WHAT is the hants health and wellness TEAM? The Hants Health and Wellness Team is a health care team that promotes emotional and physical wellness and self management of chronic conditions. We do this by helping individuals to gain knowledge, skills, and confidence to take charge of their own health. We provide the following core services for citizens and their families in the Hants area: Health and wellness programs We provide free health and wellness programs on healthy eating, physical activity, emotional wellness and risk factor management. In addition to programs offered at the Hants Community Hospital, we also work closely with community partners to offer our programs in a variety of locations. WELLNESS NAVIGATION Navigators work with you one-on-one to find you the right services to help. We re health professionals who know healthcare, the community and government systems. We work with adults, children, youth and families. CHRONIC DISEASE MANAGEMENT We provide free individual and group programs for chronic disease management for people with diabetes, high blood pressure, high cholesterol, and/or those who need nutrition education. COMPLEX CARE We also provide free individual appointments to support complex care for citizens who have three or more specific chronic conditions. We will work with you and your regular health care provider to manage your health care. Anyone can self-refer to our health and wellness programs. We ask that if you are referring yourself to and complex care programs that you have the support of your family physician or nurse practitioner. 2

Building Better Goals This learning session will help you set realistic health goals. You will explore what motivates you and what stops you from making changes. At the end of this session you can develop a plan to help you achieve your goal. Tuesday, June 28 1:30-3:30 p.m. Hants Community Hospital, Room 2A Build Better Sleep Do you ever wonder how you could get a better night s sleep? Join us for a discussion on what you can try to sleep better. Tuesday, June 21 6-8 p.m. Hants Community Hospital, Room 1B Drop-In For people who have attended a Hants Health and Wellness Team group program and would like extra support with their goals to achieve better health. No sign-up needed. Tuesdays: May 17, June 28, July 12 & August 9 11 a.m. - noon Hants Community Hospital, Room 2A Heart Healthy Living: 2-session program What are the top five heart numbers you need to know? They are blood pressure, weight, cholesterol, fasting blood sugar and waist circumference. Want to know why? Join the Hants Health and Wellness Team to find out and start on your road to a healthier heart. Wednesdays: June 15 & 22 1:30-3:30 p.m. Hants Community Hospital, Room 2A Living Well with a Chronic Condition - 4 session program Do you ever wonder what it means to have diabetes, high blood pressure, or high cholesterol? Do you feel overwhelmed trying to decide what you should eat, how much exercise you should do, or how to manage your risk factors? This program will help you and your family to take control of managing your chronic conditions. Wednesdays: 9-11 a.m. Hants Community Hospital, Room 2A May 4, 11, 18 & 25 Wednesdays: June 8, 15, 22 & 29 9-11 a.m. Hants Community Hospital, Room 2A Personal Wellness Profile Complete a lifestyle survey and have your cholesterol, blood sugar, blood pressure, body fat, and waist circumference measured. Receive a personalized report that lets you know how you are doing in nine health areas and find out your health age. reducing your health risks (continued) Prediabetes Wouldn t it be nice if the body had a check engine light to let us know if something could go wrong with our health? Prediabetes, when blood sugars are outside the normal range, offers a warning and gives us a chance to change the future. Not everyone with prediabetes will develop diabetes, but the risk increases if steps are not taken to manage it. In this session you will learn about lifestyle changes to manage the risk factors that contribute to developing diabetes. Thursday, June 23 9-11 a.m. Hants Community Hospital, Room 1A Understanding Pain Do you have pain, experience chronic pain or would like to support someone who does? This session will explain pain, some of the changes that can occur with chronic pain and help you understand it. Having a better understanding of pain may allow you to live with chronic pain better. Tuesday, May 24 1:30-3:30 p.m. Hants Community Hospital, Room 2A Nutrition and HealthY Eating Discovering Your Best Weight: 4-session program In this four week program we will explore expectations about weight and strategies to help you to achieve your best weight while living the healthiest lifestyle you can truly enjoy. Mondays: June 21 & 28 Tuesdays: July 5 & 12 6-8 p.m. Hants Community Hospital, Room 1A How Healthy is Your Plate Interested to know what is a portion size, serving size, what and how much to eat? Come and learn about Canada s Food Guide in a new way. Thursday, May 12 10-11:30 a.m. Hants Community Hospital, Room 2A Thursday, August 11 10-11:30 a.m. Hants Community Hospital, Room 2A Parenting My Child is Anxious. Should I Worry? 