2 At Opticlar we have developed a unique range of laryngoscopes and other ancilliary intubation accessories designed to aid and simplify airway management in exotics, companion animals and larger species. Endotracheal intubation is one of the most important skills a veterinary technician should master. Preparation with the proper equipment and knowledge of the correct technique for intubating every species can be lifesaving. Endotracheal intubation is the placement of a tube that extends from the oral cavity into the trachea. Purpose To administer inhalation anesthetic drugs To ensure a patent airway in unconscious animals To administer oxygen To provide ventilatory assistance Some Complications 1. Trauma to teeth or mucous membranes of the mouth, soft palate, pharynx, or larynx 2. Tracheal inflammation or necrosis can be caused by over inflating the cuff or forcing the endotracheal tube 3. Subcutaneous emphysema secondary to tracheal trauma 4. Obstruction of the airway with secretions 5. Inadequate ventilation due to introduction of endotracheal tube into a bronchus. Excessive advancement of the endotracheal tube down the airway may result in endobronchial intubation. In these circumstances, one lung receives no ventilation and blood deoxygenation may occur. 6. Aspiration of endotracheal tube 7. Laryngeal spasm Basic Technique Pass a cotton bandage or IV tubing behind the canines on the upper jaw to open the jaw. Grasp the tongue with a gauze sponge and open the mouth fully by firmly pulling the tongue out and down Adjust the light so that you have good illumination of the larynx Use the tube or laryngoscope to gently displace the epiglottis ventrally or the soft palate dorsally until the glottis can be visualized. Gently insert the tube past the vocal folds using a rotating motion. never force the tube Check the tube to ensure that the it is in the appropriate distance and is oriented to match the natural curve of the trachea Secure the tube with cotton bandage or used IV tubing (over the nose for dolichocephalic dogs and behind the head for cats and brachycephalic dogs. Be sure the tie is secure enough not to slip but does not compress the tube. Inflate the cuff and check for leaks Ensure a patent airway by checking the position of the patient and tube.
