Pharmacist Objectives At the conclusion of this program, the pharmacist will be able to: 1. Explain why it is important to have pharmacists and/or pharmacy technicians involved in medication reconciliation (MR) at transitions of care (TOC). 2. Identify times when TOC MRs should occur. 3. Discuss strategies to implement a pharmacydriven MR program.
