1 UNICEF/2015/South Sudan/ Modolo South Sudan Humanitarian Situation Report 27 FEBRUARY 12 MARCH 2015: SOUTH SUDAN SITREP #54 SITUATION IN NUMBERS Highlights Since its launch end March 2014, UNICEF has completed 40 Rapid Response Missions (RRMs), reaching a total of 705,000 people including 144,000 children under five. In 2015, the RRM remains a critical response model to provide humanitarian assistance in hard to reach areas. Since the start of the year, six RRM missions have been completed in northern Jonglei and northern Unity States, reaching over 102,000 people, including 17,000 children under five. Two missions are currently underway in Kuach and Nimni in Guit County, Unity State. UNICEF continues to support the release and reintegration of children formerly associated with the Cobra Faction. Following a release in Fertet on 6 March, the total number of released boys is now 660. Two new local partnerships have been established to undertake community-based monitoring, including monitoring of possible re-recruitment. UNICEF is continuing its advocacy on the recruitment of children. While some of the forcibly recruited children in Wau Shilluk are now returning home for meals and to sleep, they continue to be required to attend military training during the day. There has been no formal release of children. UNICEF commissioned a water system in Nabweliangakine, Kapoeta East County providing 3,000 people with safe water. Kapoeta East is in the epicentre of the remaining Guinea Worm cases in South Sudan. 2 Situation Overview & Humanitarian Needs Since the beginning of the conflict in December 2013, two million people have fled their homes in South Sudan, including 1.5 million people who remain displaced inside South Sudan and 506,938 who have fled to neighbouring countries. According to the UN Mission in South Sudan, 112,840 civilians are sheltering in Protection of Civilian (PoC) sites. The security situation remains unstable and both SPLA and SPLA-iO continue to maintain their fighting posture mainly in Unity and Upper Nile states with reports of several ongoing skirmishes. The SPLA has amassed forces in the areas of Kaka and Melut in Upper Nile State and also in Bentiu, Rubkona and Mayom in Unity State. A major offensive is expected in Unity. In Western Equatoria, Lord s Resistance Army presence was reported in Yambio County and an attack launched against civilians with one person killed and six people abducted. Reports of mobilization of forces and military resources across the country also continue to be received, in both government and opposition-controlled areas. The IGAD sponsored peace talks collapsed with the passing of the 5 March deadline in Addis Ababa; the likelihood of an escalation of the conflict is high. It is assessed that the UN Security Council s unanimous decision to impose targeted sanctions could result in anti-un sentiments and hostile reprisal actions against the UN. Humanitarian leadership and coordination UNICEF actively participates in the Humanitarian Country Team and the Inter Cluster Working Group, which lead strategic and cross-sectoral coordination of humanitarian programmes. UNICEF leads the WASH, Nutrition and Education clusters as well as the Child Protection sub-cluster. Within the Health cluster, UNICEF provides leadership on vaccination, communication and social mobilization. UNICEF also supports the core supply pipelines for the Education, WASH and Nutrition clusters, providing essential humanitarian supplies to all partners. While continuing to focus on the delivery of life-saving interventions, UNICEF is also investing in providing access to education and a protective environment for affected children. Humanitarian Strategy: Rapid Response Mechanism (RRM) As part of the interagency rapid response teams, UNICEF continues to expand activities in remote locations together with WFP and partners. To date, UNICEF and partners have undertaken 40 RRM missions, reaching a total of 705,000 people including 144,000 children under five. In 2015, the RRM continues to remain a critical response model to provide humanitarian assistance in hard to reach areas. Counties with very critical nutrition phases are targeted as a priority. Since the start of the year, six RRM missions have been completed in northern Jonglei and northern Unity States, reaching over 102,000 people, including 17,000 children under five. Two missions are currently underway in Kuach and Nimni in Guit County, Unity State. In Jazeera, Rubkona County in Unity State, UNICEF reached 25,339 people with life-saving interventions. This was the first time multi-sector humanitarian assistance was provided to this location since the start of the conflict. A total of 5,269 children under 5 were screened for malnutrition, with 40 cases (0.8 per cent) of severe acute malnutrition (SAM) and 153 cases (2.9 per cent) of moderate acute malnutrition (MAM) registered and treated. 