14 Strategies to Prevent errors If reconstitution of vaccines is necessary, reconstitute ONLY with diluents supplied for specific vaccine. DO NOT MIX diluents
15 Strategies to Prevent errors Do not store sound-alike and look-alike vaccine close to each other. Check the vaccine vials 3 times before you administer it. Store pediatric and adult vaccines on separate shelves
16 Strategies to Prevent errors Labeling the area where vaccines are stored can help staff quickly locate and choose the correct vaccine. Labels can be placed on the containers or bins or directly attached to the shelves Color coding the labels(one color for pediatric and another for adult vaccines)
17 Reporting Vaccination Errors Error reporting should provide opportunities to discover how errors occur and to share ideas to prevent or reduce those errors in the future. Follow clinic policy for reporting medication errors Our Mission: To protect and improve the health and environment of all Kansans.
