1 Chapter 1 : Joint United Nations Programme on HIV/AIDS - Wikipedia Title(s): Monitoring and evaluation of the civil society contribution to tackling HIV/AIDS in Namibia, current levels of activity among NGOs, CBOs, and faith based organisations in relation to tackling HIV/AIDS in Namibia. Project Objectives and Components: According to the Financing Agreement p. The project contained three components: Activities under this component were also to include psychological and economic support to infected and affected persons. These activities were to be grouped under two subcomponents: This component was intended to provide support for a unified national and decentralized coordination system and leadership, a uniform national monitoring and evaluation system, and the coordination and management of the Project Administration Unit. Specific activities were to include training and workshops, provision of equipment, support for operating expenses, and recruitment of a financial management agency. The components were not changed at either of two project restructurings. During two restructurings in and, allocation of funds to the components was adjusted. In, the adjustments accounted for higher than anticipated costs for rehabilitation works and equipment and for a depreciation of the dollar. In, the reallocation shifted funds to the purchase of medical equipment, drugs, and reagents with the aim of scaling up activities related to prevention of mother-to-child transmission; to compensate, cuts were made to grants to civil society organizations, with that grant program tightening its focus on high-risk groups and quality of projects over quantity. No Borrower contribution was planned. The Mid-Term Review took place in January of The project was restructured twice: Relevance of Objectives is rated Substantial. Relevance of Design is rated Substantial. Targeting of female sex workers was especially critical, as HIV prevalence levels were times higher among female sex workers than among women in the general population, and several-fold higher among male clients of female sex workers than among men in the general population. However, the PAD does not mention other high-risk groups routinely cited as important along the Abidjan-Lagos corridor border officials, vendors, vulnerable women and girls living along the corridor, truck drivers, and other mobile populations, although it does envisage the mapping of high-risk groups under Component 3. Planned investments for treatment, care, and support were tightly linked to the objective of increasing utilization and coverage of services. Achievement of Objectives Efficacy: According to the ICR p. Much or most of the achieved outcomes related to prevention can therefore plausibly be attributed to the project. Most baseline data are from the ESDG. The ICR does not specify the source for baseline data. Increase and improve the coverage and utilization of prevention services for specific high-risk and vulnerable groups is rated Substantial, based on outcome data related to behavior change among female sex workers, the primary group at high risk for contracting and transmitting HIV infection. Activities included training, workshops, development of communications programs, development of workplace strategies and policies, and provision of furniture and equipment. Staff, equipment, training, and studies were provided in support of development of the Project Administration Unit and a national monitoring and evaluation system. Increase and improve the coverage and utilization of treatment and care services for specific high-risk and vulnerable groups is rated Substantial, based on exceeded numerical targets for provision of antiretroviral therapy and psychosocial and nutritional support for PLWHA. The project deliberately complemented interventions of the Global Fund and other financiers by supporting testing, increased access to services for prevention of mother-to-child transmission, purchase of some antiretrovirals, promotion of testing and treatment for sexually transmitted infections and opportunistic infections, blood safety activities, capacity building for health staff, management of medical waste, nutritional support for PLWHA, strengthening of epidemiological surveillance, and support for traditional medicine. Faith-based and charitable organizations and other community-based groups were financed for subprojects involving antiretroviral treatment and management of opportunistic infections, care for infected children, nutritional support, and psychosocial care and counseling. The project subsidized an unspecified number of existing or new facilities for provision of antiretroviral treatment. PLWHA associations were supported at the departmental and national levels. The project supported OVCs by providing access to a national minimum package of support that had been identified by the Ministry of Family, Women, and Children. The ICR does not provide information on coverage. The ICR does not provide information on coverage; the project team later explained that an assessment of the number of people in need was not done. Care and support was provided to 12, households with orphans and other vulnerable children, surpassing the original and revised target of 2, The ICR does not provide information on the content of this support, or on coverage. The project team later added that the orphans received primarily tuition, school supplies, and school feeding support, and that no assessment of the total number of needy households was done. Efficiency is rated Modest. Line ministries were efficiently prioritized for participation in the project based on their potential effectiveness in reaching the greatest coverage and targeting of vulnerable groups for prevention and care; key line ministries included the Ministries of Education; Youth; Family, Women, and Children; Agriculture; and Defense, and their activities targeted not just their own staff but also their key clients ICR, p. Project administration was carried out by existing institutional entities, rather than creating new ones. A evaluation of the performance of the Fiduciary Management Agency found that the grants to community organizations were managed efficiently. However, there were shortcomings. While the focus of prevention interventions on female sex workers was efficient, other important risk groups appear to have received less or no attention, or little information is provided on activities: Mapping, targeting, and measuring of high-risk behaviors and vulnerabilities on the corridor truck stops, borders, marketplaces, etc. The ICR states that millions of beneficiaries were reached without specifying what reached them and how they were reached, and with what outputs or outcomes. Without information on coverage of whom, and with what, it is difficult to assess the cost-effectiveness of prevention interventions. Page 2

