if knwn. If the resident takes bth the generic and brand name f a single drug, cunt as nly ne medicatin. Antigens and vaccines als are cunted here. Write the apprpriate number in the answer bx. Cunt nly thse medicatins actually administered and received by the resident ver the last seven days. D nt cunt medicatins rdered but nt given.
Clarificatins: If a dietary supplement, given t a resident between meals, has a vitamin as ne f its ingredients, cde it as a dietary supplement, nt as a medicatin. Cding Examples: If a resident receives a daily Vitamin C capsule, add it t the medicatin cunt in number f medicatins (O1). If a resident receives a dietary supplement between meals and the label cntents specify that Vitamin C (r any ther vitamin, etc) is ne f the ingredients, cde (K5f = check) fr dietary supplement between meals. The basic TPN slutin itself (that is, the prtein/carbhydrate mixture r a fat emulsin) is nt cunted as a medicatin. The use f TPN is cded in Sectin K., Oral Nutritinal Status. Medicatins, such as electrlytes, vitamins, r insulin, which have been added t the TPN slutin, are cnsidered medicatins and shuld be cded in this sectin. Herbal and alternative medicine prducts are cnsidered t be dietary supplements by the Fd and Drug Administratin (FDA). They are nt regulated by the FDA (e.g., they are nt reviewed fr safety and effectiveness like medicatins) and their cmpsitin is nt standardized (e.g., the cmpsitin varies amng manufacturers). Therefre, they shuld nt be cunted in this item. These substances may be cded at MDS Item K5f, prvided they meet the definitin f dietary supplement fr this Item. Keep in mind that, fr clinical purpses, it is imprtant t dcument a resident s intake f such substances elsewhere in the clinical recrd and t mnitr their ptential effects, as they can interact with ther medicatins. Mre infrmatin n dietary supplements identified by the FDA can be fund at the fllwing web site: http://www.nih.gv/health. All medicatins used by the resident in the 7-Day assessment perid need t be cunted in Sectin O. All medicatins administered ff-site (e.g., while receiving dialysis r chemtherapy) must be cnsidered when cmpleting this item. The facility is respnsible fr cmmunicating with the utpatient site t identify the use f any medicatins received while the resident was under their care, and fr mnitring the effect, including any adverse effects, f medicatins after the resident s return t the facility. Cmbinatin prducts such as Crzide (which cntains a diuretic and a betablcker) are cunted as ne medicatin. Administratin f Epgen shuld be recrded in several places in Sectin O, depending n its rute f administratin and date f initiatin. It shuld be cunted at MDS Item O1 (Number f Medicatins), and if it was initiated during the last 90 days, it shuld als be indicated at MDS Item O2 (New
Medicatin). If the Epgen was given subcutaneusly, als recrd it in Item O3 (Injectins). If it is given intravenusly, it shuld be indicated at MDS Item P1ac (IV medicatin). Heparin included in a saline slutin used t irrigate a heparin lck is nt cunted in this item. Each type f insulin that a resident receives shuld be cunted separately. Fr example, Lente, Neutral Prtamine Hagedrn (NPH), and Regular are different types f insulin and are cnsidered different medicatins. Ensure r any nutritinal supplement is nt cunted as a medicatin fr cding in Sectin O. The dietary supplement culd be recrded in Sectin K5f, prvided it fits the definitins. If the resident received an injectin f Vitamin B12 prir t the bservatin perid, cde in Item O1. Vitamin B12 maintains a bld level, as d lng acting antipsychtics. Determine if a specific lng-acting medicatin is still active based n physician, pharmacist, and/r PDR input.
