Our wellness initiatives at Winter have allowed us to keep employee costs for healthcare premiums the same for a fifth consecutive year, thanks in part to your participation. A successful and award-winning wellness program, like Go365, is all about helping you improve your lifestyle and manage rising healthcare costs. We have partnered with Humana to be the model of excellence, achieving 100% participation of covered employees and passing the savings on to you. Your participation is critical to our ability to keep healthcare premiums low! In the past, employees were required to reach Silver Status to be eligible for the Wellness Rate for the following plan year. In 2018, employees are required to reach Gold Status to be eligible for the Wellness Rate in 2019. Read on to find out about the program features and see how easily you can get to GOLD and live a healthier life in the process! Gold Status 8,000 One adult per policy 12,000 combined Two adults per policy 4,000 additional For each dependent 18+ on the policy Your first steps to get to Gold Starting 1/1/2018: Step 1: Complete Health Assessment Log onto your account at Go365.com to complete your health assessment. This will give you Recommended Activities designed specifically for you. Recommended Activities can have a big impact on your overall health, as well as earn you a lot of points. Step 2: Biometric Screening Winter will be holding two biometric screening days, one at the 191 office and one at the Norcross office. Family members are welcome to attend. To sign up for Norcross, go online to: bit.ly/norcross-2018 To sign up for 191, go online to: bit.ly/191-2018 Step 3: Preventive Screenings Complete preventive exams based on your age and gender and submit within 90 days to earn up to 400 additional points. Employees will receive an additional day of PTO to complete their screenings in 2018. (See the compliance chart and forms for more details). Step 4: First Workout Earn 750 points by completing your first verified workout. Step 5: Continue Earning Points to Reach Gold Status See the following pages, as well as your Ready. Set. Preventive Screenings Our employees are our most valuable asset. We are committed to making sure that you are at your healthiest. Preventive exams are an important part of ensuring that you are staying on top of your health. In 2018, employees will be allowed 8 hours of paid time off to complete health screenings that are appropriate for their age and gender. You can also earn up to 400 Go365 points for completing appropriate screenings, helping you get to Gold staus. 1. Visit your doctor & get your appropriate screenings. See the following pages to determine what screenings are appropriate for your age and gender. 2. Earn your Go365 Points To earn your points, you may either go online or mail/fax the Go365 Prevention Activity form to Humana. 3. Earn your PTO Submit your doctor s note to Human Resources.

Preventive Health Guide Here are suggested guidelines for screenings and checkups for healthy adults. There are also vaccination and immunization guidelines for children and adults on the back page. Individuals with a family history of certain diseases and those with other risk factors should discuss preventive health measures with their primary care physician (PCP). For complete benefit details, refer to your plan s Certificate of Coverage or Summary Plan Description. Preventive care keeps you healthy, prevents illness and detects disease in the early stages when it s easier to treat. Healthy adult preventive healthcare guidelines Wellness visit Annually. This should include a health review with your provider and a physical exam. Your exam should include measurement of height, weight and body mass index (BMI). It s also a good time to talk to your doctor about ways to increase physical activity. Cholesterol/lipid screening Every five years, or more often if you re at risk or have above-normal levels. A 9-12-hour fast is required. If you have cardiovascular problems or diabetes, you may need a test every year. Diabetes screening and tests Every three years, or more often if you re at risk. If you have diabetes, the following exams and tests are recommended each year: HbA1c test, LDL cholesterol, kidney test and retinal or dilated eye exam. Blood pressure Every 1-2 years if normal (less than 120/80), more often if you have high blood pressure or diabetes. Bone density screening for osteoporosis Periodically as directed by your doctor, usually around age 65 or older. Some may need frequent screening after a bone fracture or if you re at high risk for osteoporosis. Eye exam Every 2-4 years up to age 64 and then every 1-2 years. Eye exams test for glaucoma and macular degeneration two common age-related conditions. Colorectal cancer screenings Talk to your doctor about your risks and which colorectal cancer screening test and schedule is best for you. For ages 50 and older, most recommend an annual fecal occult blood test, a flexible sigmoidoscopy every five years or a colonoscopy every 10 years. Women only Mammogram Every 1-2 years for women age 40 and older. Younger women should follow their doctor s recommendations. Also talk to your doctor about clinical breast exams. Pap test and pelvic exam At least every three years or more often as directed by your doctor. Talk to your doctor about your risks and which Pap testing schedule is best, especially if you ve had a hysterectomy or are 65 or older. only Men only Prostate exam Talk to your doctor about your risks and which test and schedule is right for you. Exams include a digital rectal exam and a prostate-specific antigen test (PSA).
