Part 3 The following products will be considered for Pharmacare reimbursement upon an individual prescriber/patient request basis. Codeine Contin codeine monohydrate/sulfate 50 mg 100 mg 150 mg 200 mg Controlled Release PFR A) For treatment of palliative and chronic pain in patients where hepatotoxicity is a concern due to high doses of acetaminophen (e.g. taking over 12 tablets of acetaminophen compound with codeine 30 mg per day) OR B) For treatment of codeine addiction using tapering doses (e.g. acetaminophen compound with codeine addiction). Diamicron MR gliclazide 30 mg SEV - For patients poorly controlled on maximal doses of glyburide, metformin and diet (unless metformin is contraindicated because of renal/hepatic dysfunction or g.i. intolerance). Reminyl galantamine hydrobromide 4 mg 8 mg 12 mg JAN Must have a confirmed diagnosis of Alzheimer Disease based on DSMIV criteria with memory impairment (impaired ability to learn new information or to recall previously learned information); plus at least one of the following: Aphasia; problems with language (receptive and expressive) Apraxia; impaired ability to carry out motor activities despite intact motor function Agnosia; failure of recognition, especially people Disturbance in executive functioning AND 3

The above deficits have caused significant decline in function from previous levels; AND A gradual onset and continued cognitive decline; AND The absence of other causative conditions; AND The deficits do not occur exclusively during the course of delirium. AND Normal test results for all of the following values: CBC, TSH, Electrolytes, Vitamin B 12, Folate and Glucose. AND The initial MMSE score must be between 10 and 26, and measured within 30 days of the application. For continuance of coverage, the patient must have shown improvement or stabilization of symptoms and the MMSE score must be maintained above 10, and measured within 30 days of the application.
