REFERATE GENERALE GENERAL REPORTS INFLUENÞA DIETEI ÎN ACNEE DIET INFLUENCE ON ACNE LESIONS DANA PETRESCU-SECELEANU* Rezumat Multã vreme, legatura dintre dietã ºi apariþia leziunilor de acnee a fost consideratã a fi un mit. Studiile recente aratã un posibil risc legat de dietele cu exces de glucide, ca ºi cele cu un consum exagerat de lapte de vacã. Prezentul referat face o trecere în revistã a ultimelor concluzii în acest domeniu. Cuvinte cheie: acnee, dietã, exces glucide, lapte de vacã. Summary For a long time, the relationship between diet and acne lesions was considered as a myth. Recent studies show a potential risk related to excessive carbohydrates diets. (Buc.), 56: 323-329 Acneea este o boalã a omului, aparutã la nivelul foliculului pilo-sebaceu. Pentru iniþierea leziunilor de acnee sunt necesari multipli factori endogeni ºi exogeni. Primul pas îl constituie hiperkeratoza prin proliferarea-retenþia la nivel de infundibul. Se formeazã un filament folicular din care apare microcomedonul. Ulterior apar comedoane închise ºi deschise. Retenþia hiperkeratozicã în comedoanele închise ºi în papulele inflamatorii se datoreazã unei disfuncþii a diferenþierii keratinocitelor terminale. În acest moment propionibacteria colonizeazã foliculul ºi comedoanele. Pot apãrea fenomene inflamatorii cu diferite intensitãþi, în funcþie de severitate depinzând ºi cicatricea. Propionibacterium acnes, acþionând la nivelul TLR-2 (toll-like receptor), pot stimula secreþia de citokine (IL-6, IL-8- de cãtre keratinocitele foliculare ºi IL-8 ºi IL-12 la nivelul macrofagelor) care iniþiazã inflamaþia. Anumite specii de P.acnes pot induce reacþii imunologice prin stimularea producþiei de peptide anti- Acne is a human disease, developed at the level of sebaceous follicles. For the initiation of the acne lesions, multiple endogenous and exogenous factors are necessary. The first step is the hyperkeratosis in the infundibulum through the proliferation and retention. A follicular filament is forming and thus the microcomedo arises. Finally open and closed comedones appear. The hyperkeratotic retention in the closed comedones and in the inflammatory papules is due to a disfunction of the differentiation of the terminal keratinocytes. Propionibacteria colonize the follicle and the comedones. Inflammatory phenomena of different intensities can appear. The scar is also depending of the severity of inflammation. Propionibacterium acnes, actioning at TLR-2 level (toll-like receptor), could stimulate the cytokine secretion (IL-6, IL_8 by the follicular keratinocytes, IL-8 and IL-12 by macrophages), initiating the inflammation. microbiene la nivelul sebocitelor si keratinocitelor. Schimbãri calitative ale lipidelor sebumului induc alterarea diferenþierii keratinocitare ºi secreþia de IL-1, contribuind la dezvoltarea hiperkeratozei foliculare. Glandele sebacee joacã un rol important participând activ la imunitatea înnãscutã a pielii. Ele produc neuropeptide, excretã peptide antimicrobiene ºi prezintã caracteristici ale celulelor stem. Androgenii afecteazã sebocitele ºi keratinocitele infundibulare influenþând diferenþierea celularã, proliferarea, lipogeneza sebacee si comedogeneza. Factorul genetic este indiscutabil, dar cascada inflamatorie este up-reglatã de factori imunologici [1, 2]. Un studiu pe 1002 elevi iranieni (vârsta 16+/- 0.9 ani) aratã leziuni de acnee moderat-severã în 14% din cazuri (19.9% cu istoric familial de acnee). Riscul de severitate a crescut cu numãrul membrilor din familie cu istoric de acnee, istoricul mamei fiind cel care a influenþat cel mai mult severitatea[3]. Factorii exogeni, subestimaþi în trecut, par a avea un rol important. În diverse studii, prevalenþa acneei este crescutã (55-93%) [3-5]. 