Dominion will submit a check for the reimbursement(s) to the primary subscriber at the end of the plan year. If you participate with FSAFEDS, Dominion may coordinate the reimbursement through your FSA. 4

NEXT STEPS You will receive an email from Dominion in mid-december once Open Season ends. This email will confirm your enrollment, provide a link to your online account and prompt you to select a participating dentist. As a reminder, you must use a participating general dentist to receive care. You may select a dentist online or by calling Dominion s Member Services Department. You will receive ID cards with your plan documents, on average, within 4 business days. 17