4,565 children were provided with Vitamin A and 3,748 children dewormed. In addition, 11,341 children under 15 were vaccinated against measles and 12,241 against polio, while 5,233 women were vaccinated against tetanus. Long-lasting insecticide-treated nets (LLITNs) were provided to 3,680 households. A child-friendly space was established in Jazeera and 27 separated, 14 unaccompanied and 8 missing children were identified and registered for family reunification and tracing. In Nyanapol, Ayod County in Jonglei State, UNICEF recently deployed a mobile team with the aim to address the reported critical nutrition situation. A total of 1,146 children under 5 were screened for malnutrition, with 54 cases (4.7 per cent) of SAM and 308 cases (26.9 per cent) of MAM registered and treated. A semi-permanent nutrition outpatient treatment centre facility was set up which will also be used as a temporary health facility. 1,021 children were provided with Vitamin A and 888 children dewormed. In addition, 3,435 children under 15 were vaccinated against measles and 3,713 against polio, while 1,562 women were vaccinated against tetanus. Hygiene promotion messages reached 3,302 people, to be shared with their community. A child-friendly space was established in Nyanapol and 21 unaccompanied and separated children, as well as 44 missing children were identified and registered for family reunification and tracing. 2

3 Summary Analysis of Programme Response HEALTH: No further cholera cases have been reported in South Sudan during the reporting period. UNICEF s partner in Ikotos, AVSI, has continued to operate a cholera treatment unit and undertake community mobilization to increase awareness on improved hygiene practices and early health seeking behaviour. Health partners remain vigilant for any new cases in nearby counties. UNICEF continued its support to health implementing partners in the PoCs in Bentiu, Malakal and Juba as well as in IDP sites in Wau Shilluk and Mingkaman, including supplies, technical assistance and operating costs. During the reporting period, 1,940 consultations were provided in Bentiu PoC, with acute respiratory infection remaining the leading cause of consultations. The Ministry of Health has approved MSF-H to provide pneumococcal vaccines in the PoC as a strategy to decrease the burden of pneumonia among infants. Preparations for UNICEF to import the vaccines are ongoing. Vaccinations will commence once the vaccines are in the country. UNICEF supports the prevention of mother to child transmission of HIV in Mingkaman, Bor, Bentiu and Nimule Hospital, with 351 pregnant women who accepted testing services counselled during the reporting period. The women who tested HIV-positive were referred for treatment in Bor and Bentiu. So far this year, 3,658 pregnant women have been counselled and tested for HIV. In response to the circulating vaccine derived polio virus (cvdpv2) outbreak confirmed in October 2014, short interval additional dose (SIAD) campaigns are on-going targeting children under 15 in the three conflict affected states. As per the cvdpv2 Outbreak Response Plan, three rounds are being implemented in a staggered manner depending on access, partner presence, processing of funds and availability of air assets for vaccine distribution. Round one started 2 December 2014 and so far 844,483 children have been reached in 27 of 32 counties. The second round is also on-going, with 421,276 children vaccinated to date in 14 counties, whereas the third round has been completed in 9 counties reaching 297,455 children. All Protection of Civilian (PoC) have completed three rounds. In addition to provision of vaccines, logistics and cold chain, UNICEF together with Ministry of Health and partners is undertaking community and household social mobilization activities. The first round of Polio National Immunization Days (NIDs) for 2015 took place February in seven states and will be implemented in staggered manner in accessible areas of the three conflict affected states targeting a total of 3.3 million children under five years with polio vaccination. Data for the seven states is being collected and compiled by WHO. In Jonglei, the integrated vaccination campaign was completed in Akobo East where 41,521 children were reached with measles vaccination and polio data is being compiled (target 79,993). Akobo West has also completed the campaign targeting 28,000 children and data is being compiled. In Pibor County the recently concluded integrated campaign targeted 48,692 children with measles, 50,764 children with polio, 19,684 children with vitamin A and 17,612 children with deworming and results are being compiled. NUTRITION: So far in 2015, UNICEF and partners have admitted 9,548 children Trend of SAM Admissions in Bentiu by Week with SAM to therapeutic feeding programmes. The high number of SAM admissions can be attributed mainly to the scale up in nutrition services and the close follow-up of partner reporting. The number of Outpatient Therapeutic 50 Programmes (OTP) sites have increased from 252 in 2012 to 470 in Admission 0 to treatment for malnutrition in Bentiu PoC 1 has had a slight spike in this reporting period, which is assumed to be due to the high screening numbers of children in the previous reporting period. 3