21.7% dintre studenþii mediciniºti din Pakistan considerã factorii ereditari rãspunzatori [4]. De asemenea, unii pacienþi recunosc influenþa hormonilor (faza premenstrualã, pubertatea întârziatã) în declan- ºarea leziunilor de acnee [3, 4]. Dar, pacienþii considerã ºi alþi factori ca fiind declanºatori sau agravanþi ai acneei. Printre factorii exogeni menþionaþi sunt: dieta[1-8], stresul emoþional [3, 8], canicula[8], transpiraþia [8], igiena pielii [8]. Pacienþii cu acnee din diferite culturi considerã dieta ca o cauzã majorã a acneei. Nu se poate dovedi, însã, dacã este adevaratã implicarea dietei ºi în ce proporþie sau este doar o concepþie falsã [7]. Prima lege a dieteticii pare a fi: dacã are gust bun, nu e bunã pentru tine (Isaac Asimov - biochimist si scriitor de science-fiction). Aceastã politicã din anii 50 nu se regãseºte acum, deºi pacienþii se aºteaptã ca medicul sã le dea o listã de alimente care sunt legate de acnee. Pânã la 32% dintre pacienþi aleg dieta ca o cauzã principalã a acneei, iar 44% din pacienþi cred cã alimentaþia le agraveazã leziunile. 41% din studenþii mediciniºti australieni considerã dieta ca un factor de exacerbare a acneei[6]. reactions through the stimulation of the antimicrobial peptides production by the sebocytes and keratinocytes. Qualitative changes of sebum lipids induce the alteration of the keratinocytic differentiation and also IL-1 release, playing a role in the development of follicular hyperkeratosis. Sebaceous glands are very active for the innate immunity of the skin. They are producing neuropeptides, releasing antimicrobial peptides. They are also presenting characteristics of stem cells. The androgens are affecting both sebocytes and infundibular keratinocytes with influence on the cellular differentiation, proliferation, sebaceous lipogenesis and comedogenesis. Genetic factors are undisputable, but the inflammatory cascade is up-regulated by the immunological factors [1,2]. A study on 1002 Iranian pupils (age 16+/- 0.9 years) shows moderate to severe acne lesions in 14% of cases. Almost 20% of the patients had family history of acne. The severity risk was increasing with the number of family members with acne history, mother s history being the most important influencing the severity [3]. Exogenous factors, underestimated in the past, seem to play also an important role. Different studies are presenting a high prevalence of acne, between 55 and 93% [3-5]. 21.7% of medical students from Pakistan are considering hereditary factors as responsible for acne [4]. At the same time, some patients are recognizing the influence of hormones (premenstrual phase, late puberty) in the appearance of acne lesions [3, 4]. But, patients are taking in consideration other factors as precipitating or aggravating acne. Some exogenous factors mentioned are: diet [1-8], emotional stress [3, 8], high temperature [8], sweat [8], skin hygiene [5]. Acne patients from different cultures are considering diet as a major cause of acne. It cannot be proved if there is any diet involvement, in which proportion or is only a false conception [7]. First law of dietetics seems to be: If something has good taste, it is not good for you (Isaac Asimov- biochemist and science-fiction writer). This politics of the 50-s is not actual now, even the patients are still waiting that the doctor will give them a list of food related to acne. Up to 32% of patients are choosing diet as a principal cause 324

Alimentele dulci ºi grase au fost considerate ca factori de risc pentru acneea moderat-severã. Pe de altã parte, alimentele condimentate, fumatul nu au fost asociate cu severitatea acneei [3]. 29% dintre studenþii pakistanezi au menþionat bãuturile nealcoolice drept factorul major de exacerbare [4]. În medicina occidentalã, asocierea dietãacnee a fost sugeratã, dar nu a fost doveditã. Relaþia dintre dietã ºi acnee este, însã, consideratã importantã în medicina tradiþionalã chinezã. 322 de studenþi au fost clasificaþi în douã grupuri: predominant yin (48%), predominant yang (52%). 