Charles Hayden MD Patients considering TMS Therapy Thank you for your interest in TMS Therapy. Please take time before you meet with Dr. Hayden to carefully fill out this form as completely as possible. It is vital to get an accurate history to determine the appropriate treatment options for your problem. Missing or inaccurate information could result in misdiagnosis or application of the wrong approach to treatment, causing delay in response to therapy or even harm. Please take the time to gather the information about previous treatments and medication trials. Sometimes this information is available from previous providers or from pharmacy records. It would be helpful if you bring any available medical records or lab work (especially from last 18 months) and bottles of medication and supplements you currently take (including herbals and vitamins). Because medical issues can cause emotional symptoms, it will be important to rule out contributing medical disease. For this reason I insist that my patients have a primary care physician to follow them while I am involved with their care. Frequently I also refer to psychologists for psychological testing which can be very helpful in making a diagnosis and treatment plan. After evaluating your history and previous treatments, I hope that I can help you explore the new options for treating depression. TMS therapy may or may not be appropriate, but perhaps there are other approaches you have not considered.
