1 Inquiry KLASSEN, Susanna, 85, Linden On January 21, 2008, Susanna Klassen was admitted to hospital suffering from carbon monoxide toxicity. She developed medical complications and died on January 31, DAVIS, John Frederick, 58, Calgary On November 4, 2006, John Davis was found deceased in his home. The temperature in his home was below freezing as the gas had been disconnected due to failure to make payments. A.O. ** On February 17, 2008, A.O. was admitted to hospital with symptoms of gastroenteritis. Despite medical efforts he died later that day. January 25-29, 2010 BEAUDRY, Vincent Patrick, 32, Edmonton On June 29, 2006, Vincent Beaudry was discovered unresponsive in a police holding cell. February 23, :00 pm Judge P.G. Sully OCCENA, Mary Francis Kathelynn, 9, Calgary On October 18, 2007, Mary Occena was a passenger in a school bus that collided with a parked dump truck. March 22-April 6, 2010 Judge T.C. Semenuk
2 MILAKOVIC, Mark John, 25, St. Albert On July 8, 2005, Mark Milakovic was stopped by police in a stolen car. He was combative and was restrained by police. He collapsed and Emergency Medical Services attended to find him in cardiac arrest. He was transferred to hospital and died July 13, Inquiry March 22-April 1, 2010 April 12-April 16, 2010 Judge L.J. Wenden K.G.** On November 18, 2007, K.G. was found unresponsive pinned underneath an all terrain vehicle. He had been driving the ATV beside a hayfield when it flipped on top of him. April 13-15, 2010 Judge B.R. Hougestol VARLEY, Darren, 26, Pincher Creek On October 3, 1999, Darren Varley was involved in an altercation with RCMP in the cells in Pincher Creek. He sustained two gunshot wounds and was taken to the hospital where he died later that day. April 19-23, 2010 Fort Macleod Provincial Court Judge D.G. Redman
3 PALMER, Winnifred Belle, 88, Calgary On June 19, 2007, Winnifred Palmer fell into a pit that had been dug on her property. On June 21, 2007, she presented at the emergency department with neck pain. Ms. Palmer was admitted and treated for approximately on month, after which she was transferred to a long-term care facility. She died on August 26, Inquiry April 20-22, 2010 Judge J.S. Dunnigan JENSEN, Dr. Michael Anthony, 29, Calgary On January 11, 2007, Michael Jensen went into respiratory distress while in recovery from surgery. He was transferred to the Intensive Care Unit where his condition deteriorated and he died on January 12, AUGER, Jack Ernest, 39, Atikameg On January 2, 2008, 39-year-old Jack Ernest Auger was found unresponsive in the medical ward at Peace River Correctional Centre. The cause of death was determined to be Streptococcus Pyognes (Group A) Septicemia. LATIMER, Sylvie Claudette, 36, Acme On December 27, 2007, Sylvie Latimer was found suspended by her neck in her cell at the Airdrie RCMP detachment. April 29-30, 2010 Judge S.L. June 14-16, 2010 Wetaskiwin Provincial Court Judge B.Rosborough March 8, :00 am Wetaskiwin Provincial Court Judge B.Rosborough D.L. ** On October 24, 2007, one-year old D.L. was found unresponsive with the cord from the window blinds wrapped around his neck. He was transported to hospital where he was pronounced dead. June 16-18, 2010 Judge J.D. Bascom ARGENT, Taylor James, 17, Red Deer On April 2, 2007, Taylor Argent was found unresponsive at a private drug rehabilitation centre and transferred to hospital. 5 Inquiry RAMSEY, Charles Robert, 50, Fairview On April 1, 2007, Charles Ramsey became violent and combative in hospital and was taken into police custody. He was placed in a monitored cell and was later noted to be unresponsive. He was resuscitated and transferred to hospital where his condition deteriorated and he died on April 9, July 14-15, 2010 Grande Prairie Provincial Court Judge W.G. Paul May 13, 2010 Grande Prairie Provincial Court Judge W.G. Paul FILLION, Joleena Dawn, 26, Red Deer On June 6, 2007, Joleena Fillion was found dead in her home. The cause of death was ethanol and zopiclone toxicity. January 13-15, 2010 Judge J.D. Holmes SLOTTKE, Andreas Austin, 41, Spruce Grove On February 10, 2006, 41 yearold Andreas Austin Slottke was found unresponsive in hospital after admission for treatment of pancreatitis.
