2 Advocate Condell Medical Center Advocate Condell Medical Center has been providing high quality care to residents in the north suburbs for more than 80 years. Advocate Condell is the largest health care provider and only Level I trauma center in Lake County, Illinois. Advocate Condell is a non-profit, acute care hospital offering a full spectrum of medical services. This includes everything from obstetrics, radiology services and rehabilitation to open heart surgery, neurosurgery and oncology. In addition, Advocate Condell is accredited by DNV Healthcare as a Primary Stroke Center. Advocate Condell offers all private adult patient rooms.

4 Community Needs Assessment Goal/Need Identified Action/Response to Need Language Barriers Language barrier addressed using: Language line through Pacific Interpreters. Video remote interpreting (VRI) and ipad for sign. language and interpretation to assist deaf population. Recently added a spliced line adapter so the physician and patient can speak without some of the difficulties encountered with speaker phone, making it more personal. Transportation Transportation resources expanded to include: ACS Road to Recovery PACE bus First transit and township vans when applicable
5 2015 Condell Cancer Program Outcomes Year End Safety Leapfrog hospital safety score letter grade A since Quality Exceeded health care quality index goals for all measures. Service Cancer Resource Center: more than 2,000 served. Improvements in the patient experience. Growth Radiation Oncology Installed BrainLab technology to provide stereotactic radiosurgery (SRS) to treat small tumors of the brain and stereotactic body radiotherapy (SRBT) for treatment of body tumors. This technology delivers precisely targeted radiation in fewer high-dose treatments than traditional therapy, which can help preserve healthy tissue. Women s Center: Opened new mammography center in Grayslake. Added 3D mammography unit (tomosynthesis). Redecoration of gowned waiting area to improve comfort. Marketing: Stories of the Girls and Breast Month activities. Funding Third annual Advocate Condell Rocks the Cure fundraising event to support Cancer Resource Center free services for patients and families. Coordinated Began initiatives to reduce re-admissions for cancer patients. Implemented use of Care NCI Toxicity Scale to help educate patients and families about chemotherapy side effects and care.

14 Lung Cancer Screening Lung cancer is the leading cause of cancer death in the U.S. The primary reason is that people do not experience symptoms until their cancer has reached an advanced stage. At Advocate Condell Medical Center, we offer a Lung cancer Screening Program to help identify lung cancer early in high-risk patients. Eligibility Criteria: Age years. Asymptomatic (no signs or symptoms of lung cancer). Current smoker or one who has quit smoking with the last 15 years. Tobacco smoking history of at least 30 pack-years (number of packs smoked per day by the number of years smoked. People identified as at risk for lung cancer will undergo annual lowdose lung CT testing and counseling and counseled on the importance of cigarette abstinence or smoking cessation. 15 Breast Health and Community Outreach Breast cancer is the most prevalent cancer in Lake County, Illinois. To address that need, Advocate Condell focused on: Adherence to the National Accreditation Program for Breast Center excellence and quality. Breast health program planning. Breast health community outreach. Breast screenings and marketing with a call to action to get a mammogram with the advantage of same day scheduling and same day results.
16 Breast Health Advocate Condell is recognized as a Breast Center of Excellence and is certified by the NAPBC. Advocate Condell is committed to and focused on enhancing breast health in the community we serve.

Community education: Women, Wellness & Wine & Women s March Madness. Letters mailed to all current patients to introduce 3D mammography and new location in Grayslake. Condell website updated for 3D mammography and Grayslake Mammography Center. April Tomosynthesis (3D mammography) started 4/1/15. April April April April & May April & May April May June October Google Word Ads Search Ad with a separate landing page created for the searches. Grayslake Mammography Center opened 4/23/15. Accepting walk-ins. Momentum, Physician Forum and APP newsletter notice in April and May. Ads regarding 3D mammography and Grayslake facility Tribune, Daily Herald & News-Sun ¼ page ad for three consecutive weeks. Mailer distributed to physician offices and hospital lobbies. Mailer to 9,000 households in NW quadrant to pure prospects (Condell non-users). Video with Dr. Katz highlighting 3D mammography for website, YouTube and internal broadcast. Pamper Yourself Pink breast cancer program with YWCA presented by Dr. Sharpless and Dr. Manquen. Over 90 people attended. Stories of the Girls campaign. Community Pink Day. Participation in ACS Strides walk.
18 Advocate Condell Medical Center & the American Cancer Society The Making Strides Against Breast Cancer of Northeast Suburban event had 108 teams and 1,633 participants who raised more than $183,446. The Advocate Condell Powerful Pink Crusaders raised $3,216 to support the American Cancer Society event.
