The Maryland Department of Health publishes a comprehensive guide to affordable dental care that can be used in conjunction with this guide to providers serving Montgomery County residents. Affordable Dentures (Columbia East Marketplace) 7351 Assateague Dr., Suite 480, Jessup, MD 20794 Phone: 443-440-5883 young adults M-F, 7:30-4 English Offers dentures, partial dentures, denture repairs, implants, tooth extractions, sedation dentistry, X-rays, and gold crowns on dentures. No insurance accepted. Prefer appointments. Walk-ins accepted but may be longer wait. A complete set of dentures ranges from $755 to $2,085, including exam and X-rays. Depending on the child's needs, the clinic also offers routine dental check-ups and cleanings, oral sedation dentistry and treatment under general anesthesia, sealants, fillings and crowns, and wisdom teeth extraction. Orthodontics, braces, and oral surgery are also offered. The hospital accepts most insurance plans and Medicaid. Eligible patients may receive discounted services through the hospital's Financial Assistance Policy. Dentists work out of their offices. Care includes X-rays, cleaning, fillings, etc. Participating specialists also provide free care. Partial and full dentures provided as a part of overall oral health. There is a 10- to 24- month waiting list depending on the Maryland county. Serves all of Maryland. All services are fee-for-service. The initial exam and X-ray is $149. Dental treatment in the predoctoral clinic is provided by dental and dental hygiene students under the supervision of faculty members. Prospective students are primarily selected on the basis of their teaching potential and therefore may not be accepted into the predoctoral program. When this occurs, patients are referred to one of the college's postdoctoral programs for evaluation. Patients who are not accepted into a postdoctoral program will receive a list of private practitioners for possible care. Postgraduate dentists also provide general dental treatment and specialized care in oral and maxillofacial surgery, pediatric dentistry and orthodontics. The clinics are open year round, but close for one month during the summer. Sliding fee scale for uninsured patients. For initial appointment, bring photo ID, proof of address, list of benefits, employment information and pay stubs for the latest month; if unemployed, bring notarized letter from person supplying support, if any. The clinics are able to treat homebound patients with mobile dentistry equipment. Page 5

Mission of Mercy Maryland/Pennsylvania Mobile Dental Program Frederick Church of the Brethren 201 Fairview Ave., Frederick, MD 21702 Administrative Offices: 301-682-5683 Adults 18 and up, in-state or out-of-state; free to everyone Twice monthly on Mondays, hours: 9-4, extractions only (no wisdom teeth), walk-ins only, no appointments, doors open at 8, but many patients line up during the early morning hours; for schedule, visit www.amissionofmercy.org/m aryland-pennsylvania/clinicschedule/ English/Spanish The free dental program is staffed by one volunteer dentist per day, seeing a maximum of 10 patients per day. Only extractions are available at the Frederick clinic. Mission of Mercy clinics at Reisterstown, Taneytown and Brunswick offer fillings and extractions. Page 6

New patients should register online and they will be called for an appointment. There is a $25 consulting fee for new patients providing an ID and proof of income; those without documentation will be charged a $50 fee for a consultation. Fees for dental work are on a sliding scale depending on family income. The Georgia Avenue clinic requires a referral from a primary doctor at any medical clinic in Montgomery County or from the Spanish Catholic Center medical clinic in order to receive dental services. No referral needed for D.C. residents at the Monroe Street clinic. *DOCTOR'S REFERRAL NEEDED United Way of Central Maryland Project Homeless Connect 410-895-1535 www.uwcm.org/phc Adults 18 and up Holds two free clinics a year, a two-day clinic in Baltimore and a one-day clinic at another Maryland location; 8-5 each day English/Spanish Partners with the University of Maryland School of Dentistry with the goal of holding a free clinic once a year in Baltimore. The next Baltimore clinic is tentatively scheduled for October 2018, with location to be announced in April 2018. Clinics are open to anyone over the age of 18. Please consult web site or call Project Homeless Connect for upcoming clinics and locations. The first day of the clinic focuses on social service resources, as well as dental services, with the second day devoted to dental services. Cleanings, fillings and extractions are provided, but no root canals, crowns or dentures. Arrive early, clinic is on a first-come, first-served basis, with some patients camping overnight. No ID or other documents needed. Page 9
