Abstract Background: Body weight change commonly occurs in breast cancer atients who receive adjuvant chemotheray. However, there was lack of information associated with weight loss and hematological arameter change. The urose of this study was to evaluate the body weight and hematological arameter change following adjuvant chemotheray in advanced breast cancer. Methods: This rosective observational study included 50 advanced breast cancer (Stage IIIB and stage IV) subject who were receiving adjuvant chemotheray with Doxorubicin and Cyclohoshamide regiment. Body weight, Body Mass Index (BMI), Body Surface Area (BSA) and hematological arameter were measured before the onset of chemotheray, then after first and second cycles of chemotheray. Results: Based on the study results, the subject body weight was decrease after first and second cycles chemotheray from ± kg to ± and ± kg (<0.05), resectively. The BMI was also decrease from ± 4.10 kg/m 2 to ± 4.12 and ± 4.12 kg/m 2 (<0.05), resectively. Body surface area also decrease from 1.58 ± 0.16 m 2 to 1.58 ± 0.16 and 1.57 ± 0.15 m 2 (<0.05), resectively. Hemoglobin and Total Lymhocyte Count (TLC) level significantly decrease after first and second cycles of chemotheray. Hemoglobin level decrease from ± 1.35 g/dl to ± 1.32 and ± 1.47 g/dl (<0.05), and TLC decrease from ± /mm 3 to ± and ± /mm 3 (<0.05), resectively. There was no significant decrease in leukocyte and Absolute Neutrohil Count (ANC) level after first and second cycles of chemotheray (=0.06 and =0.56). Conclusion: Body weight, BMI and BSA significantly decrease after first and second cycles of chemotheray. Hemoglobin and TLC level also significantly decrease but there is no significant decrease in leukocyte and ANC level. Aroriate nutritional management interventions with follow-u rograms are needed. Keywords: Breast cancer; Adjuvant chemotheray; Body weight; Hematological arameter changes Introduction The majority of breast cancer atients are treated with adjuvant chemotheray often in combination with other systemic treatment [1,2]. Weight gain commonly occurs in western breast cancer atients who receive adjuvant chemotheray [3-6]. Weight gain is also associated with cancer recurrence and oor rognosis [7-10]. Several studies have demonstrated that the majority of breast cancer survivors exhibited a mean body weight gain of 1-6 kg following chemotheray and identified adjuvant chemotheray as an indeendent rognostic factor for weight gain, with a otential longterm effect [11,12]. However, other studies did not reort a significant difference in weight gain between breast cancer atients receiving adjuvant chemotheray and healthy controls [3,13,14]. The causes of weight gain may include reduced hysical activity and menoause rior to diagnosis. However, the exact mechanisms have not been fully elucidated. In addition, the revious observations were focused on atients from the USA and also atients from Western Euroe [3]. There is lack of information associated with body weight decrease in advanced breast cancer atients following adjuvant chemotheray in Indonesian oulation. Information about hematological arameter change in advanced breast cancer atients following adjuvant chemotheray is also very limited. Therefore, we conducted an observational analytical study among women treated with adjuvant chemotheray for advanced breast cancer. The urose of this study was to evaluate the body weight and hematological arameter change following adjuvant chemotheray in advanced breast cancer.

