Employee Feedback Form The following physiotherapy service questionnaire was handed to those new patients who were seen over a 3 month period (December February 7). There were 15 new patients seen, of which 1 completed the feedback questionnaire. Giving a very good return rate of 93%. 1. Lifting, due to lifts being out of order. Sitting at a computer for long periods. Due to bad set up of workstation this has been resolved. Possibly due to sitting for long periods at desk. 1. Please indicate your level of satisfaction with the service you received from the physiotherapist (mark out of 1, being very dissatisfied, 1 being very satisfied). 1/1 5% 9/1 % /1 % Any other comments: Convenient to receive physio at work I feel if I had been allowed more sessions of treatment, my back pain would have improved further My physio was very good and always explained things well Very useful and informative With suggested improvements to my workstation and exercises given, my problem was much better My physio was very professional, personable and committed to her patients Occ health very hands on and got to the root of the problem quickly The physio couldn t have managed my treatment and phased return to work any better. My sincere thanks to her and the rest of the department I appreciated all the help and advice given The only problem was I wanted more than sessions I felt I need more sessions of physio than I was allowed I felt as though I was pushed into taking physio at the University from my department as I was taking time off work to travel to Sunderland for NHS treatment. In my opinion, I would still be have some sort of treatment it if had kept with the NHS.