25% dintre subiecþi au avut leziuni de acnee, restul intrând în lotul martor. Nu au fost gãsite asocieri între dietã ºi acnee câtã vreme a fost luat lotul omogen. În grupul predominant yin, consumul de mâncare de la standurile stradale a fost asociat cu o incidenþã scãzutã a acneei. În grupul predominant yang, aportul de deserturi, sucuri proaspete a fost asociat cu o incidenþã crescutã a acneei, în timp ce consumul de lactate ºi produse din soia a fost semnificativ asociat cu o incidenþã scãzutã a acneei. Abordarea medicinei tradiþionale chinezeºti conduce la observarea de asocieri semnificative între dietã ºi incidenþa acneei [9]. Dincolo de credinþele pacienþilor, anumite tipuri de alimente au fost dovedite ca fiind implicate activ în patogeneza acneei: alimentele cu încãrcãturã glicemicã ridicatã ºi laptele prin inducerea unor niveluri crescute de 5 alfadihidrotestosteron [1, 2]. Studii recente antropologice pe populaþii neoccidentalizate sugereazã cã dieta, incluzând cantitatea de carbohidraþi, ar putea fi implicatã în etiopatogenia acneei. 43 de pacienþi de sex masculin au fost recrutaþi pentru o observaþie de 12 sãptamâni. Tratamentul experimental a fost o dieta în care 25% din energie era asiguratã de proteine ºi 45% din carbohidraþi cu indice glicemic scãzut, acesta comparându-se cu o dietã care cuprindea alimente bogate în carbohidraþi, fãra vreo referire la indicele glicemic. Dupã 12 sãptãmâni numãrul leziunilor ºi severitatea lor a of acne and 44% of them are believing that diet could aggravate their lesions. A lot of patients are relating the aggravation of acne lesions to some elements of their diet: nuts [8], chocolate [3, 8], fat food [3, 6, 8], fried food [8], eggs [8], sweets and biscuits [3, 8], spices [8], tea and coffee [8]. Sweet and fat food were considered as risk factors for moderate to severe acne. On the other hand, spice food, smoking were not associated with the severity of acne [3]. 29% of students from Pakistan mentioned soft drinks as major factor of exacerbation [4]. Western medicine suggests the association between diet and acne, but it was not been proven. The tradition Chinese medicine considers the relationship diet-acne as very important. 322 students were divided into two groups: predominant yin (48%) and predominant yang (52%). 25% of subjects had acne lesions, the rest remaining as control group. There were no associations between diet and acne as long as the group was considered homogenous. In yin group, the intake of food from the street stalls was associated with a low incidence of acne. In yang group, the intake of desserts, fresh fruit juices was associated with a high incidence of acne, whereas the intake of dairy and soy products was significantly related to a low incidence of acne. The Chinese traditional medicine approach could give significant associations between diet and acne [9]. Beyond the patients beliefs, some types of food have been proven to be actively involved in the pathogenesis of acne. High glycemic load food and milk can induce increased tissue levels of 5alpha-dihydrotestosterone [1, 2]. Recent anthropological studies on non- Westernized populations are suggesting that diet, including the quantity of carbohydrates could be involved in the ethio-pathogenesis of acne. 43 male patients were recruited for observation during 12 weeks. The experimental treatment was a diet with 25% of the energy supplied by proteins and 45% by low glycemic load carbohydrates. There was a comparison with a diet with food rich in carbohydrates, without any referral on the glycemic load. After 12 weeks, the 325

Aceste studii au subliniat rolul factorilor nutriþionali în mecanismul patogenic al acneei [10, 12]. Pe de altã parte, un alt studiu caz-control pe 49 pacienþi cu acnee ºi 42 de subiecþi sãnãtoºi nu aratã diferenþe semnificative ale nivelurilor glicemiei, insulinei, leptinei sau ale indicelui glicemic, infirmând rolul acestora în patogeneza acneei la pacienþii tineri [13]. S-a estimat o asociere între aportul de lapte ºi derivate ºi prezenþa acneei. S-a presupus cã asocierea ar putea fi datoratã prezenþei hormonilor ºi moleculelor bioactive din lapte[14,15]. O cohortã de 4273 de bãieþi a fost studiatã din punct de vedere al stilului de viaþã ºi a factorilor implicaþi, comparându-i pe cei cu un aport crescut (de 2 ori/zi) ºi pe cei cu un aport scãzut (1/sãptãmânã) de lapte (lapte integral, semidegresat ºi degresat). S-a constatat o asociere pozitivã între cei cu aport de lapte degresat ºi acnee. Acest lucru sugereazã faptul cã laptele degresat conþine constituenþi hormonali sau factori care influenþeazã