Please keep this information and let it serve as a reminder for your appointment date/time. Our office is located at 5501 West Bethel Avenue, Muncie, IN 47304. We ask that you complete this packet and bring it with you to your first visit. If you are scheduled for an initial evaluation you will need to report to the office 30 minutes prior to your appointment time to complete any additional information we may need. Due to limited seating in each waiting area, we ask only the patient and one caregiver be present. If you must bring your children with you, you will need to bring someone to watch them in the waiting area while you are at your appointment. The estimated time you may be here for your first appointment is 2 hours. You may be asked to change into an exam gown for your first visit. This is helpful during your initial evaluation for your pain management. Prior to your first visit, you may receive a telephone call from a representative of IU Ball Hospital to pre-register you for your first visit. You must be pre-registered for each and every visit. If you have not been contacted, you will need to register upon arrival to the office and will need to come early enough so you can be seen at your appointment time. You may call them at 765-751-5075 or toll free 866-477-3734. On the day of your appointment you will need to make sure you present your insurance card(s), driver s license, and any current medications. We will collect any co-payments if any is due. We do accept cash, cashier s check, money order, Visa and Mastercard. IT IS YOUR RESPONSIBILITY TO CHECK WITH YOUR INSURANCE CARRIER TO MAKE SURE YOUR VISIT WILL BE COVERED. After your visit you may receive two separate bills. You may receive one bill from IU Ball Hospital for the use of the facility. A second bill may come from American Health Network, which includes any practitioner you may see. For any questions regarding billing, please contact the appropriate number(s) as indicated on your statement. The numbers for the billing departments are as follows: IU Ball Hospital (765) 747-3068 and American Health Network (765) 213-6330. We are pleased to have the opportunity to be able to assist you with your pain management needs. If you have any questions, please feel free to contact our office at (765) 741-2957. We are open Monday through Friday, 8:00am to 4:30pm. We ask that you extend us the courtesy of notifying our office 24 hours in advance if you need to change or cancel your appointment.
