NADIM Z. BABA, DMD, MS Professor, Department of Restorative Dentistry School of Dentistry Loma Linda University Loma Linda, California, USA Dr. Nadim Baba received his DMD degree from the University of Montreal in 1996. He completed a Certificate in Advanced Graduate Studies in Prosthodontics and a Master s degree in Restorative Sciences in Prosthodontics from the Boston University School of Dentistry in 1999. Dr. Baba currently serves as a Professor in the Department of Restorative Dentistry and the Director of the Hugh Love Center for Research & Education in Technology at the Loma Linda University School of Dentistry. He is a Diplomate of the American Board of Prosthodontics and a Fellow of the American College of Prosthodontics. Dr. Baba is an active member of various professional organizations including the Academy of Prosthodontics, the American Academy of Fixed Prosthodontics, the International College of Prosthodontists, and the International College of Dentists. Dr. Baba has received several honors and awards during his career including the David J. Baraban Award from Boston University, the Claude R. Baker Faculty Award for Excellence in Teaching Predoctoral Fixed Prosthodontics in 2009 from the AAFP, and the California Dental Association Arthur A. Dugoni Faculty Award in 2010. He is also the Regional Pacific Director of the American College of Prosthodontics, the Associate Editor for the Aesthetics/Prosthetics/ Restorative section at the Journal of Dental Traumatology, and a reviewer for the Journal of Prosthodontics. Dr. Baba is the author of numerous publications, has recently published a book entitled Restoration of endodontically treated teeth: evidence-based diagnosis and treatment planning, and has lectured nationally and internationally. 84

GUNNAR BERGENHOLTZ, DDS, DR ODONT (PHD) Professor Emeritus Department of Endodontology/Oral Diagnosis Sahlgrenska Academy University of Gothenburg Gothenburg, Sweden Dr. Gunnar Bergenholtz is Professor Emeritus and former Chairperson of the Department of Endodontology/Oral Diagnosis at the Sahlgrenska Academy of the University of Gothenburg. After graduation from the Royal School of Dentistry at Umeå University in 1964, he practiced general dentistry for 6 years. In 1973, Dr. Bergenholtz received his Specialist Certificate in Endodontics from the Swedish Board for Health and Welfare and in 1977 he defended his Doctor of Odontology thesis entitled Experimental pulpitis in the monkey. He has authored or co-authored well over 100 scientific reports, overviews, and textbook chapters primarily relating to clinical endodontics and pulp biology. Together with Drs. Preben Hörsted-Bindslev and Claes Reit, he edited the Textbook of Endodontology. Over the years, Dr. Bergenholtz has assumed various leading academic positions in Sweden as well as in the USA, where he was the first holder of the Jacob B. Freedland Endowed Chair at the University of North Carolina. He has served as Dean of the Faculty of Odontology, University of Gothenburg, and as Vice- Chair of the Board for Medical Research, Swedish Research Council. Dr. Bergenholtz has also been actively involved with the European Society of Endodontology and served as its Executive Director and President between 2001 and 2009. He has received an IADR Distinguished Scientist Award and holds Honorary Doctorships with the Aristotle University of Thessaloniki and Malmö University. Dr. Bergenholtz is an Honorary Member of the American Association of Endodontists and the European Society of Endodontology. 85

TATIANA M. BOTERO, DDS, MS Clinical Associate Professor Department of Cariology, Restorative Sciences & Endodontics School of Dentistry University of Michigan Ann Arbor, Michigan, USA Dr. Tatiana Botero received her dental degree (DDS, 1987) from CES University in Medellin, Colombia, specialty training in Endodontics (MS, 1995) at Pontificia Universidad Javeriana in Bogotá, Colombia, and the advanced Specialty Certificate in Endodontics from the University of Michigan in 2005. She was a Clinical Assistant Professor at CES University and had her private practice in Colombia (1989 2000). Dr. Botero joined the University of Michigan in 2001 and presently she is a full-time Clinical Associate Professor and teaches Endodontics to graduate and undergraduate students. Her practice is limited to Endodontics at the Dental Faculty Associate Clinic of the University of Michigan. In 2012, Dr. Botero became a Diplomate of the American Board of Endodontics and she has been on the Regenerative Endodontic Committee of the American Association of Endodontics since 2013. Dr. Botero has authored or co-authored numerous scientific peer-reviewed articles and book chapters. She serves as a reviewer for several endodontic and dental journals. Her research areas of interest and publications are on caries-induced angiogenesis and regenerative endodontics. 86

