Clinical Fellowship for TMS/Inpatient Services/ Mood Disorders Program/ ECT -Electroconvulsive Therapy (TIME) Name of Institution: Location: Number of positions: McGill University Health Centre Montreal General Hospital Royal Victoria Hospital/ Allan Memorial Institute Montreal Neurological Hospital 1 for the 1-year stream (without Short-Term Dynamic Psychotherapy) and 1 for the 2-year stream (with Short-Term Dynamic Psychotherapy) Length of fellowship: 1 or 2 years Program information for TIME: Academic affiliation: McGill University Department of Psychiatry Hospital involved: McGill University Health Center 100% Background: The McGill University Health Centre provides services for electroconvulsive therapy and transcranial magnetic stimulation for patients, inpatient, and outpatient and consultation service. Patients treated with ECT/TMS are selected from a group of diagnostic categories. Schizophrenia, schizoaffective disorder, schizophreniform disorder, bipolar disorder, major depressive disorders, Parkinson s disease, catatonia, neuroleptic malignant syndrome, postpartum psychosis. After referral proper evaluation, consultation and informed consent procedures, patients are treated in the PACU (Post aesthetic care Unit) at the Montreal General Hospital or the Gate House (part of the Allan Memorial Institute - AMI) Research activity is available in inpatient services, mood disorders, TMS and ECT. In ECT particular research may focus on the schizophrenia, maintenance or continuation of treatments, anaesthetic procedures and informed consent. In TMS, research can focus on typologies of depression and related diagnosis. The McGill University Health Center also provides teaching and training in various formats of Short-term dynamic psychotherapy (S-TDP) as well as in Dr. Davanloo s technique of Intensive Short-Term Dynamic Psychotherapy (IS-TDP), which is based on his discovery of the technique of unlocking of the unconscious. This is carried out at the Allan Memorial Institute at the S-TDP Center. Videotechnology remains an integral part

of our day to day operation and plays a major role in our teaching, training and research activities. Publications: Opportunity for publishing a paper in the area of ECT/TMS as well as of S-TDP (for fellows in the 2-year stream) will be provided and encouraged. Also poster presentations will be promoted for local, national, and international conferences. Publications from psychiatrists in inpatient, outpatient, and CL services are collated for fellows. Mission: The TIME Fellowship is structured to provide clinical, educational, and administrative and research experience and training. The aim of the fellowship is to produce clinicians who are fully competent to utilize ECT and/ortms in a select group of patients. Also the fellow will be competent to educate medical students, residents, other clinicians and be capable of designing and administering a program for ECT and/ortms in a hospital setting. Fellows in the 2-year stream will also be trained in the area of psychodiagnostic assessment- patient s capacity to experience and tolerate depression, anxiety and painful affect; technique of mobilizing the patient against their psychopathology; applying some of the basic technical interventions inherent in Dr. Davanloo s technique of IS-TDP; as well as learning other modalities of short-term dynamic psychotherapy. How will fellowship enhance residency training? The TIME fellow will have a greater appreciation, understanding of diagnostic categories which may benefit from ECT/TMS. The fellow will have the enriched academic/clinical opportunity to function in mood program, inpatient service and CL service. The fellow will participate, present cases in clinical conferences, supervise residents and provide sophisticated care to patients and their families. A broad literature exposure pertaining to ECT/TMS is readily available (Journal of Convulsive Treatment and the Journal of ECT Libraries availability). Being a fellow on the inpatient/outpatient/cl services will enlarge the number of clinician s attending to patients and offer a stimulating academic environment. Fellows in the 2-year stream will also have a greater understanding of several modalities of dynamic psychotherapy. The fellow will learn how to assess patient s suitability for S- TDP and how to apply inherent interventions to the technique. Name of Fellowship Supervisor: Thomas M. Milroy, M.D., FRCP(C) Director, MUHC Adult Inpatient and ECT Services Life Member of the American Psychiatric Association Member, Canadian Psychiatric Association Member, International Society of Electroconvulsive Therapy and Neurostimulation Assistant Professor of Psychiatry Board Certified USA (Psychiatry & Neurology), Canada (FRPCS), Quebec (QCS)
