ASIPP Spinal Cord Stimulation Comprehensive Review Course and Cadaver Workshop October 21-22, 2016 From The Chairman of the Board and CEO On behalf of the Board of the American Society of Interventional Pain Physicians (ASIPP ), it is our privilege and pleasure to invite you to attend this Spinal Cord Stimulation Hands-On Cadaver Workshop. The course will run October 21 through October 22, 2016 in Orlando Florida. Lectures on October 21 will be at the Caribe Royale Hotel in Orlando Florida and the hands on labs on October 2 will take place at the state of the art training facility at the University of Central Florida. This unique course meets the continuing needs of interventionalists, based on input and needs assessment over the years and is a cutting edge educational program. It entails the equivalent of 1 day of didactic lectures and 1 day of hands-on workshops covering various Spinal Cord Stimulation modalities and techniques. The Didactic sessions and Cadaver Workshop involve a comprehensive review of Spinal Cord Stimulation techniques and is an essential component for interventional pain physicians. This course provides education to assist you in providing high-quality, competent, safe, accessible, and cost-efficient services to your patients. Both the lectures and Cadaver Workshop are fashioned to accommodate training levels and capacity of participants. The course provides necessary CME credits and/or an in-depth review of interventional techniques. Participants will experience a comprehensive and intense learning opportunity focused on meeting their needs at the highest level of medical education. Whether you have been practicing interventional pain management for many years or are new to the field with basic skills, we are confident you will find this course and cadaver workshop to be beneficial, as it is essential to learn new and relevant information to survive in the new millennium. Our Educational Program: This course will provide extensive training in the safe and effective utilization of Spinal Cord Stimulation for the treatment of chronic pain. Statement of Need: This Interventional Techniques Course includes: 1 day of lecture and 1 day of hands on cadaver workshop. These training sessions are planned as a CME activity as an in-depth review in areas of Spinal Cord Stimulation techniques. Educational Objectives/Target Audience: This comprehensive review course and cadaver workshop for Spinal Cord Stimulation is targeted to interventional pain physicians seeking CME credit hours and introduction or review of Spinal Cord Stimulation techniques, Educational Objectives: Review basic science and core concepts Attain understanding of Spinal Cord Stimulation techniques Incorporate interventional techniques in treating your patients so that patients have better outcomes and reduced side effects Provide high-quality, competent, safe, accessible, and cost-efficient services to your patients Improve existing skills and/or develop new skills in the delivery of Spinal Cord Stimulation Outcome: Physicians should be able to integrate multiple aspects of Spinal Cord Stimulation techniques in treating their patients so that patients have better outcomes. Accreditation Statement: This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of The Institute for Medical Studies and the American Society of Interventional Pain Physicians (ASIPP ). The Institute for Medical Studies is accredited by the ACCME to provide continuing medical education for physicians. ASIPP is now an ABA Registered CME Provider. The Institute for Medical Studies designates this live activity for a maximum of 15.75 AMA PRA Category 1 Credits Physicians should claim only the credit commensurate with the extent of their participation in the activity. Physicians of Osteopathic Medicine the American Osteopathic Association (AOA) accepts AMA PRA Category 1 Credits as AOA Category 2-B credit.

ASIPP is a not-forprofit organization representing interventional pain physicians across the country. The organization has been growing rapidly since 1998 and has had a substantial impact on the practice of interventional pain medicine. The Didactic Lectures will be held at the Caribe Royale Hotel in Orlando Florida. The hands on training lab will be held at the University of Central Florida. Rules and Regulations 1. Contract The following rules and regulations become binding upon acceptance of this contract between the applicant and its employees, and the Kentucky and Indiana Societies of Interventional Pain Physicians, the show sponsor. 2. Space Assignment Exhibit locations will be assigned at the sole discretion of show management. Placement will be made based on date of reservation. 3. Booth Package Description Exhibit packages include 1 8 draped table, 2 chairs and 1 wastebasket. Total individual exhibit floor space is limited to 10 x10 unless requested and approved in writing. No walls, partitions, signs or decorations may be erected which will interfere with the general view down the aisle, or with other exhibits. 4. Show Move-In & Move-Out Move-In: Friday morning, October 21, 2016 Move-out: Friday evening, October 21, 2016 5. Exhibit Hours: Exhibit Hours: Set up Friday, October 21, 2016 6. Unoccupied Space ASIPP reserves the right, should any rented space remain unoccupied after the first hour of the show s opening, to rent or occupy said space. 7. Payments & Refunds The total amount for exhibit space is due upon the reserving of space and signing of the contract. No refunds will be made for cancellations, although, for cancellations in excess of 45 days of the meeting, we will honor a credit for the following year s meeting. 8. Food Service ASIPP reserves the right to provide food and beverage service during certain hours in the exhibit area. 9. Noisy and Obnoxious Equipment The operation of whistles or any objectionable devices will not be allowed. After the show opens, noisy and unsightly work will not be permitted.

10. Security Providing security for exhibits, exhibitor property and for exhibitors themselves, as well as for their employees, agents, representatives and guests, shall be the sole responsibility of the exhibitor and of the exhibitor only. 11. Conflicting Events Exhibitors and Meeting Participants may not conduct any ancillary events that conflict with any part of the ASIPP Meeting without permission from the meeting organizers. This includes all on-agenda scheduled events. 12. Fire and Safety Regulations All local regulations will be strictly enforced and the exhibitor assumes all responsibility for compliance with such regulations. Fire hose cabinets and fire exits must be left accessible and in full view at all times. All display materials and decorations must be flameproof and subject to inspection. No flammable substances may be used or shown in booths. 13. Liability and Insurance The Hotel management and ASIPP will take all reasonable precautions to avoid loss of exhibitors property by theft or fire, but under no circumstances shall the Hotel management or ASIPP be responsible for such losses, and it is recommended that exhibitors cover their property with suitable insurance. In the enforcement and interpretation of these rules and regulations, the decision of the Society is final. 14. Damage to Property Exhibitors are liable for any damage caused to the building, floor, walls, columns, or to standard booth equipment, or to other exhibitors property. Exhibitors must not apply paint, lacquer, adhesive or any other coating to building, columns, floors, or to standard booth equipment. 15. Public Policy Exhibitors are charged with knowledge of all ordinances and regulations pertaining to taxes, health, fire prevention, customs and public safety while participating in this show. Compliance with such laws is mandatory for exhibitors and the responsibility of the exhibitor. 16. Exhibit Floor Access ASIPP reserves the right to limit access to the exhibit floor to anyone during times the show is not officially open.

17. Use of Booth Space Exhibitors shall reflect their company s highest standards of professionalism while maintaining their booths during the show hours. No exhibitor shall assign, sublet or share booth space. 18. Exhibition Sales Policy Exhibitors may not accept payments in cash or checks or deliver merchandise in the exhibit hall. 19. Irregular Canvassing & Advertising Distribution Solicitation of business or meetings in the interest of business except that of exhibiting firms is prohibited. Exhibitors are urged to report to ASIPP any violations of this rule. Canvassing by exhibitors outside of their booths is also forbidden, except by hosts of specially hosted events.