CHARLES J. GOODACRE, DDS, MSD Professor, Department of Restorative Dentistry School of Dentistry Loma Linda University Loma Linda, California, USA Dr. Charles Goodacre received his DDS degree from the Loma Linda University School of Dentistry in 1971. He completed a 3-year combined program in Prosthodontics and Dental Materials at the Indiana University School of Dentistry and in 1974 earned his MSD degree. Dr. Goodacre served as Chair of the Department of Prosthodontics at Indiana University and from 1994 to 2013 served as Dean of the Loma Linda University School of Dentistry. He currently teaches in the Advanced Education Program in Prosthodontics at the Loma Linda University School of Dentistry. Dr. Goodacre has received several awards such as Educator of the Year, Distinguished Lecturer, and Distinguished Service Award from the American College of Prosthodontists; the George Moulton Award from the American Academy of Fixed Prosthodontics; the William J. Gies Award from the American Dental Education Association; Honorary Fellowship in the Faculty of Dentistry of the Royal College of Surgeons in Ireland; the Golden Medallion Award from the American Prosthodontic Society; the Distinguished Service Award from the Academy of Prosthodontics; the Jerome and Dorothy Schweitzer Research Award and also the Distinguished Lecturer Award from the Greater New York Academy of Prosthodontics. He is a Diplomate of the American Board of Prosthodontics, Past-President of the American Board of Prosthodontics, Past-President of the American College of Prosthodontists, and Past-President of the Academy of Prosthodontics. 87

MARKUS HAAPASALO, DDS, DR ODONT (PHD), FRCDC (ENDODONTICS) Professor and Chair, Division of Endodontics Head, Department of Oral Biological & Medical Sciences Faculty of Dentistry University of British Columbia Vancouver, Canada Dr. Markus Haapasalo (DDS, PhD, Fellow of the Royal College of Dentists of Canada in Endodontics) received his dental degree from the Faculty of Dentistry at the University of Helsinki, Finland, in 1978. From 1995 to 2004 he was a Professor of Endodontics in Oslo, Norway, and since 2004 he has been Professor and Chair of the Division of Endodontics at the University of British Columbia (UBC) in Vancouver, Canada. He is also the Head of the Department of Oral Biological & Medical Sciences at UBC. Dr. Haapasalo has authored or co-authored over 170 scientific peer-reviewed articles. He is the Editorin-Chief of Endodontic Topics, a former Associate Editor of the Journal of Endodontics, an Editorial Board Member of the International Endodontic Journal, and the Editor of Visual Endodontics 2012 and Visual Endodontics for ipad multimedia. In clinical endodontics, his areas of special interest include instrumentation, biofilms, irrigation, persistent infections, and resorptions. Dr. Haapasalo has received several teaching awards including the Louis I. Grossman International Award from the French Endodontic Society in 2007 and the Bisco National Teaching Award from the Association of Canadian Faculties of Dentistry in 2014. He was a keynote speaker at the 2013 annual meeting of the International Association for Dental Research and is actively lecturing on endodontics worldwide. 88

G. REX HOLLAND, BSC, BDS, PHD Professor Department of Cariology, Restorative Sciences & Endodontics School of Dentistry University of Michigan Ann Arbor, Michigan, USA Dr. Rex Holland undertook both his scientific and undergraduate dental education at the University of Bristol (UK) before leaving for the United States. He spent time in the Dows Institute at the University of Iowa where he later undertook specialty training in Endodontics. Dr. Holland has held positions in Canada at the University of Manitoba and the University of Alberta. He returned to the U.S. and joined the faculty of the School of Dentistry at the University of Michigan in 1995. Dr. Holland s research activities have centered on the odontoblast, the innervation of the pulp, and the consequences of nerve injury in the trigeminal system. He has published a number of articles on these topics and prepared chapters on pulp biology for several textbooks. He is Editor-in-Chief of the Archives of Oral Biology and reviews papers for a number of basic science and clinical journals. 89

