Art The 2006 Volume 4 Number 1 of articulation a communication and reference tool for today s restorative and esthetic professional NOLA Dry Field System for Implant Surgery by Charles E. English, DDS Little Rock, Arkansas When performing implant placement in the interforaminal symphysis area of the mandible, control of the environment is very important. This minimizes or eliminates salivary contamination of the implant which enhances the probability of successful integration. The author has been employing the NOLA Dry Field System with placement of IMTEC MDI implants for nearly 2 1 2 years with great success. The NOLA system was invented by New Orleans orthodontist Dr. Jack Hickham in the mid-80 s. It was developed for the placement and bonding of orthodontic brackets as it controls tongue restraint, cheek retraction, and salivary evacuation. These are the same requirements needed for MDI placement. NOLA Dry Field System Some patients have very little control of their tongue placement and try to help during the procedure, but end up creating more interference. The tongue holster allows the patient to be more relaxed during the procedure as their tongue is secure. In addition to the evacuation system, the author places a rolled 2 X 2 in the floor of the mouth in front of the tongue guard for additional salivary control. Cheek retraction is very important as it allows more access to the premolar to premolar implant sites. Gagging has not been seen as a problem with this device in my practice. In fact, I think it helps rather than hinders. IMTEC Corp. is now distributing a one-time use NOLA Dry Field System which eliminates any issue ~ continued on inside TONGUE GUARD Holds the tongue in place and allows the patient to continue swallowing. ADJUSTABLE CHEEK RETRACTORS Keeps the cheeks back and fully exposes both dental arches. CENTRAL SUCTION Y connector with low-volume adapter (high-volume also available) LOW- OR HIGH-VOLUME BILATERAL SILICONE TUBING Part of the unique central suction systemthat precisely directs saliva flow and ensures unrestricted evacuation for a totally dry environment. Jim Srader Division Sales Manager Great Lakes Prosthodontics The author of the clinical article in this edition of the Great Lakes Art of Articulation is Dr. Charles English. For those of you who did not know Charlie he was a good friend of mine and a good friend of dentistry. Charlie passed away last November, but prior to his passing, while visiting his home in Little Rock I had asked him to contribute an article for publication in our Newsletter. Dr. English had a great sense of humor and the best mind for recalling clinical articles verbatim that I had ever known. So it is with sadness and pride that I encourage you to read his article. Thanks for your support of Great Lakes Prosthodontics and our quest to bring you the best products and services in the dental profession. SMBP20REV071906
NOLA Dry Field System for Implant Surgery - continued from front of cross contamination or time for cold sterilization techniques. Patients with large tongues or with a tongue thrust tendency can be slightly problematic when using the winged wrench tool for screwing the MDI into place. In that case, I merely use an instrument or an assistant s finger to push the tongue back slightly for ample clearance. All things considered I find this NOLA Dry Field System to be unique and very applicable to techniques requiring isolation in the anterior portion of the mouth. CASE 1: NOLA manages excess salivation and aggressive tongue. CASE 2: NOLA controls saliva, tongue, and lips for flap procedure.