2 Page 2 of 5 Methods Data collection This study was conducted in the eriod of Setember 2016 to March 2017 in 50 atients who newly diagnosed with breast cancer and will get chemotheray at the Hosital in Cirebon, West Java, Indonesia. Diagnosis of breast cancer based on athological anatomy result from atient undergoing mastectomy or biosy. Breast cancer stage is determined based on the results of hysical examination, anatomical athology result and imaging. We erformed body weight and height assessment as well as comlete laboratory examination including hematology arameter before chemotheray. Data on body weight and hematological arameters are followed u during atients undergoing first and second cycle of chemotheray. We collected athological anatomic diagnosis, age, body weight, body height and hematological arameter including hemoglobin, leucocyte, differential count and thrombocyte on adult atient newly diagnosed with advanced breast cancer (stage IIIB and IV) and undergoing chemotheray with doxorubicin and cyclohoshamide regimen. Data analysis Data rocessing using the rogram SPSS version 21.0 for windows. Categorical data is resented with the number/frequency and ercentage whereas the numerical data is resented with the mean, median, standard deviation and range. Numerical data were analyzed using Kolmogorov Smirnov test to determine the data distribution the data were analyzed by using the Reeated Anova test if the data were normally distributed and Friedman's test if the data were not normally distributed. Body mass index, hemoglobin and TLC data are normally distributed so that the Reeated Anova test is erformed while body weight, BSA, latelets, leukocytes and ANC data are not normally distributed. So, Friedman test is erformed. The significance criterion is determining using value. The value is statistically significant if 0.05 and not statistically significant if >0.05. Results A number of 50 atients with advanced breast cancer (stage IIIB and IV) were treated with doxorubicin and cyclohoshamide adjuvant chemotheray in the articiating hositals. The mean age of the study subjects was ± 9.68 years, whereas the mean body weight and BMI of the subjects before the illness were ± kg and ± 4.65, resectively (Table 1). Platelet levels in the grou before chemotheray, after first and second cycle of chemotheray had an average of ± , ± , and ± , resectively. Total lymhocyte count in the grou before chemotheray, after first and second cycle of chemotheray had an average of ± , ± and ± , resectively. Hemoglobin level decrease from ± 1.35 g/dl to ± 1.32 and ± 1.47 g/dl (<0,05), and TLC decrease from ± /mm 3 to ± and ± /mm 3 (<0,05), resectively. The ANC in the grou before chemotheray, after first and second cycle of chemotheray had an average of ± , ± , ± , resectively. Leukocyte level decrease from ± , ± , and ± /mm 3 (=0.062), resectively, while the ANC level also decrease from ± , ± and ± /mm 3 (=0.568), resectively. Base on study result, there is no significant decrease in leukocyte and ANC level after first and second cycles of chemotheray (Table 4). 4 Page 4 of 5 Discussion Studies have demonstrated that adjuvant chemotheray correlates with weight gain in Western breast cancer atients following adjuvant chemotheray [15,16]. In our study, atients with advanced stage breast cancer exhibited significant decrease in body weight (60.06 ± to ± 10.4, <0.05), BSA (1.58 ± 0.16 to 1.57 ± 0.15, <0.05), and BMI (25.66 ± 4.10 to ± 4.12, <0.05) following two cycles of adjuvant chemotheray with doxorubicin and cyclohoshamide regimen. Thus, unlike the dominant weight gain observed in Western oulations, atients in our study have a significant decrease in body weight, BSA and BMI. There are several factors contributing to weight change following adjuvant chemotheray, such as age and weight at diagnosis, menoausal status, chemotheraeutic regimen, recetor status, clinical stage and number of chemotheray cycles. Other factors, such as lifestyle, educational level and economic status were also shown to affect weight gain [17]. We observed that atients age over 40 years were more likely to decrease in body weight comared to those aged less than 40 years. This study also shown that atient s body weight over 60 kg before chemotheray may be associated with the greatest weight loss than atient s age less than 60 kg (Data not shown). The menoausal status was reviously reorted to be a ositive redictor for weight gain in women receiving adjuvant chemotheray [18-20]. Although it was also reorted that menoausal status did not affect body weight [21]. Our study does not analyse the correlation of menoausal status and hormonal recetor status to body weight. However, all subjects in this did not receive hormonal theray because they newly lanned to get chemotheray first and also did not have ovarian ablation. Our study did not analyse correlation between different chemotheray regimen and body weight change because all subject have the same chemotheray regimen (doxorubixin and cyclohoshamide). Previous studies in animal have demonstrated that doxorubicin correlates with neutroenia and thrombocytoenia [22,23]. Metaanalysis study also shown that doxorubicin and cyclohoshamide causing neutroenia and thrombocytoenia [24]. Our study found that there is influence of chemotheray on hematological arameters. Hemoglobin, latelet and TLC levels decreased significantly after chemotheray but no significant decrease in leukocyte and ANC counts. Our study had several limitations, due to it s from a single centre and the relatively small samle size and we involve analyses only of two cycle of chemotheray with doxorubicin and cyclohoshamide regimen. We did not assess additional factors which would otentially affect weight change, including energy intake, exercise, education and sychological status. Further multicentre, long-term and random casecontrol studies are required to further address these issues. However, this study was the first to reort that adjuvant chemotheray in advanced breast cancer may be associated with weight change in Indonesian breast cancer atients. The differences between Asian and Western oulations may be attributed to racial genetic differences, a subject that requires further investigation. Conclusion Body weight, body mass index and body surface area is significantly decreased during chemotheray in advanced breast cancer atients even when given dietary recommendations. Hemoglobin and including TLC level also decrease during chemotheray. There is no significant decrease in leukocyte and ANC level after first and second cycles of chemotheray.
