Potential Complications of Vasectomy Vasectomy offers the most effective, permanent means of surgical contraception. It has one of the lowest incidences of side effects as compared with other contraceptive methods. It is important to note that no deaths have been attributed to vasectomy in developed countries. Selected large-scale studies conducted in 13,500 men show that the overall incidence of complication is less than 5 per 100 vasectomies performed. Minor side effects immediately following vasectomy are some discomfort, swelling, bruising of the scrotal skin. These usually disappear without treatment. Complications can occasionally occur after any surgery. These include hemorrhage and infection. Other possible complications following vasectomy, though les common, include epididymitis and sperm granuloma/ Epididymitis ins an infection of the epididymis, frequently associated with previous history of infection. Sperm granuloma is a chronic inflammatory reaction which is thought to develop as a result of leakage of sperm from the cut ends of the vas. Occasionally, the body fails to reabsorb the granuloma causing it to become painful requiring further treatment. Based upon large- scale studies, the overall incidence of either hematoma (collection of blood), epididymitis ofr infection occurs less than 2% of the vasectomies and sperm granuloma incidence is less than 1 percent. Although it is considered to be virtually 100% effective procedure, vasectomy has been known to fail because, among other reasons, the cut ends of the vas may reconnect. This occurs in less than one percent of all cases. Studies on men after vasectomy have demonstrated there is no change in hormonal levels. It has been shown that there is a proportion of men who produce sperm antibodies following vasectomy. However, current studies indicate that these antibodies do not affect general health and well-being. Vasectomy provides a means of birth control with a minimum, percentage of complications and a maximum percentage of effectiveness and safety. I have read the above information and my questions have been answered to my satisfaction. Patient: Witness: Date:

Cancellation policy for Office Procedures or Surgery Please confirm your availability before scheduling your surgery. We have reserved the Physician s time, the room, and staffing for your procedure date and time. Because we are typically unable to utilize an operating room when a surgery has been cancelled at the last minute, we have a charge structure in place for late cancellations for procedures and surgeries. Policy for in office procedures: Cancelations with less than ten days notice prior to our procedure will result in a $100 charge to the patient. Cancels or no shows on the same day of the procedure will result in a $200 charge to the patient. The patient is responsible to pay this fee and it will not be billed to the insurance company. Policy for hospital or surgical center cancelations: Scheduling for surgery is a labor intensive process which requires careful planning and coordination between our office, the surgical site. If a patient cancels or reschedules their surgery within two weeks of their scheduled surgery date, the patient will receive a charge of $200. I am available on my surgery date and understand I will be responsible for the above charges if I cancel or reschedule my surgery. Name: Date:

Vasectomy Instructions Important* Read carefully prior to your surgery Please shave all hair from the entire scrotum the night before surgery and bathe after shaving. If taking aspirin or blood thinners, please stop one week prior to surgery. YOUR SURGERY WILL BE CANCELLED IF YOU TAKE ANY ASPIRIN OR BLOOD THINNERS ONE WEEK PRIOR TO YOU SURGERY! If you need to be pre-medicated with antibiotics for any type of procedure; please contact you primary care physician to obtain the proper medications. If you surgery is scheduled for a morning, please have a light breakfast. If scheduled for an afternoon, please have a light lunch. Please be sure to bring an athletic supporter or tight underwear to be worn following the surgery. If married, your wife must come with you for the consultation. You may not drive home following surgery, please make arrangements for a ride. Sterility is not accomplished at the time of surgery, but rather after a lapse of time, usually three (3) months after vasectomy. Resume normal sexual intercourse when comfortable, but contraceptives should be used until sterility has been demonstrated after three (3) months. Bring a fresh specimen of semen to our office for examination at two (2) months and again at three (3) months after the vasectomy. You are still considered fertile until we receive two (2) negative specimens. You may shower at any time after the vasectomy. Procedure Education Literature We recommend that you read this handout carefully in order to prepare yourself for the proposed procedure. In doing so, it will improve the outcome and safety of your procedure. If you still have any questions or concerns, we strongly encourage you to contact our office prior to your procedure so we can answer any questions you may have. If you need to be pre-medicated prior to a surgical procedure, please contact your family physician regarding this. If your dentist or physician requires you to take antibiotics prior to any medical procedure, please let us know as soon as possible. Medications that may inhibit your ability to clot blood When undergoing some procedures or operations, it is important that your blood be able to clot. Many over-the-counter and prescription drugs, in one or several ways may prevent your blood from clotting in a timely fashion. The most common of these medications are pain relievers (analgesics) and anti-inflammatory (NSAID) medications. Medications which contain acetaminophen do NOT by themselves affect your ability to clot your blood. Therefore, these medications are generally safe to take in the days preceding surgery provided they do not contain a mixture of any of the compounds listed below. The list below contains some of the more common medications. If you have any questions or if, within the past week, if you have taken any of the below medications, please call prior to your appointment at 517-324-3700. Aspirin of any brand (Common brands Ascriptin, Anacin, Bayer, Bufferin, Doans, Dristan, Ecotrin, Excedrin) Acular Advil Anaprox Aleve Apixaban Baycol Brilinta Celebrex Clopidogrel Clinoril Coumadin- Warfarin Dayrpo Dolobid Effient Eloiquis Etodolac Feldrene Fish Oil Fragmin Glucosamine Ibuprofen Indomethacin Indocin Lovenox Lodine Meloxicam Midol IB Misoprostol Mobic Motrin Nuprin Ocufen Orudis Percodan Persantine Plavix Pradaxa Relafin Synflex Tolectin Toradol Trental Vicoprofen Voltaren Xarelto Your to do list: Review the medication list above very carefully If you take aspirin or any blood thinner, please contact your primary care physician to make sure it is safe for you to stop taking the medication 7 days prior to your appointment then notify us. Your appointment may require additional tests. Call us at 517-324-3700 if you have questions

Post Vasectomy Instructions A little swelling or knot may develop at the site of the surgery. If there is a large amount of swelling of the scrotum, contact the office to discuss. A small amount of bruising is to be expected. If a large amount of bruising and severe pain develops, contact us to see if you need an appointment. On rare occasions, an infection may occur. This may require antibiotics if prescribed by the physician. Swelling and bruising may take up to 4-6 weeks to subside. On rare occasions, a sperm granuloma (reaction of your body to sperm) may occur requiring additional care or removal. It takes an average of two months for sperm to completely clear from fluid. Semen analysis should be done after 50 ejaculations. Please bring semen specimen back at the two months after your procedure and then again six weeks later. You may bathe one week after your procedure. Showers can be taken one day after the procedure. If you have any questions or concerns after your procedure, our phone nurse is available between 8am and 4 pm at 517-324-3701 Urgent assistance after hours 517-324-3700 to get on-call provider.
