Dear Athletic Health Care Team and Administrators, The athletic trainers and team physicians are here to provide medical services to the student athletes of Lock Haven University. We strive to provide the most complete and up-to-date services possible. In doing so, we attempt to help you field the best team possible and provide you with the best opportunity to achieve success. This Emergency Action Plan (EAP) has been developed to assist the athletic training staff and provide the coaches with a plan of action in case they find themselves faced with a medical emergency. It is our responsibility as athletic trainers to provide medical coverage to provide the best care and follow the appropriate plan of action in case of an emergency. This document provides a guideline to the most appropriate plan of care for the athletes within this institution. There will be times when an athletic trainer will not be present during offseason practice hours. Therefore, it is your responsibility to familiarize yourself with this document and prepare for any foreseeable scenarios. This document will provide you with flexible information that will help you respond to just such a situation. It will also provide information and services provided by the athletic training staff of Lock Haven University that you may want to utilize in the future. The Head Athletic Trainer will distribute this EAP to all members of the athletic health care team at Lock Haven University. It is your responsibility to read, understand and prepare yourself to implement the contents of this EAP. Additional copies will be made readily available and will be found in each of the LHU athletic training facilities. You may also contact any one of the LHU athletic training staff provided in the directory. Under specific orders placed by the administration of LHU, athletic trainers will be working limited hours. This limitation arose from the ever-increasing demand for medical coverage caused primarily by the increased utilization of the non-traditional sport seasons and the continued competition for limited athletic facilities. I have included an outline of the medical coverage limitations for you to explore. I want to thank you for your attention to this matter. Guidelines for Medical Care of LHU Athletes 1. NCAA sports can practice whenever and wherever they feel will most benefit their student athletes and team, while following the rules and regulations. 2. The Athletic Trainers at this university will provide medical coverage of athletics according to the following guidelines: Medical coverage will be provided to all in-season sports. However, this coverage will not be provided before 2:00 PM and will not extend beyond 8:00 PM on weekdays. This coverage will not be provided for before 8:00 AM and will not extend beyond 8:00 PM on weekends, unless worked out prior with the athletic trainer providing coverage. These time constraints apply to all practices unless exceptions are agreed upon by the Head Athletic Trainer and the Athletics Administration prior to the practice date. This time restriction on medical coverage excludes competitions, football, and wrestling because of the inherent risks. Coverage of the out of season practices will remain a responsibility of the assigned athletic trainer. Additional coverage will be offered and readily available if the given sport is practicing in close proximity to an in-season sport with AT coverage. The athletic training staff will work together to make sure out of season sport competitions are being covered. Athletic Training Students will not be used unless under the direct supervision of the certified athletic trainer. 3. The athletic health care team at Lock Haven University is responsible for attending to the medical needs of all NCAA athletes present at any event. Should an emergency arise with anyone other than an athlete, EMS should be activated; however the staff is not responsible for the medical services provided to these individuals. Should it be a dire situation, Good Samaritan law goes into effect, and the medical professional may act on his or her own instincts. 4. All members of each coaching staff will be required to maintain current certifications in CPR, AED, First Aid, ConcussionWise, CardiacWise, and OSHA bloodborne pathogen prevention. All members of each staff will also be required to take courses recognizing heat related illnesses, as part of their continuing education requirements. It is the responsibility of the Athletic Department to monitor the certification status of each member of the coaching staff. 5. Coaches who practice beyond the timeframe of medical coverage, or who are practicing without the direct coverage of an athletic trainer, should follow the policies and procedures outlined by the Lock Haven University Athletic Training Emergency Action Plan. 5

General Operating Procedures Pre-Participation Physical Examination All student athletes who wish to participate for Lock Haven University athletics will be required to obtain a pre-participation physical examination at their own cost. This exam can and should be done in conjunction with the university's admissions exam. The athletics Pre-participation Physical Examination form is incorporated into the University s standard entrance physical form, which is sent to each new student prior to his or her arrival on campus. These forms are also available to the incoming student athlete on the LHU athletics webpage. Each head coach should forward a roster to the athletic training staff prior to the start of their respective season so our records can be checked and a list of those in need of a physical exam can be generated. Please do so in the form of a Microsoft Excel Spreadsheet. New or transfer student athletes should be clearly identified. The athletic training staff and/or team physician reserves the right to refuse a pre-participation physical and require a release by a specialist. This will be evaluated on a case-by-case basis and may result in an additional charge to the student athlete by the physician. Instances where such a refusal may occur include, but are not limited to, the following examples: 1. A student-athlete with a heart condition may require a release by a cardiologist. 2. A student-athlete with an orthopedic problem may require a release by their orthopedic surgeon or our team physician. Returning players will not be required to obtain a pre-participation physical examination prior to each season of competition. The athletic training staff will thoroughly screen all returning players. In doing so, each student-athlete will be asked to update their medical history. As a result of the updated medical history, the athletic training staff again may require the student-athlete to obtain an updated preparticipation physical examination. Most of these updated exams will be orthopedic in nature and will involve our team physician, but could include any specialist such as a cardiologist, neurologist, etc. Team Physician John Bailey, MD is our team physician. He is a board certified orthopedic surgeon whose primary office is located in Williamsport, PA. Our team physician has the ultimate word in deciding whether or not an athlete can participate. We are very proud of the working relationship we have with our team physician. We are confident that we are providing the best care available. Dr. Bailey is available to our student athletes during the school year every Tuesday at the UPMC Susquehanna Medical Plaza (610 High St.) in Lock Haven. Please see one of the certified athletic trainers to arrange for appointment. Dr. Bailey is also on site for all home wrestling and football contests. 6

