Schedule of fees are available upon request. Billing for placement of dental professionals The dental professional provided through Dental Solutions Staffing Agency will bring a time-slip. That time-slip needs to be completed with the hours worked, signature of the dental professional, and signature of the office manager or dentist. If the dental professional is scheduled to work more than one day within a week, that same time-slip can be used for that week ending on Saturday. The dental professional, dental office, and Dental Solutions Staffing Agency will keep a copy of the time-slip provided. All payrolls expenses are handled through Dental Solutions Staffing Agency and not the office. Payroll expenses include: payroll costs, FICA, Medicare tax, Federal unemployment tax ( FICA ), State u e plo e t ta SUTA, Worker s Co pe satio, a d professio al lia ilit i sura e. If the dental professional is scheduled to work at your office and she gets sent home, you are obligated for 4 hours of pay and a half day agency fee.