4 During the reporting period, 14,300 children were screened for malnutrition in the conflict-affected states. In Upper Nile the proxy SAM and proxy global acute malnutrition (GAM) rates were 0.4 per cent and 2.9 per cent, though this is not reflective of the overall nutrition situation due to the low number of children screened. In Unity, GAM and SAM rates were 10 per cent and 3 per cent, also based on too few screenings to be representative. In Jonglei, the proxy GAM and SAM remain high at 25.8 per cent and 6.4 per cent respectively Proxy GAM and SAM from MUAC screening in Jonglei Akobo Ayod Old Fangak Pibor Nyirol Pigi/West Uror Combined Akobo SAM Proxy GAM GAM rates remain high in Akobo, Ayod, Old Fangak and Pibor. SMART surveys are planned in Pibor and Ayod while surveys in Akobo West and Nyirol have just been concluded and are pending validation. Partners are preparing to implement the recommendations from the coverage surveys in Panyijar and Akobo. This includes improving outreach services through recruitment of skilled community volunteers to improve early detection and identification of acute malnutrition. Community sensitization to address the effects of rejection of OTPs, the use of traditional healers and misconceptions about healthy infant and young child feeding (IYCF) practices are also considered by partners. An increase in admissions was reported by MSF in Leer in February. While the data is yet to be complied, about 900 children were admitted to the nutrition programme. However, the data currently does not distinguish moderate and severe malnutrition. This makes it difficult to conclusively state there is an increase in SAM. The increase is mainly attributed to the movement of IDPs to Leer County from neighboring counties seeking services, including the monthly general food distribution by ICRC. MSF-H reported that 20 per Cases Acute Watery Diarrhoea, Meyandit County in under5 above5 total Epidemiological weeks 2015 cent of the children were coming from Meyandit. The scale up of nutrition services in Meyandit has been delayed due to partner capacity, starting only in March 2015, which might have contributed to population movement to Leer. The high reported number of acute watery diarrhoea cases could also contribute to the increase of admissions in February. Health teams on ground are closely monitoring this trend. To date, 81,995 children under five have been screened for malnutrition by social mobilizers in Juba with 867 (1.1 per cent) identified with MAM and 245 (0.3 per cent) with SAM who were referred for treatment. Key messages on IYCF and hygiene have been reached 59,995 households. In Northern Bahr el Ghazal, a further 102,333 households have been reached with key messages while 135,483 children under five years have been screened. A total of 4,168 (4.1 per cent) and 10,040 (9.8 per cent) were found to be severe and moderately malnourished respectively. This gives a proxy GAM rate of 13.9 per cent, in line with recent nutrition surveys in the area. Food Security and Nutrition Monitoring System (FSNMS) state level training started on 10 March and will continue until 14 March with field work to start from 16 March. Surveys for Aweil West, Aweil North and Juba County, were validated during the reporting period. The GAM rates for Aweil North and Aweil West were 14.3 per cent and 13.9 per cent respectively. In Juba the GAM rate is 6.4 per cent. The overall nutrition situation in most parts of the country remains above emergency levels (GAM above 15 per cent); these areas include Upper Nile, Unity, Northern Bahr Ghazal, Jonglei and Warrap. The prevalence of malnutrition is expected to remain above emergency thresholds in most parts of the

Safe water supply, basic sanitation and hygiene continue to be provided for IDPs in Bentiu, Malakal, Bor and Wau PoCs and Mingkaman IDP site. UNICEF and WASH Cluster partners are ensuring well-functioning WASH facilities through continued monitoring and maintenance. In Mingkaman, UNICEF is providing 46 per cent of the WASH needs for the IDPs while the rest are provided by other agencies. WASH services are also being expanded in host communities accommodating IDPs. During the reporting period, 8,500 people in Wau Shilluk (3,500 people), Maban (2,500 people), and Maiwut (2,500 people) in Upper Nile State as well as 339 people in Eastern Bank (Jur County, Wau, Western Bahr el Ghazal) were reached with WASH services. Since the beginning of 2015, UNICEF has reached over 60,000 people with safe water supplies and hygiene promotion messages and over 6,000 people with basic sanitation facilities outside the PoCs. In Jonglei, the rehabilitation of two boreholes in Akobo benefitted over 1,770 people. A 10,000L water bladder system with 12 taps has been established Lekuangole Interim Care Centre ahead of the demobilization exercise expected to start this week. Construction of two latrines have also been completed, while four more will be completed by the end of the week. Borehole drilling has commenced in Gumuruk and is facing challenges as drilling to a depth of 85m has still not yielded any water. To address this, a geo-hydrological survey assessment is ongoing to optimize successful drilling in the areas. The availability of borehole water will reduce dependence on water trucking services and assure the communities with a steady supply of safe water. 5

