Step One Grasp the inner leafs of the. Step Two Gently place the mesh into the center of the defect then pull back slightly seating the outer mesh against the posterior wall of the defect. Step Three Allow the inner leafs to expand in the defect. Trim the inner leafs if less mesh is required. Step Four Suture the inner leafs into the defect sides. Step Five An onlay patch is used to reinforce the potentially weakened indirect space. Secure the tails of the patch loosely around the spermatic cord with 1 or 2 tacking sutures.
Another Technique Used: Step One Fold the mesh in half with the inner leafs inside then fold the mesh into a quarter shape. Step Two If needed place a suture through the side of the plug to hold the mesh in place. Insert the plug into the defect. The sides of the inner leafs can be sutured into the defect sides. An onlay patch is placed on the anterior surface of the posterior wall of the inguinal canal; the patch opening is placed around the spermatic cord.
