Influenza B continues to exceed influenza A among typed specimens in both the Army and civilian populations. The World Health Organization is monitoring influenza A virus A(H7N9) in China; no human-to-human transmission has been documented. ILI Activity: Army incident ILI outpatient visits in week 14 were at the same level as this time last year. Influenza cases: Three hospitalized influenza cases were reported to USAPHC in week 14. 95 hospitalized cases have been reported during this influenza season, 83 in dependents and 12 in Active Duty. Acute Respiratory Disease at BCT sites: ARD rates remain low; rates did not increase significantly at any sites during week 14. Viral specimens: During week 14, 98 of 617 (16%) laboratory specimens tested were positive for respiratory pathogens. Influenza B accounted for 44% of positive specimens. Most of the influenza positive specimens were from ERMC and PRMC (27% each); 18% were from SRMC. Geographic Spread ILI Activity United States United States During week 14, only 11 states reported widespread or regional activity. 38 states, Washington DC, & Puerto Rico reported local or sporadic activity. Guam & Kansas reported no activity. Global 38 human cases of H7N9 avian influenza have been reported from China (10 fatal). No evidence of ongoing human-to-human transmission has been detected. Live poultry trade has been banned in the regions and wild bird and pig surveillance is being initiated. For week 14, 1.5% of outpatient visits reported through ILINet were due to ILI, which is below the national baseline. Only 2 of 10 regions reported a proportion of outpatient visits for ILI above their specific baselines. The percentage of ILI outpatient visits ranged from 0.7 to 2.1% across regions. ILI Activity Army During week 14, incident ILI activity was at the same level as this time last year and 22% lower than during the 2010-2011 season. Active Duty (AD) Service Members (SMs) accounted for 29% of incident ILI clinic visits during week 14; in 2011-2012, AD SMs comprised 28% of ILI clinic visits on average. Laboratory Respiratory Specimen Testing 16% (98) of respiratory specimens were positive during week 14; influenza B accounted for 44% of positive specimens. Influenza A accounted for 22% of positive specimens, and RSV accounted for 15%. Adenovirus and parainfluenza each accounted for 9% of positive specimens. DRSi Influenza Case Reporting In week 14, three dependents were reported in DRSi for influenzaassociated hospitalization. 95 hospitalized influenza cases have been reported this season for the US Army, 83 (87%) in dependents and 12 (13%) in AD SMs. At the same time last influenza season, 12 AD SMs were reported hospitalized with influenza. Laboratory Testing for Influenza A The proportion of positive influenza A tests in week 15 (5%) remained stable from week 13 (6%). 16 of the 22 (73%) positive influenza A specimens in week 14 were from non-ad personnel. ERMC and PRMC each reported 6 (27%) positive influenza A specimens. 4 (18%) of the 22 positive specimens were from SRMC. NRMC and WRMC each reported 3 (18%) influenza A specimens.
