For adults with chronic liver disease and a low platelet count before a scheduled medical or dental procedure Get to know DOPTELET This brochure will help you understand why you need to take DOPTELET before your procedure and what you need to do to complete your treatment. DOPTELET is a prescription medicine that has been prescribed to you because you have a low platelet count (thrombocytopenia), in addition to chronic liver disease. Follow the directions you get from your healthcare provider or specialty pharmacy to get the best results and to make sure you re ready on your procedure day. The most common side effects of DOPTELET are fever, stomach (abdominal) pain, nausea, headache, tiredness, swelling of the hands or feet. These are not all the possible side effects of DOPTELET. 2 3

Understanding low platelet count Platelets are blood cells that work together to form clots that help stop bleeding and repair damage. If you bleed, the brain sends a signal to the platelets to start working. The platelets then rush to the site of bleeding and form a clot or plug to repair the damage. If your platelets are low, having more platelets on the day of your procedure may help reduce bleeding. Your healthcare provider prescribed DOPTELET (avatrombopag) tablets for you with this in mind. About DOPTELET Here are some of the most common questions and answers about DOPTELET. If you have any other questions about DOPTELET or your upcoming procedure, speak to your healthcare provider right away. Q: Why do I need DOPTELET? A: DOPTELET can help increase your platelet count before your procedure. Q: How does DOPTELET work? A: DOPTELET helps your body by creating more platelets. It does not interfere with the normal way you make platelets, but instead, adds more to what you already have. Q: What are the alternatives to DOPTELET? A: Up until now, a platelet transfusion has been the most common option for people who needed to raise their platelet count before a procedure. However, with platelet transfusions you need to go to a hospital or hospital-like setting (outpatient center) and it can take a few hours. With DOPTELET, you just take 5 daily doses at home to begin preparing for your procedure. The most common side effects of DOPTELET are fever, stomach (abdominal) pain, nausea, headache, tiredness, swelling of the hands or feet. These are not all the possible side effects of DOPTELET. 4 5

Q: How should I take DOPTELET (avatrombopag)? A: Starting on the right day is important. You should take DOPTELET just as your healthcare provider prescribed. You will begin taking DOPTELET 10-13 days prior to your procedure. Someone from your healthcare team, most likely the specialty pharmacy, will contact you to remind you when you should start taking DOPTELET. Three things you should know about taking DOPTELET: ONCE A DAY Your DOPTELET Welcome Kit contains important information and the tablets you will take once a day. If you have any questions about how or when to take DOPTELET, call your healthcare provider. You can also call your specialty pharmacy, or Dova 1Source at 1-833-DOVA-ONE (1-833-368-2663), and they will help you. WITH FOOD Take DOPTELET every day with food. 5 DAYS FOR 5 DAYS Take DOPTELET for 5 days in a row as directed by your healthcare provider. Other things to remember when taking DOPTELET: If you forget to take your daily dose of DOPTELET, take it as soon as you remember. Take your next daily dose at your usual time the next day. Do not take two daily doses at one time to make up for a missed dose. Complete all 5 doses of medication 6 7

Q: What does DOPTELET (avatrombopag) look like? A: DOPTELET will come in a box with the tablets in a blister pack (shown below). Your healthcare provider will prescribe to you either 40 mg (2 tablets) per day or 60 mg (3 tablets) per day, based on your platelet count. DOPTELET 2 tablets (40 mg) a day for 5 days Q: What happens after my healthcare provider prescribes DOPTELET? Your healthcare provider will send your DOPTELET prescription to Dova 1Source. Dova 1Source reviews your insurance coverage, finds the specialty pharmacy that your insurance company recommends, and determines your appropriate support options. Take your 2 tablets of DOPTELET every day with food. DOPTELET 3 tablets (60 mg) a day for 5 days Dova 1Source will call you to review DOPTELET information, including your procedure date and when you should begin taking DOPTELET. Your specialty pharmacy will contact you to confirm your delivery details before sending DOPTELET to you. After you get DOPTELET, the specialty pharmacy will contact you again to make sure you are taking DOPTELET on the schedule your healthcare provider prescribed. Take your 3 tablets of DOPTELET every day with food. Q: How do I get DOPTELET? A: A specialty pharmacy will deliver DOPTELET right to your home. It can be shipped to another location, if requested. 8 9

Q: What are the possible side effects of DOPTELET (avatrombopag)? A: DOPTELET may cause serious side effects, including blood clots. People with chronic liver disease and people with certain blood clotting conditions may have an increased risk of developing blood clots. Tell your healthcare provider right away if you have signs and symptoms of a blood clot, including swelling, pain or tenderness in your leg; shortness of breath; chest pain; fast heartbeat; or stomach (abdominal) pain or tenderness. The most common side effects of DOPTELET are fever, stomach (abdominal) pain, nausea, headache, tiredness, swelling of the hands or feet. These are not all the possible side effects of DOPTELET. Q: What should I tell my healthcare provider before taking DOPTELET? A: Before you take DOPTELET, tell your healthcare provider about all of your medical conditions, including if you: have ever had a blood clot. are pregnant or plan to become pregnant. DOPTELET may harm your unborn baby. Tell your healthcare provider if you become pregnant or think you may be pregnant during treatment with DOPTELET. are breastfeeding or plan to breastfeed. It is not known if DOPTELET passes into your breast milk. Do not breastfeed during your treatment with DOPTELET and for at least 2 weeks after the last dose. Talk to your healthcare provider about the best way to feed your baby during this time. Tell your healthcare provider about all of the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements. What is DOPTELET (avatrombopag)? DOPTELET is a prescription medicine used to treat low blood platelet counts in adults with long-lasting (chronic) liver disease who are scheduled to have a medical or dental procedure. DOPTELET is not used to make platelet counts normal in adults with chronic liver disease. It is not known if DOPTELET is safe and effective in children. IMPORTANT SAFETY INFORMATION FOR DOPTELET What are the possible side effects of DOPTELET? DOPTELET may cause serious side effects, including blood clots.
