Back to the Basics- Immunization 101

Objectives Increase knowledge of vaccination administration techniques. Identify at least 2 strategies to prevent vaccination administration errors Explain how to report vaccination administration errors

Proper Administration Proper vaccine administration is a critical component of a successful immunization program Key part of ensuring that vaccinations are safe and effective

Vaccination Preparation Remember the 5 R s Right Patient Right Vaccine and diluent Right Dose Right Route Right Time Interval

Immunization Techniques Best Practices Video Order Staff should receive comprehensive, competency-based training Keep current reference materials available for staff

Question 1 MMR and Varicella are both given subcutaneously True or False

Question 2 Proper needle length for an SC injections in a child or adult is 5/8 in. T or F

Question 3 The vastus lateralis (lateral thigh) is the muscle of choice for administering an IM injection to a child less than 12 months of age. T or F

Question 4 IM injections should be inserted at an 80 to 90 angle to the skin. T or F

Common vaccine administration errors TDAP instead of DTAP Doses administered too early(before the minimum age or interval has been met) Pediatric formulation given to an adult Administered expired vaccine Wrong diluent used to reconstitute Vaccine administered that was stored improperly Wrong route

Strategies to Prevent errors

Strategies to Prevent errors Vaccine administration errors can result in a patient receiving an ineffective immunization Vaccine administration errors may also diminish patient confidence in their healthcare providers

Strategies to Prevent errors Rotate vaccines -shortest expiration dates are in front Remove any expired vaccines

Strategies to Prevent errors If reconstitution of vaccines is necessary, reconstitute ONLY with diluents supplied for specific vaccine. DO NOT MIX diluents

Strategies to Prevent errors Do not store sound-alike and look-alike vaccine close to each other. Check the vaccine vials 3 times before you administer it. Store pediatric and adult vaccines on separate shelves

Strategies to Prevent errors Labeling the area where vaccines are stored can help staff quickly locate and choose the correct vaccine. Labels can be placed on the containers or bins or directly attached to the shelves Color coding the labels(one color for pediatric and another for adult vaccines)

Reporting Vaccination Errors Error reporting should provide opportunities to discover how errors occur and to share ideas to prevent or reduce those errors in the future. Follow clinic policy for reporting medication errors Our Mission: To protect and improve the health and environment of all Kansans.

VERPS National Vaccine Error Reporting Surveillance System VERP-report vaccine errors anonymously Purpose-capture the causes and consequences of vaccine-related errors. Advocates for changes in vaccine names, labeling or other appropriate modifications that could reduce the likelihood of vaccine errors in the future. Our Mission: To protect and improve the health and environment of all Kansans.
