CY2017 Hospital Outpatient: Vascular Procedure APCs and Complexity Adjustments Comprehensive Ambulatory Payment Classifications (c-apcs) In CY2015 and in an effort to help pay providers for quality, not quantity of care, CMS created comprehensive APCs (c-apcs). C-APCs package payments for services and supplies rather than providing separate and multiple payments for each individual service. c-apcs provide a single, all-inclusive payment for the primary service with no additional reimbursement for additional adjunctive services and supplies used during the delivery of the primary service. For CY2017, CMS increased the number of clinical families from 14 to 21. New for CY2017, diagnostic catheters and angiographies are now classified as a c-apc; and as a result, the PCI vascular family will be comprised of four c-apc levels instead of three. Note, the c-apc changes for 2017 are listed in the table below, and reflect the CY2017 vascular clinical family (VASCX) c-apcs. c-apc - 2017 Description National Average Payment 5191 (formerly 5188) Level 1 Endovascular Procedures $2,832 5192 (formerly 5191) Level 2 Endovascular Procedures $4,823 5193 (formerly 5192) Level 3 Endovascular Procedures $9,748 5194 (formerly 5193) Level 4 Endovascular Procedures $14,776 Complexity Adjustments To ensure appropriate accounting of all resources, CMS is also applying complexity criteria for multiple procedure combinations on the same date of service to allow for mapping to a higher APC within that clinical family of procedures. Code combinations are pairs of HCPCS. The highest payment for the family is the top level of the family of APCs. For the vascular family, APC 5194 Level 4 Endovascular Procedures is the highest level of payment.