Dr. Bailey makes every attempt to see our student athletes in his office on very short notice. This becomes an issue when an athlete should not, or cannot wait until the regularly scheduled, weekly clinic to be seen. Please do not take it upon yourself to contact Dr. Bailey on behalf of a student athlete. Please allow the athletic training staff to correspond with his office. Over the years, we have been able to established very effective lines of communication with Dr. Bailey and his staff, and therefore will most likely result in more expeditious results. Graduate Assistants Each year LHU recruits a number of Graduate Assistants to help provide medical care for our student athletes. These students are enrolled in graduate school at Lock Haven University. These individuals are certified and licensed to practice in the state. 7

Transportation It is not the responsibility of the athletic trainers or athletic training students to give or arrange transportation of athletes to and from doctor s appointments, X-rays, etc. The athletic training staff can help to arrange transportation, but are not obligated to do so. There will be times when the athletic training student may accompany an athlete to the doctor's office or hospital. These instances will be left to the discretion of the athletic trainer and the athletic training student and usually involve an educational opportunity for the athletic training student. In those instances, the athletic training student may volunteer to provide transportation for the student athlete. The student athlete should accept such an offer at his or her own risk. In some instances, arrangements for transportation can be made through Glennon Health Services for transportation to the Lock Haven Hospital. Please ask the athletic trainer working with your sport to assist in these arrangements. Directions from Campus Circle to Susquehanna Health: 1. Head east down Campus Drive. 2. Turn right at the light and head south on N Fairview St. towards Bellefonte Ave. 3. Turn right onto Bellefonte Ave, proceed for 0.7mi and the destination is on your left. 4. Susquehanna Health address: 610 High St, Lock Haven, PA 17745. Directions from Campus Circle to Lock Haven Hospital: 1. Head east down Campus Drive. 2. Turn right at the light and head south on N Fairview St. toward Glenn Rd. 3. Turn right onto Glenn Rd. and keep left after the Stop sign. 4. Continue on Glenn Rd. and then turn left onto Irwin St. and proceed for 0.4mi. 5. Turn right onto Cree Dr. for 0.2mi and the hospital is on your left. 6. Lock Haven Hospital Address: 24 Cree Drive, Lock Haven, PA 17745 8

LifeFlight LifeFlight has recommendations for establishing a landing zone for an emergency situation. EMS has the authority and education necessary to request LifeFlight and establish a landing zone. Emergency services will be contacted and be on site before LifeFlight is called in. The landing zone requirements are as followed: 1. Size 100 x 100' (larger if more than one aircraft is requested) 2. Flat surface without debris 3. Free of overhead obstacles (wires, towers etc.) 4. Mark landing zone at each corner using secured cones or fire personnel during the day and strobes at night 5. Landing Zone coordinator should be standing with the wind to his or her back (or mark which direction the wind is blowing) 6. Provide Life Flight with the following patient information: number of patients, adult or pediatric, extricated or still entrapped 7. Provide Life Flight with the following landing zone information: surface (road or field), location in relation to accident scene (Northwest etc.), any overhead hazards such as wires, trees or towers * There is only one established LifeFlight Landing Zone, located on 86 Long Spring Lane, approximately 5.8 miles north of Lock Haven. 9

Communications The predominance of cell phones has made their usage our primary means of communication. Emergency call boxes and stationery phones are easily accessible from most locations. The locations of these call boxes will be outlined in each site specific venue layout. Each landline phone will be checked for both proper incoming and outgoing communications prior to the start of each season and documented, while being serviced when necessary and annually. A posted emergency contact list will be alongside each stationary phone at all venue sites. These contact sheets will include instructions for reaching EMS, site specific directions to give emergency personnel including the venue address, as well as the coaches and staff personal cell phone numbers. The certified athletic training staff has a system of 2-way radios which are used periodically to assist in the coverage of certain sports. This system features 2 channels. The primary channel for use among the athletic training staff is Channel #6. You must make certain that the radios between which you hope to communicate through are set on the same channel. They do not scan both channels. Remember: Use the 9 extension when calling EMS and Campus Safety from a campus phone. When activating EMS from a campus phone, one must dial 9-911. When calling LHU Campus Safety from a campus phone, one must dial 2278. When calling LHU Campus Safety from a cell phone, one must dial 570-484-2278 10

