In order to recognize members of the Technologist Section who have demonstrated leadership and have made a significant contribution to the profession of Nuclear Medicine Technology at the national level, the Technologist Section has instituted a Fellow Member Category. Using the evaluation point scale on the attached form, members may submit an application for Fellow Status. SNMMI-TS APPLICATION FOR FELLOW STATUS 1

Applications may be sent to the Chair of the Membership of the SNMMI-TS with a $100 processing fee. The processing fee is non-refundable. All applicants will be notified of the committee decision. Those members who qualify for Fellow status and are approved by the National Council will be recognized at the Technologist Section Business Meeting at the SNMMI Annual Meeting. Applicants not approved by the committee may resubmit their application up to three years later without paying an additional processing fee. Please note: SNMMI-TS Immediate Past Presidents will automatically receive fellow status at the completion of their term as Immediate Past President. No application is required. instructions It is important to submit appropriate documentation to support the total value of each category. Documentation with respect to titles of lectures and locations, as well as references of publications must be submitted. Inclusion of a resume or curriculum vitae is also required. To be eligible, nominees must be a member of the Technologist Section for (5) continuous years immediately prior to application, must have completed an accredited program and must be currently certified or registered in nuclear medicine technology. There are five major evaluation categories: SNMMI-TS participation in professional activities, education, professional experience, professional contributions and civic activities. Each category has a minimum point requirement The minimum point requirement must be met in each category to be eligible. A total of 100 points is required to achieve Fellow status. Items within each category are assigned a point value as indicated. The applicant identifies the accomplishment and multiplies that by the point value. Example: Item 1A6 - Member (1). The applicant served 3 years. Therefore, 3 X 1 for this line. All accomplishments must have been completed by the time of application in order to be eligible for consideration.