6 UNICEF/2015/South Sudan/Wechrial Repaired borehole being handed over to Commissioner and Minister of Education Latrine construction is also ongoing in Gumuruk town. UNICEF has supplied the community with 100 latrine slabs to construct their own latrines as part of a community participation project to increase ownership of these facilities to improve their sustainability. UNICEF also supplied 1,500 households in Gumuruk with household water treatment supplies and distributed 200 buckets to vulnerable families in the community. To support these activities, 25 Hygiene Promoters have been trained and deployed to extend WASH service coverage to payams outside Gumuruk town. In the coming weeks, the Hygiene Promoters will start distribution of WASH NFIs for 10,500 people in areas surrounding Gumuruk town. With the early rains, the Greater Pibor Administration Area is expected to once again become inaccessible by road from Juba. Thus completing the establishment of WASH facilities such as drilling boreholes and constructing latrines, as well as prepositioning of WASH supplies, is critical in the remaining months of the dry season. The cholera outbreak in Ikotos, Eastern Equatoria State is waning, partly as a result of the collaborative efforts between UNICEF and its WASH Partners. WASH supplies including soap and water treatment products were distributed in Ikotos to ensure safe water supply and good hygiene at the household level. In addition, Community Home Health Promoters and Hygiene Promoters continue to carry out promotional sanitation and hygiene awareness campaigns. Community and youth leaders have gave committed promote household latrines construction in order to ward off cholera. Last week a water system was commissioned in Nabweliangakine, Kauto Payam, Kapoeta East County serving 3,000 people to combat Guinea Worm. A similar motorized system was also completed in Mogos Payam that is already serving 3,000 people. From 2006 to 2014, UNICEF and partners reached 454,000 people with access to safe drinking water through the drilling of 256 new boreholes; the rehabilitation of 367 existing water points; and the motorization of 5 water points in Guinea Worm affected areas. In 2015, UNICEF will provide 55 boreholes towards the final push to eradicate Guinea Worm from South Sudan by the end of Acute watery diarrhoea has been reported in the settlements in Aweil North, Northern Bahr el Ghazal. UNICEF and WASH partners have provided supplies from the WASH cluster core pipeline supplies including buckets, soap, water treatment and hygiene kits for over 2,000 people. EDUCATION IN EMERGENCIES: Following the national level launch, Back to Learning (BTL) implementation is taking place at state level. During the reporting period, a launch took place in Jonglei state, where 50,000 children are being targeted under BTL. A total of 4,075 children (41 per cent female) newly gained to access education with the support of UNICEF and partners. This brings the total number of children accessing learning opportunities to 23,431 since the start of UNICEF and partners rehabilitated 14 classrooms and trained 37 teachers and other education personnel to support EiE services, including provision of psychosocial support and life skills education. UNICEF continues accelerating its education assistance to the Greater Pibor Administrative Area and to date, 1,703 students (14 per cent female) are enrolled and studying in four schools. In addition, 1,376 students (4 per cent female) are registered at the Lekuangole Primary school which re-opened on 9 March. The lack of trained teachers remain a challenge to accelerate education in GPAA. UNICEF and partners are expanding education in GPAA by establishing learning spaces, providing education supplies and training teachers. During the reporting period 26 teachers out of a targeted 150 teachers were trained by Windle Trust International. An Accelerated Learning Programme is underway for 591 children who have missed out on formal education. Through UNICEF advocacy, Gumruk school which was previously occupied by armed forces, has been vacated. As a result of the mobile peace conferences held in various communities in Tonj East, Warrap, Community Peace forums have continued to hold regular meetings. In a region where inter-communal clashes are cyclical, these local initiatives are having positive impact. In the reporting period, 40 people in Anantak Payam met to reconcile disagreements. With the mediation of previously trained Parent-Teacher Associations and Peace Forum members, relatives from the rival clans settled their disagreement peacefully and agreed on a plan to prevent such conflicts in the future. In the same 6

7 area, 45 community members conducted a community peace forum to settle a cattle raid dispute. Since the start of the mobile peace forums in July 2014, community members have reported a significant decrease in inter-communal conflict. So far, 14 community Peace Forums have been held in Tonj East. CHILD PROTECTION: UNICEF continues to support the release and reintegration of children formerly associated with the Cobra Faction in GPAA. Following a release of children in Fertet on 6 March, the total number of released boys is now 660. The next release is scheduled for this week in Lukuangole, with an Interim Care Centre recently finished in the area. Released boys continue to receive interim care, psychosocial support and family tracing services. Two new local partnerships have been established to undertake community-based monitoring, including monitoring of possible re-recruitment. UNICEF continues to keep a regular team of specialists on the ground to support the multisectoral response to the needs of these children and their communities. UNICEF is continuing to maintain