Documentation In the occurrence of an emergency, the responding personnel are required to comprehensively document the emergency procedures they followed. Follow up documentation will be completed on the resulting outcome of the emergency. All personnel present at any point throughout the procedure are required to immediately complete a thorough incident report. Incident reports will be submitted to the head athletic trainer for review. Upon review, the head athletic trainer must scan all documentation into ATS. Documents at this point will be shredded. In some circumstances incident reports may need to be forwarded up the chain of command from the head athletic trainer to the University Administrators. The Emergency Action Plan should be rehearsed annually. When rehearsed the individuals in charge of the review will record the date, time, and procedures followed. Equipment will be checked throughout the emergency action plan review. If found inadequate, faulty equipment will be updated. Those involved will record what was wrong with the equipment, when it was checked, and when it was updated. Any revisions made during rehearsal should be recorded and approved by all personnel involved and the university s administration. Documentation prior to ATS usage is stored in locked cabinets in Himes Rehab Center and Zimmerli 11

General Emergency Procedure With Certified Athletic Training Coverage It is the responsibility and duty of all individuals involved in university athletics to be able to institute emergency procedures in the instance when a student-athlete is injured. When a Lock Haven University varsity athletic team has a scheduled in-season practice or competition, the athletic training staff should see that appropriate emergency equipment is made readily available and accessible. Also, emergency procedures should be implemented and practiced to provide the best possible care for the injured individual. When a student athlete is injured and deemed in need of emergency medical services, the first responder to that athlete is responsible for initiating the following plan of action: 1. The member of the LHU Athletic Training Staff who was designated EMS initiator shall call 911 to initiate Emergency Action Procedures from the nearest available phone. The caller shall inform the Emergency Medical System Operator of the emergency, the circumstances of the emergency, and that an ambulance needs to be dispatched to the appropriate location. The caller shall relay all appropriate information and not disconnect until the Emergency Medical System Operator has all the required information. 2. If a campus police officer is not already present, a member of the LHU Athletic Training Staff who was designated to contact law enforcement shall call Lock Haven University Campus Safety at (570) 484-2278 and request that a campus police officer be dispatched to the site. 3. The EMS initiator shall then inform the on-site personnel that an ambulance has been dispatched and then position themselves to direct the ambulance to the location of the emergency. 4. Game officials and coaches of their respective teams are responsible for clearing participating athletes from the field of play to provide a clear path for personnel involved in caring for the injured athlete. 5. Law enforcement shall be responsible for crowd control allowing all personnel providing care for the injured athlete to do so in a safe environment. 6. The LHU Athletic Training Staff shall then assist the ambulance service staff in preparing the injured person for transport to the appropriate emergency facility. 7. A member of the LHU Athletic Training Staff may accompany the injured person to the medical facility unless there are no other Athletic Training Staff members available. The traveling member of the Athletic Training Staff shall bring the following items: 1. Medical History Questionnaire 2. Insurance Information 12

It is the responsibility of the Athletic Training Staff to see that the injured person is appropriately managed and that the athlete and/or medical staff is/are returned to campus. 1. If appropriate, call our team physician, Dr. Bailey, to initiate appropriate orthopedic procedures. 2. Ask the student athlete if he or she would like an athletic trainer to contact their parent(s) or a guardian. If the request is made, make every effort to contact the person or persons made reference to by the student athlete. 3. Make arrangements for the transportation of the LHU Athletic Training Staff and student athlete from the medical facility back to the field or home. 13

General Emergency Procedure Without Certified Athletic Training Coverage It is the responsibility and duty of all individuals involved in university athletics to be able to institute emergency procedures in the instance when a student-athlete is injured and no athletic trainer is readily available. When a Lock Haven University varsity athletic team practices without the on-site coverage of an athletic trainer, the coaching staff should see that appropriate emergency equipment is made readily available and accessible and that emergency procedures be implemented and practiced to provide the best possible care for an injured individual. When a student athlete is injured and deemed in need of emergency medical services, the following plan of action should be implemented: 1. The nearest coach, or designee, shall call 911 to initiate Emergency Action Procedures from the nearest available phone. The caller shall inform the Emergency Medical System Operator of the emergency, the circumstances of the emergency, and that an ambulance needs to be dispatched to the appropriate location. The caller shall relay all appropriate information, which should include caller's name, location of the injured athlete, the athlete's physical description such as age and gender, nature of the injury as you understand it, and a description of any treatments being administered to the patient. Do not disconnect until the Emergency Medical System Operator has all the required information and instructs you to disconnect. 2. If a campus police officer is not already present, another coach or designee shall then call Lock Haven University Law Enforcement at (570) 484-2278 and request that a campus police officer be dispatched to the site. Please Note: The emergency call boxes will either connect you to the LHU Law Enforcement Dispatcher or the Clinton County Emergency Communication Center depending on the exact time of your call. An ambulance and LHU Law Enforcement Personnel will be summoned to your location regardless of the time of your call.