4 Chapter 3 : Guide to HIV/AIDS NGOs in South Africa NGO Pulse The National HIV/AIDS Commission wishes to acknowledge the contributions of the local stakeholders, private individuals and development partners to the preparation of this report. Since its discovery more than twenty year Wednesday 2 May, - 5: The epidemic has disproportionately affected those in poor and underdeveloped regions which has led recent research to repeatedly emphasise the role of underdevelopment and poverty in not only increasing vulnerability to infection but also decreasing the ability of people to develop effective coping strategies to deal with HIV infection. In South Africa an important factor contributing to the spread of the epidemic has been the inability of policy makers to adequately take into account these factors of underdevelopment and inequality in increasing vulnerability. Like most governments across the world, the message from the South African government has largely been focussed around the ABC Abstain, Be faithful, Condomise approach, a narrow view that focuses mainly on preventing infection by emphasising the importance of individual responsibility in remaining safe from infection while also concentrating on the provision of treatment to HIV positive people. What the state has to realise, however, is that through its day to day work it is often directly responsible for either increasing or decreasing vulnerability to HIV infection. In addition, the lack of access to the above resources also negatively affects the ability of HIV infected and affected individuals and households to deal with the effects of the disease. It also does not state how the Department of Health will work together with other national departments to develop a multi-sectoral, integrated approach to development that will address these issues. As the site of service delivery in South Africa, local governments in particular have an important role to play in addressing the developmental backlogs that increase vulnerability amongst the poor and those living on the margins of society. This is because local government is directly responsible for the provision of services like water, sanitation and electricity as well as ensuring access to health care and adequate and sustainable human settlements. Firstly, in recognising the role to be played by all spheres of government, national, provincial and local, the document attempts to do too many things at once. For example, it appears to address too many audiences at once and as a result it is often not clear who its intended target audience is. This can lead to confusion as municipalities might battle to grasp their intended role. This is illustrated by the fact that the document at times appears to be providing guidance to DPLG and possibly provincial departments on how to assist municipalities and their mainstreaming efforts while at other times it appears to be guiding municipalities in their actual mainstreaming efforts. Another shortcoming is that it does not adequately take into consideration the important role that gender plays in South African society. Given the patriarchal nature of South African society, relationships between men and women often tend to be influenced by particular prescribed roles that are exacerbated and more pronounced in impoverished and underdeveloped situations. In addition, when families have to deal with the effects of HIV infection on the household some of the copy strategies for dealing with a possible loss of income in many instances involves young girls being removed from school in order to save money and also for them to take on more responsibility in terms of caring for the sick. Situations like these contribute to the already unequal relationship between men and women and serve to entrench these gender disparities. Thus, it is crucial that the Framework addresses this issue as women are not only biologically more vulnerable to HIV infection but also have to deal with the impacts of poverty, unemployment and gender inequality which makes them the most vulnerable to the epidemic and its far reaching effects. A crucial concern relates to the fact that it is not clear how the DPLG intends to provide the resources and capacity for the roll out of the plan. While the Framework refers to possible partnerships with civil society organisations that do work on local government, if this Framework is to be successfully implemented and if it is to be of use to its target audience, it is essential that the roll out is adequately budgeted for. It is absolutely crucial that initiatives such as these receive the necessary backing and support from senior policy makers. The DPLG Framework in particular is very important as it recognises the need to Page 4
5 move beyond only implementing a health approach to what is clearly an epidemic that is disproportionately affecting the those who are poor and live on the margins of society. The Evaluation Office undertook a strategic evaluation of the role of United Nations Development Programme (UNDP) and its support in addressing HIV/AIDS in 9 countries in Southern Africa: Angola.