its advocacy on the recruitment of children and has undertaken a joint monitoring visit to Wau Shiluk with UNMISS and the SPLA Child Protection Unit. While some of the forcibly recruited children are now returning home for meals and to sleep, they continue to be required to attend military training during the day. There has been no formal release of children and communities remain fearful to discuss the issue openly, which makes it difficult to ascertain how many children remain missing. UNICEF is also investigating further reports of forced recruitment in Unity State. In 2015, the number of new children reached with child protection services is 38,301 and the number of children registered unaccompanied, separated or missing children in the national database remains at 7,696, of which 1,121 are unaccompanied (40 per cent girls), 4,274 are separated (49 per cent girls) and 2,303 are reported as missing (56 per cent girls) and 320 cases have been closed. The Standard Operating Procedures for Family Tracing have been reviewed and a final version is expected to be available to inform future tracing processes by the end of next week. This updated SOP outlines how the RapidFTR processes are integrated into the FTR system and details the accountabilities and standards that participating FTR agencies commit to maintaining. Twelve reunifications were undertaken in this period, bringing the total to 857, however, this process is becoming increasingly difficult in some areas where local authorities are preventing the movement of children, requiring frequent negotiation by UNICEF and partners to facilitate the travel of individual children. Since the beginning of the year, an additional 13,041 people (65 percent of whom were children) have been reached with mine risk education by accredited partners in three high risk states. UNICEF, through its partners, continues to widely promote information sharing on available GBV response services and provide medical and psychosocial support to survivors of GBV. During the reporting period, approximately 1,442 women, 500 men, 407 girls and 400 boys were reached with prevention services, including women's groups and weekly women s committee meetings where women share information with service providers and discuss issues of safety and other concerns. Outreach activities continue, including house to house information sharing on referral options, and regular dialogue sessions with different groups of men, women, male youth, female youth, elders and religious leaders. Other responses include awareness creation on GBV issues in churches, schools and other community forums and a regular talk show on local radio. UNICEF, together with camp management, has also established a system for regular maintenance of the solar lighting in the Malakal PoC. In some areas, communities had also directly intervened to prevent the theft of solar light batteries and these boxes are now being reinforced to deter future theft. UNICEF is also continuing to support training and capacity building of new partners, which once completed, will enable these partners to contribute to scaling up GBV responses in new areas. COMMUNICATION FOR DEVELOPMENT (C4D): Mobilization is underway for the Polio National Immunization Days scheduled to take place from March 2015 through advocacy events, radio messaging, visibility materials, community dialogue and capacity building of social mobilizers. Over 12,000 social mobilizers will be conducting the campaign in seven states and accessible counties of conflict-affected states. Social mobilization continues for the Back-to-Learning initiative. To date, three launches have been conducted, one at national level and two state level launches in Unity State and Jonglei. This initiative will continue over a period of one year with a national radio campaign in eight local languages, advocacy meetings with political leaders, PTA members and religious leaders, community dialogue with families and communities. In Bentiu, under the integrated humanitarian approach, 131 community volunteers mobilized caregivers and families to adopt essential family care practices such as seeking early healthcare. Over the last two weeks, 12,249 households 7

8 have been reached including 1,791 children under five who were identified and referred including 857 cases of diarrhoea, 13 suspected Measles cases and 319 children with malnutrition, along with 287 pregnant women who were referred for ANC services. SUPPLY & LOGISTICS: UNICEF is continuing to use the dry season to preposition essential supplies for use during the oncoming rainy season. During the reporting period, 4,864 cartons of ready-to-use therapeutic food (RUTF) were delivered to Juba, and were immediately cross-loaded on to locally hired trucks for delivery to UNICEF warehouses in Wau and Rumbek for pre-positioning. Supplies were also transported to support the on-going reintegration efforts in Pibor, including three trucks which transported the following cargo from Juba to Pibor: children s clothes, a generator, one motorbike, six health tents, two diarrheal disease kits and one primary health care unit kit. A charter plane made two flight rotations from Juba to Malakal to deliver Ministry of Education examination documents, education campaign materials and exercise books for the Back to Learning campaign.
