Introduc8on to Medica8on Assisted Treatment (MAT) A treatment op+on for opioid dependence/ opioid use disorder used in combina+on with counseling and social support Considera4ons for individuals with opioid use disorder in the criminal jus4ce system: While incarcerated: poten4al for opioid withdrawal syndrome Upon release: risk of relapse and overdose (some4mes fatal) and return to incarcera4on Council of State Governments Jus4ce Center 6

Opioid agonist that reduces cravings for opioids and blocks euphoric effects of other opioids if they are used Taken orally Methadone Administered by Opioid Treatment Programs (OTPs) cer4fied by the Substance Abuse and Mental Health Services Administra4on (SAMHSA) Evidence that star4ng methadone maintenance during incarcera4on compared to aaer release is associated with higher rates of treatment ini4a4on in the community Council of State Governments Jus4ce Center 7

Par4al opioid agonist that requires a lower maximum dose than methadone and does not produce euphoria or withdrawal Can be combined with naloxone (opioid antagonist) to prevent misuse Taken orally Buprenorphine Can be prescribed as part of OTP or by physicians who have completed 8 hours of training Rates of relapse and rearrest for jail inmates on buprenorphine are similar to outcomes for jail inmates on methadone Council of State Governments Jus4ce Center 8

Naltrexone Opioid antagonist that binds to opioid receptors in the brain and blocks the euphoric effects of opioids Can be used for treatment of alcohol dependence/ alcohol use disorder Administered as a tablet or by intramuscular injec4on No poten4al for misuse Can be prescribed by health care providers licensed to prescribe medica4ons A pilot randomized controlled trial found lower rates of relapse for inmates who began naltrexone treatment pre-release compared to those who did not receive medica4on Council of State Governments Jus4ce Center 9

Results AWer One Year 21 individuals par4cipated in medica4on assisted treatment 92% receiving medica4on con4nued in treatment upon release, prior to program implementa4on, 50% would con4nue in treatment upon release 80% of those individuals remained in treatment 90 days or longer 85% reported a reduc4on in substance use Council of State Governments Jus4ce Center 26

Results AWer Two Years 44 individuals par4cipated in medica4on assisted treatment 75% (18 individuals) receiving medica4on con4nued in treatment upon release 82% ( 28 individuals) remained in treatment 90 days or longer 8% (2 individuals) tested posi4ve for substances while in treatment 4% (1 individual) received legal charges while in treatment Council of State Governments Jus4ce Center 27

Recidivism Outcomes As of December 2015 92 % par4cipa4ng inmates have not returned to the deten4on center one year aaer release. Council of State Governments Jus4ce Center 28