THOMAS KVIST, DDS, PHD Senior Lecturer Department of Endodontology Sahlgrenska Academy University of Gothenburg Gothenburg, Sweden Dr. Thomas Kvist received his DDS in 1983. Subsequently he worked in private practice in Borås, Sweden, and in Sorrento, Italy. Dr. Kvist completed his specialization in endodontics in 1993 and received his PhD in 2001. He has been Director of the Postgraduate Program and since 2009 he has held the position of Senior Lecturer in the Department of Endodontology at the Sahlgrenska Academy, University of Gothenburg. Dr. Kvist has published articles in peer-reviewed journals on endodontic retreatment decision-making and clinical outcomes. He is a member of the editorial board of the International Endodontic Journal. During the period from 2008 to 2010, Dr. Kvist was a member of a project group appointed by the Swedish Council on Health Technology Assessment (SBU) in order to systematically review the body of evidence available for Methods of Diagnosis and Treatment in Endodontics. 90

DAG ØRSTAVIK, DDS, DR ODONT Professor and Head Department of Endodontics Dental Faculty University of Oslo Oslo, Norway Dr. Dag Ørstavik received his dental degree from the Dental Faculty of the University of Oslo in 1969, and completed his specialization in endodontics at the University of Oslo in 1984. He has taught and practiced endodontics as a Professor at the University of Oslo and as a Visiting and Adjunct Professor at the University of North Carolina at Chapel Hill, as well as maintaining a part-time private practice in Oslo. His scientific career started with the graduate program at the Dental Faculty in 1972, followed by 2 years as a Research Associate in Birmingham, Alabama. He received his PhD (dr. odont.) from the University of Oslo in 1977 on bacterial adhesion and biofilm formation. From 1978 to 2004, Dr. Ørstavik was a Senior Scientist at NIOM The Scandinavian Institute of Dental Materials, where he worked with endodontic materials and ran clinical testing programs at the institute. He is currently Professor and Head, Department of Endodontics at the Dental Faculty of the University of Oslo. Dr. Ørstavik has served as Treasurer of the European Society for Endodontology and as President of the Norwegian Endodontic Society. He was Associate Editor of Endodontics and Dental Traumatology, and now serves on the editorial board of the International Endodontic Journal and is an Associate Editor of Endodontic Topics. He is also co-editor of the textbook, Essential Endodontology: Prevention and Treatment of Apical Periodontitis. Dr. Ørstavik s main interest is in the field of clinical endodontic research, with a special emphasis on radiographic analyses of treatment outcomes and the effects of medicaments and root filling materials. He has also published on technological and biological effects of endodontic materials and the effects of medicaments and materials on various oral microbes. 91

RANDOLPH TODD, DMD Clinical Assistant Professor School of Dental Medicine Stony Brook University Stony Brook, New York, USA Dr. Randolph Todd is a technology-inspired Diplomate of the American Board of Endodontics who began his academic dental career at Tuft s University School of Dental Medicine. Upon completion of a General Practice Residency at the then Cornell University Affiliate Hospital (North Shore University Hospital/Long Island Jewish Medical Center) in New York, he pursued his Endodontic specialty training at New York University College of Dentistry (NYUCD). Over the past 34 years, Dr. Todd has maintained an exclusive private specialty practice in New York City and educated countless students at various institutions including Stony Brook University School of Dental Medicine, NYUCD, and NSUH/LIJ. He was elected to the Board of Directors for the College of Diplomates and is the current President-Elect and Director of their Board Review Course. Dr. Todd is a national lecturer on the topic of cone beam computed technology and was selected to be a scientific reviewer for the Journal of Endodontics. His most recent publication Cone Beam Computed Tomography Update Technology for Endodontic Diagnosis is available in the July 2014 issue of Dental Clinics of North America. 92
