Your Virgin London Marathon Application Form

Running the London Marathon is an incredible challenge! Thank you for applying for a 2017 St Clare Hospice Gold Bond place for the Virgin Money London Marathon. This is a great way of raising money for St Clare Hospice and we ask that runners who are successful in obtaining one of our places pledge to raise a minimum of 2,000 in sponsorship. To be considered for one of our 6 Gold Bond places, please complete this application form, ensuring that you have clearly completed all the required boxes, giving as much information and detail as possible. Please note, places are not given on a first come first served basis. The marathon committee will select the runners once we have reached the deadline and we will announce who has been successful in October, after the official ballot has been released. If you do have any further questions, or would like to have a chat in more detail about the marathon please get in touch at the details below! - Jade Clarke, Challenges Fundraiser What a great day. Legs just starting to come back to life! It was a pleasure raising money for the Hospice and the donations have been pouring in because of the wonderful help you gave Mum and Dad John, St Clare Runner 2016 Get in touch with our Challenge s Fundraiser by phone on 01279 773750 or by email at jade.clarke@stclarehospice.org.uk Forms must be received by the Hospice before 12 noon on Friday 7 October. You can return the form via post to: London Marathon, St Clare Hospice, Hastingwood Road, Hastingwood, Essex, CM17 9JX or email the form to: jade.clarke@stclarehospice.org.uk

Why run for St Clare? You will be supporting people with life limiting illnesses. From clinical supplies to staff for home visits, the money you raise will help St Clare provide our essential care. You will be invited to information sessions to meet your fellow runners. We organise several information evenings where you have the chance to meet the rest of the St Clare Marathon team, speak to fundraising and discuss any ideas or concerns as well as the chance to speak to experienced runners and qualified sports therapists about how to avoid injury. You will receive an exclusive St Clare Hospice running vest personalised with your name on it. You will receive all the advice and support you need to reach your fundraising goal. We can provide you with the tools and ideas to help you raise money, quickly and easily, through sponsorship, match funding and fundraising events. We ll be cheering you on all the way! From sharing your story on our Ballot Places By applying in the public ballot entry system you increase your chances of running in the London Marathon. The ballot opens in May for 5 consecutive days. If you have applied in the ballot and are lucky enough to be successful, then we still want you to join our St Clare Hospice London Marathon Team! social media or maybe even our website, to pumping you up on the day, we ll do what we can to let everyone know what a hero you are! There is a huge demand for places so we want to ensure that they go to runners who appreciate the commitment required both to training and fundraising. Good Luck!

The Application Form... 1. Personal Details Full Name Date of Birth Address Postcode Evening phone number Daytime phone number Email address Occupation Company name & Address Does your company have a matched giving scheme? A matched giving scheme is where your employer matches the amount of sponsorship you raise for a local charity. E.g. If you raise 250, your employer matches that figure so that the Hospice receives 500. This is a good way for the company to support a local charity and will allow you to reach your fundraising target. 2. You and St Clare Please explain why you want to run for St Clare Hospice.

Please give us any details of how you have been involved with St Clare Hospice in the past. Including any volunteering, fundraising or events that you have supported us at, and any friends or family that have influenced you in supporting us. 3. Running Did you apply for a place through the ballot system? Have you run a Marathon before? Have you taken part in any organised runs in the past? If yes, please give details. If yes, how many have you run in? How are you planning to train for the Marathon? 4. Fundraising Please give any details of previous fundraising experience you have had. How much money are you hoping to raise? We ask that you pledge to raise a minimum of 2,000. Please clearly explain how you believe you will raise this money, giving details

5. Media Would you be prepared to speak the local paper? Please provide us with any information which might make your story interesting to the local media I.e. running your 10 th marathon, turn 50 this year. Yes/No
