1 2015 CANCER PROGRAM ANNUAL REPORT

2 The 2015 Cancer Program accomplishments included: Implementing a BOOT CAMP as a new program to provide cancer patients and survivors an opportunity to incorporate fitness into their daily life. Strengthening our Clinical Trials efforts to accentuate added options when choosing quality of care standard treatments. Designing and performing a Breast Cancer Quality Review Study. This study compared national standard of care guidelines for surgical breast cancer patients with focus on females with early clinical stage disease. The data validated that we are concordant in the care we provide and identified an opportunity to incorporate enhancements into the surgical documentation notes. Designing and performing a Colon Cancer quality review study. This study compared our results against national statistics from the National Cancer Database (NCDB) provided by the Commission on Cancer. We also evaluated the results of a prognostic factor used to determine appropriateness of Genetic referrals (MSI) and made strides in streamlining this process. Additionally, our Genetic volume increased this year and, as a result, we now provide clinic two days per week. We re-designed our chemotherapy form and initiated real-time use of standard of care guidelines in an effort to enhance staff education and communication. Our weekly cancer case conference provides a multidisciplinary approach for individualized, prospective patient treatment for care planning purposes. Mid-year, we migrated to split each weekly conference to breast and general cancer presentations. The multidisciplinary team is composed of physicians that represent Diagnostic Radiology, Pathology, Surgery and both Medical and Radiation Oncology. We continue to review cases for the purpose of prospective real-time treatment planning! Patient supportive services were enhanced and we now evaluate patient psychosocial distress factors via a nationally approved Screening Tool. This tool, in conjunction with education and support services, allows our nurse navigators to effectively guide patients from diagnosis into Survivorship or Palliative Care. In closing, we continually actively reach out into the community. A Community Needs Assessment Plan provides guidance for our outreach programs. We also work with oncology organizations, such as the American Cancer Society and the Cancer Support Center, to address barriers, reduce cancer mortality through early detection and target prevention through education. We conducted a Screening for Prostate Cancer Event and also hosted multiple community events on Prevention and Early Detection for Breast Cancer. Jayant Ginde, M.D., Cancer Committee Chairman