2-session program All children have some anxiety. How do you know when to be concerned? This two week program, we will discuss anxiety often seen in children up to 12 years old. You will learn how to identify early signs of anxious feelings in your child and skills to help your child manage everyday anxiety. You will have the opportunity to practice these new skills at home and come back to discuss with the group. Information on additional health and community resources will also be available. Physical Activity Is It Just Me? Why Is Change So Hard? Explore how ready you are to make physical activity lifestyle changes and receive support as you work toward making healthy behavior a priority. Wednesday, June 8 6-7 p.m. Hants Community Hospital, Room 1A NEW Physical Activity on a Budget Do you wish you were more physically active, but feel that cost gets in the way? Discover the many ways to be physically active with little or no cost in this single session. You will have a chance to try out some of these activities, and discover affordable options to become and stay physically active within your community. A participant physical activity screen is required. Please call no later than five business days before the program start date to complete this form. Tuesday, May 10 1:30-3:30 p.m. Hantsport Baptist Church Wednesday, May 25 6-8 p.m. Hants Community Hospital, Room 1A Ready, Set, Move! 4-session program Take the whole four week series or the session that interests you! Join our physiotherapist to learn about the components of a physical activity program and practice skills in stretching, strengthening, and aerobic exercise. A participant physical activity screen is required for sessions two, three and four. Please call no later than five business days before the program start date to complete this form. Session 1: Beware your Chair! - Join us for this ground-breaking session as we uncover the impact of sitting on health and discuss strategies to build more light physical activity into your day (no physical activity screen required). Thursday, June 16 1:30-3:30 p.m. Hants Community Hospital, Room 2A Session 2: Aerobic Physical Activity Workshop - Learn and practice basic techniques for aerobic physical activity. We will also help you make an action plan and find low or no-cost physical activity resources in your community (physical activity screen required). Thursday, June 23 1:30-3:30 p.m. Hants Community Hospital, Room 2A Session 3: Strengthening and Core Stability Workshop - Learn and practice the basic techniques of an effective strength training program so that you can perform these exercises at home! (physical activity screen required). Thursday, June 30 1:30-3:30 p.m. Hants Community Hospital, Room 2A Session 4: Stretching and Balance Workshop - Learn and practice the basic techniques of stretching and balancing exercises so that you can perform these exercises at home! (physical activity screen required). Emotional Wellness Communicate with Results: Introduction Join us to learn and to practice assertiveness skills. Assertiveness means asking for what you want and expressing how you feel, while respecting other people. Assertiveness is a communication style that can help you feel confident and improve your relationships. Wednesday, June 29 6-8 p.m. Hants Community Hospital, Room 1A Discover Your Personal Strengths 2-session program Recognizing and understanding your own strengths and using them in your daily life can help you deal with difficult situations and find more meaning. In the first session you will participate in group discussions and activities to discover your own strengths. In the second session you will learn how to apply them to real life situations so you can benefit from them. Mondays: June 13 & 20 1:30-3:30 p.m. Dykeland Lodge Feel Better About Yourself The way we feel about ourselves can have an impact on our relationships with others, the things we do in our lives and our overall well-being. Join us for a discussion about focusing on our strengths and becoming the best version of ourselves. Thursday, June 9 6-8 p.m. Hants Community Hospital, Room 1A First Things First: Time Management Do you find yourself overloaded by the things you need to fit into your day? Are you interested in learning tips and tools to achieve a better balance in your life? Join us to learn more about time management skills. Monday, May 16 6-8 p.m. Hants Community Hospital, Room 1A Living with Stress: Introduction Stress is a part of everyday life. Join us for this one session to learn about the stress response and explore stress management techniques. Wednesday, May 11 1:30-3:30 p.m. Dykeland Lodge Living with Stress: Skills for Life 4-session program Stress is a part of everyday life. Having a toolkit to handle stress can help us feel capable of handling new stressful situations as they come up. Join us for this 4 week program and learn skills to deal with stress and practical techniques such as exploring your values, staying present, and relaxation. Mondays: June 13, 20, 27 & July 4 6-8 p.m. Hants Community Hospital, Room 2A Renovating Your Relationships 2-session program Our lives are full of relationships with other people. Join us for group activities to help you find ways to nurture relationships in your life. Mondays: July 11 & 18 6-8 p.m. Hants Community Hospital, Room 1A 7

Program Partners The Hants Health and Wellness Team provides space free of charge for community groups to offer programs and services to the public. The following programs are offered by these partners. To sign-up or for more information, please refer to the contact information in the description. ADHD Information Session - West Hants Community Mental Health For adults supporting children with ADHD, have a diagnosis themselves, or those who would like to learn more about ADHD. Participants will come away with a better understanding about ADHD as well as information about programs and resources to help better manage ADHD. For more information, please call the West Hants Community Mental Health at 902-792-2042. Incredible Years 10 - Week Parenting Program The Incredible Years program is a 10 - week program that will address a variety of topics to strengthen all of the great parenting skills you already have and give you the opportunity for support from other parents. Registration is required. The program for parents of children five to twelve years old will run this fall. The program for parents of children three to five years old may be offered in the summer depending on registrations. Please call Lisa at 902-798-2264 to sign up for either program. Stop Smoking - Nova Scotia Health Authority Whether you want to stop, cut down, or are simply looking for reliable information about tobacco use, the Nova Scotia Health Authority offers free programs to help you. The programs are run by trained professionals.
