1 ABDOMINAL/PELVIC PAIN Overview August 28, Had Chipotle (Steak Burrito, Chips, Soda) - Weight Ate at When went to bed, high uncomfortable pressure feeling in lower abdomen and pelvic region. Pain in abdomen and pelvis August 29, Went to school (6 th grade/new Student Orientation am; Open House pm) - After 6/NSO went to lunch at Old Country Buffet with co-worker. - When back at the Open House, pressure felt again intensily. Called Tysons Corner Urgent Care (for post-5 meeting) - Went to Tysons, saw Dr. Chaudary was given an Abdominal X-Ray, Blood work taken - X-Ray came back normal was told that I had massive constipation as there was a lot of bile and gas built up - Was told to take Metamucil, Stool Softeners, and Gas Relief (ex. Gas-X) - Was told to go on a bland diet - Had chef salad, apricots, propel (electrolyte replacement water) - Weight 324 August 30, Didn t feel like eating much had a bag of pull Twizzlers, and 2 diet Peach Snapple - Took 2 stool softeners, and 2 gas-x pills - Abdominal pressure and pelvic pressure still very present August 31, Had 2 stool softeners, a gas-x pill - Ate 2 chicken breasts fried - Didn t feel like eating much September 1, Had a stool softener pill - Didn t feel like eating much - Went to Parents house weighed self was at Immediately called Tysons and went in that evening

2 - Dr. Chaudary saw me again, ordered a CT Scan & bloodwork - CT Scan taken, came back normal. - Weight taken officially at Told again to go on Metamucil, Stool Softeners, and Gas-X September 2, Finally had a bowel movement was soft, but nonetheless - Was light brown in color - Had oatmeal - Later in the day stool turned more pale yellowish and had spots of blood on them - Had slight back pain - Had awful pain in the groin (stabbing/knife like) - Bursitis in foot popped (very painful) - Went back to Tyson s UC saw Dr. Walu - Was told the blood was to keep an eye on - Was told the pale color was because of the CT Scan dye - Weight at 317 September 3, Awaken at 2:30 by intense pain in the groin area - Called Tyson s UC Went in at 4 am - Called School (First day of Classes) told them I may or may not be in - Saw doctor. Had Rectal Exam. Doctor mentioned no rectal bleeding which is a good thing - Abdominal check performed - Back in pain - Went to school sub was also there - Stayed for the 1 st 2 classes. Started feeling a knot feeling in left lower abdomen - Left and went and saw PCP Dr. Saleh in Burke - Asked Dr. Saleh for a referral to a GI (gastroenterologist) - Was checked out for symptoms was told it most likely is IBS or Crohn s disease - Weight was at Temperature at Felt sick that night. Bad cough, runny nose, clogged throat September 4, Went into see Dr. James Liu (GI- Falls Church) - Reviewed blood work

3 - Suggested that I need not have a colonoscopy, but if anything, a sigmonoscopy - Insisted I get full colonoscopy - Scheduled one for November 8, 2013 September 5, Went into school - Had a very stiff neck on the right side of my neck - In first class, pinky and ring finger on right arm were numb. Right arm was in pain. Right foot went numb. As I went to the second class, my right jaw became numb then soon after my right face. - Bad pain in right elbow - Left after 2 nd class went to doctors saw in Burke - Followed up another meeting in Tyson s. Same symptoms - Foot went numb driving home - Weight at 313 September 6, Went in to school - Was mostly pain free September 7, Pain in abdomen started becoming greater (lower left, lower right, middle (above navel, below breastbone), September 8, 2013 September 9, 2013 September 10, 2013 September 11, 2013 September 12, 2013 September 13, 2013

4 - Blood work done at Tyson s. - ALT rose from 46/50 to 72/50. (Had been taking cough drops consistently for the previous 3/4 days) - Had a X-Ray on the back. No issues told just a muscle strain. September 14, 2013 September 15, 2013 September 16, 2013 September 17, 2013 September 18, 2013 September 19, Saw Dr. Saleh in Burke - Had new bloodwork done September 20, 2013 September 21, 2013 September 22, 2013 September 23, 2013

5 September 24, 2013 September 25, 2013 September 26, Went to Woodbridge Tyson s after severe abdominal pain in lower left area. - Called Advice nurse and asked how to resolve it. Told me to lay down. That aggravated back. Could barely move. Had parents drive me to Woodbridge September 27, Back pain (between shoulder blades (stiff feeling as well), mid area, lower back pain) September 28, Flaky yellowish stools - Burning sensation when urinating - Back pain continued (between shoulder blades (stiff feeling as well), mid area, lower back pain) - September 29, Dry forehead - Stool long and pencil thin - Slight pain in mid abdomen (above Navel (~3 above)) - Back pain continued (between shoulder blades (stiff feeling as well), mid area, lower back pain) - Weight at 300
