Getting Started Go to www.ourhealthylives.org and click on the Member Login button located at the top right hand corner. Click the text under Login which reads Not a member? to register. Accept the Healthy Lives Terms of Use and fill in your Company ID information and click Continue. Select your work location from the drop-down list. If you are a spouse of an employee, the location is determined by where your spouse works.
Getting Started Participant ID: Your unique identification provided by your employer. Username: Create your own username. Once you are registered, you will use this to log into the website. It must be between 6 to 16 characters long. Password: Your password must be at least eight (8) characters long. It must contain at least one uppercase letter. It must contain at least one number. It must contain one special character (!, @, #, %, ^, &, *, -). Security Question/Answer: Select a question from the drop-down list and provide an answer to the question selected in the Security Answer box. This information is used to verify you if you need to reset your password.
Let s Take a Tour Homepage You can click on the Healthy Lives logo in the top left corner of the screen to get back to the homepage at any time. Here you can complete your Screening Questionnaire, view your screening results, and reward statements, review program resources, and more.
Fruit, Veggie and Water Trackers Use the trackers to keep log of your daily intake of fruits, vegetables and water. Trackers reset at 12:00 am CST. Move the mouse over fruit/vegetable circles to view similar calorie choices. Click on the fruit/vegetable circle to add 1 cup serving to your Nutrition Log. The number of servings will appear under Servings in the middle. Click on Servings to view your Nutrition Log. Click on + symbol to add 10 ounces of water consumed. Select symbol to remove. Click on the water glass to view your entry in the Nutrition Log.

Complete HRA Questionnaire The first step to get a better understanding of your overall health and wellness is to complete the Health Risk Assessment (HRA) before attending your Healthy Lives screening. All fields marked with an asterisk (*) are required to be answered.
Complete HRA Questionnaire Click on the Enrollment Consent Form underlined below to review the form before checking the box on the left and entering your initials into the box on the right. After you click on Submit, you receive the confirmation below that your questionnaire was submitted successfully.