producþia endogenã de hormoni, în cantitãþi suficiente pentru a avea efecte biologice asupra consumatorilor [16]. Studii foarte recente aratã mecanismele prin care laptele este implicat în patogeneza acneei ºi a altor boli. Consumul de lapte de vacã ºi a proteinelor din laptele de vacã duce la o modificare a axei hormonale a insulinei, hormonului de creºtere ºi a factorului de creºtere insulin-like (IGF-1). Consumul de lapte duce la creºterea nivelului seric al IGF-1 în perioada perinatalã, în adolescenþã ºi la adult. La pubertate, odatã cu debutul fiziologic al creºterii secreþiei de hormon de creºtere, nivelurile serice de IGF-1 sunt ºi mai mult crescute de consumul de lapte. IGF-1 este un mitogen puternic, dupã legarea de receptorii sãi din diferite þesuturi, el induce proliferarea celularã ºi inhibã apoptoza. Keratinocitele ºi sebocitele, ca ºi adrenalele ºi gonadele sintetizatoare de androgeni sunt stimulate de IGF-1. Incidenþa epidemicã a acneei number and the severity of lesions has decreased more in the group with low glycemic load compared to control group. The experimental diet has also reduced weight, body mass index and a greater improvement in insulin sensitivity. An increase of glycemic load in the diet could induce the increase of the activity of sexual hormones and IGF-1, aggravating the acne lesions. These studies had underlined the role of nutritional factors in the pathogenic mechanism of acne [10-12]. On the other hand, another case-control study on 49 acne patients and 42 healthy controls does not show significant differences of the levels of glucose, insulin, leptine pr glycemic index, infirming their role in the acne pathogenesis in young patients [13]. An association between milk and dairy intake and acne was estimated. There is a presumption that this relationship could be due to the presence of hormones and bioactive molecules from milk [14, 15]. A cohort of 4273 boys was studied regarding the lifestyle factors. There was a comparison between the boys with high intake (2 times/day) of milk (whole milk, low-fat milk and skim milk) with those with low intake (1/week). There was a positive association between intake of skim milk and acne. This finding suggests that skim milk contains hormonal constituents or factors that influence endogenous hormones, in sufficient quantities to have biological effects on consumers [16]. Very recent studies show the mechanisms relating milk to the acne and other diseases pathogenesis. The intake of cow milk and proteins of cow milk can induce a change of the hormonal axe of insulin, growth hormone and insulin-like growth factor (IGF-1). Milk intake increases the serum level of IGF-1 in the perinatal period, in the adolescence and as adult. At the puberty, with the physiological debut of the increase of secretion of growth factor, serum levels of IGF-1 are higher with the milk intake. IGF-1 is a potent mitogen. After its binding on the receptors on different tissues, it induces the cellular proliferation and inhibits the apoptosis. Acneea poate fi privitã ca un model pentru bolile cronice vestice prin stimularea crescutã patologic a IGF-1. Multe alte organe, cum ar fi timusul, oasele, toate glandele ºi celulele musculare netede vasculare, ca ºi neuronii, pot fi influenþate de aceastã stimulare hormonalã anormal crescutã. Modificãrile induse de lapte la nivelul axei IGF-1 pot contribui la dezvoltarea macrosomiei fetale, inducerea atopiei, creºterea acceleratã, aterosclerozã, carcinogenezã ºi la declanºarea bolilor neurodegenerative. Datele oferite de biologia molecularã sunt susþinute de date epidemiologice ºi de consumul de lapte ca un promotor al bolilor cronice ale societaþii occidentale[17]. Dupã ce o lungã perioadã de timp implicarea dietei în acnee ca factor declanºator sau agravant a fost consideratã un mit, în ultimii doi-trei ani se remarcã un interes deosebit pentru studii pe aceastã temã. Studiul norvegian din 2009 pe un lot de aproape 4000 de subiecþi vine cu urmãtoarele observaþii: asocierea acneei la bãieþi cu stresul mental(or=1.63), consumul frecvent de ciocolatã/dulciuri (OR=1.4), chipsuri (OR=1.54), iar