Enhanced program analytics identify Metabolic Syndrome and its leading indicators that signify critical risk factors that lead to Metabolic Syndrome, diabetes and coronary artery disease, if left untreated. Metabolic Syndrome and Leading Indicators 2 or more of: Fasting Blood Glucose Hypertension High Triglycerides Low HDL Waist Size 9

ELSM Outreach Criteria Member identified from Health Assessment (HA), Integrated Predictive Model, self-referrals, biometric / worksite wellness events and other BCC programs (Internal Referrals). HA/Biometrics*Tobacco Cessation Trigger: Positive response to tobacco use. Appropriate readiness to change HA / Biometrics *Weight Triggers: Overweight: Body Mass Index (BMI) 25 29.99 Obese: Respondent with a BMI 30 Appropriate readiness to change Integrated Predictive Model Triggers: Obesity, Tobacco Use Disorder, Hyperlipidemia, Hypertension, Pre-Diabetes, or Diagnosis of Metabolic Syndrome Leading Indicators of conditions above (must meet 3 or more triggers for outreach) Experience.