la fete cu stresul mental (OR=2.16), consumul redus de legume crude (OR=1.41), venit redus al familiei (OR=2.14). Nu s-au remarcat asocieri legate de consumul de bãuturi rãcoritoare dulci, peºte gras, alcool sau de fumat[18]. Acneea se poate asocia cu afectare psihologicã semnificativã (depresie, anxietate, gânduri suicidare). Anumite suplimente alimentare (acizi graºi omega 3 din uleiul de peºte, crom, zinc, seleniu) pot avea efecte pozitive atât antiacneice, cât ºi asupra dispoziþiei [19]. De altfel, încã din 1993 se publicau observaþiile legate de efectul benefic al dozelor moderate de zinc asupra leziunilor de acnee [20]. Suplimentarea cu vitamina A are efecte benefice în acnee. Vitamina A este necesarã pentru diferenþierea normalã a þesuturilor epiteliale, având un rol vital pentru menþinerea ºi funcþionarea la parametri optimi a sistemului imun înnãscut ºi dobândit. De asemenea, vitamina A are rol de agent antiinflamator [21]. stimulated by IGF-1. The epidemic incidence of acne through the teenagers of Western society, that is high milk consumer, could be explained by the increase of the insulin and IGF-1 stimulation by the sebaceous glands, mediated through the milk intake. Acne could be seen as a model for Western chronic diseases through the pathologic high stimulation of IGF-1. Many other organs, like thymus, bones, all glands, vascular smooth muscular cells and also neurons, could be influenced by the high abnormal hormonal stimulation. The changes induced by the milk, on the IGF-1 axe could contribute to the fetal macrosomy, atopy induction, accelerated growth, atherosclerosis, carcinogenesis and neurodegenerative diseases. Data offered by molecular biology are sustained by epidemiological data and by the milk intake as a promoter of chronic diseases of Western society [17]. A long time, diet was seen as a myth speaking about its role as precipitating or aggravating factor in acne. But, in the last couple of years, there is an increased interest on this subject. There is a Norwegian study done in 2009, on a lot of 4000 subjects. There are some observations derived from this huge study: in boys there is an association between acne and mental stress (OR=1.63), frequent intake of chocolate/sweets (OR=1.4), chips (OR=1.54). As regarding girls, there is a relationship between acne and mental stress (OR=2.16), low intake of raw vegetables (OR=1.41), low income of the family (OR=2.14). There were no associations with the intake of sweet beverages, fat fish, alcohol or smoking [18]. Acne could be associated with significant psychological problems (depression, anxiety, suicidal thoughts). Some supplements (fatty acids omega 3 from fish oil, chromium, zinc, selenium) could have benefic effects both on acne lesions and on the mood [19]. Far from 1993, there are some observations regarding the positive effect of the moderate doses of zinc on acne lesions [20]. Vitamin A supplementation has also positive effects on acne. Vitamin A is necessary for the normal differentiation of epithelial tissues, with a vital role for the function of innate and adaptive immune system. It has also an antiinflammatory role [21]. 327

Dietele de excludere nu au arãtat, însã, ameliorãri semnificative ale leziunilor decât în anumite cazuri [22]. Negatã cu vehemenþã decade întregi, asocierea acnee-dietã este din ce în ce mai mult studiatã, noi porþi fiind deschise. Pânã atunci, poate ar fi util sã adaptãm fiecãrui caz dieta, în funcþie de proprii factori agravanþi. O informare a posibilului risc legat de dietele cu glucide în exces ºi cu un consum exagerat de lapte de vacã, ar fi o atitudine potrivitã pânã la noi concluzii. Intrat în redacþie: 22.05.2011 Some acne patients could present positive reactions on allergic tests to different food (almonds, cheese, mustard, red pepper, malt). But, exclusion diets did not show significant improvement of the lesions [22]. The diet-acne association was denied for decades, but now, is intensively studied with open results to come. Up to new results, we could adapt the diet to every case, taking in consideration the aggravating factors for every patient.
