1 Information Pack Thank you for your interest in The Ear Foundation s Summer Residential to Dukes Barn. The week is staffed by Teachers of the Deaf, Speech and Language Therapists, Care Workers, The Ear Foundation and Dukes Barn teams who are all experienced in working with young deaf people. You can therefore be confident that they be well looked after and we will make sure that have a fantastic time and get the most out of it. This event was inspected by Ofsted in 2015 and awarded Outstanding further reassurance that your children are in safe hands. This information pack has all the details that you need for the event. 2 Booking Timeline Information packs issued Parent/carer contact Clare Allen to place their young person's name on the provisional list 50 Deposit and consent form to be received by Clare Allen to confirm their young people's place - booking is on a first come first served basis (it is possible to pay in full at this stage) Confirmation of a young person's place will be ed to the parent by The Ear Foundation Balance to be received by The Ear Foundation by 31st May 2015 Confirmation of receiving all information will be sent to the parents with the confirmed detailed programme If the cost of the event means that your child will not be able to attend please speak to us.
3 Day time: Programme Monday 15 th August Morning - Meeting at The Ear Foundation and travelling to Dukes Barn Afternoon - Outdoor activities at Dukes Barn Tuesday 16 th August Day Trip Wednesday 17 th August Outdoor activities at Dukes Barn Thursday 18 th August Day Trip Friday 19 th August Morning - Outdoor activities at Dukes Barn Afternoon Return journey to The Ear Foundation Outdoor Activities: The teenagers will be divided into groups by The Ear Foundation team. The groups will be divided into ages and language preference so that children who use sign language will be together with a leader who signs and the children that use spoken language are with leaders who use spoken language. The details of the exact activities will be confirmed closer to the date, for more information about Dukes Barn please have a look at their website: Each group will have 1 instructor from Dukes Barn and minimum of 2 Ear Foundation leaders. For some of the activities we will travel in a mini-bus to get to the location. Dukes Barn will provide all the specialist equipment. Your children will need to have trainers and sensible clothing to complete the activities. Day Trips: The teenagers will be divided into groups by The Ear Foundation team; again they will be with leaders who can communicate in the best way for them personally. We will go on a mini bus for a day trip details of the day trips will be confirmed closer to the date. Evening Activities During the evening The Ear Foundation team will arrange games and activities at Dukes Barn.
4 Kit List Packed lunch for the first day (all other meals will be provided) Spare batteries for implants or hearing aids and dry box (and any other spares they have) Trainers for outdoor activities Extra trainers raft building / canoeing or water activities Alternative shoes for wearing indoors Comfortable old clothes for outdoor activities Sun hat and sun tan lotion (if we re lucky!) Jacket or fleece (in case it s cold!) Towel Swimming costume (to wear under clothes for water activities) The usual stuff: underwear, socks, washing things etc Pocket money for tuck shop and day trips They can bring the following but they will need to be responsible for them: Phones Ipods Camera Hand help games consoles Please ensure that there are the appropriate safety settings on mobile devices so that the young people are not able to access inappropriate websites. All items need to be clearly named.
5 Booking form In order that we can support your child to have a fantastic time and be kept safe we need the following: A recent photo of your child passport style so that it shows their face clearly The booking form below The completed Dukes Barn consent form on the next 2 pages Name Parent s address Age at time of event Cause of a deafness Other conditions/syndromes Medication taken (please note you will have to complete Medication form to allow us to administer their medication) What do they like to do What don t they like to do What support do they need to have a great time This information will be kept on The Ear Foundation database for this and future events. The consent form will be shared with Duke Barn but neither party will pass on any details to third parties.
6 Parental Consent Form This form must be completed by the parent or guardian of any person under the age of 18 before they may participate in any activities organised by Dukes Barn. All information contained in this form will be treated as confidential. Please give an honest appraisal of your child so that we can support them appropriately. 7 Doctors Name: Doctors Doctors address: Declaration I hereby consent to my child participating in organised activities described in attached programme, travelling in a minibus under appropriate supervision at all times and also authorise you to act in loco parentis with reference to my child s general health and well being. Most courses at Dukes Barn include one or more adventurous activities. The Centre s staff have, and fully accept, a duty of care to make those activities as safe as is reasonably practical. They are required to adhere to a comprehensive set of safety rules and there is a quality management system to monitor the fact that rules and procedures are adhered to. However, adventurous activities are inherently hazardous and cannot be completely risk free however hard we try. Accidents can happen without any contributory negligence from the Centre or its staff. Moreover, the environment at the Centre is such that we cannot fence off all hazardous areas. Your son or daughter must therefore help our staff to look after their safety by listening carefully to instructions, by doing what they are asked to do and by not being reckless by trying to do more than they have been briefed to do. Only give your consent if you are confident that they will behave responsibly in this way. The Centre can accept no responsibility for loss of or damage to personal property or for personal injury not arising as a result of its own action or default. I understand and accept the above statements. My son or daughter is fit for the course and will inform the Centre before the course of any special medical conditions that might affect my son s or daughter s safety. I will ensure that my child has all the appropriate batteries and spares for their cochlear implant / hearing aid / bone anchored hearing aid and I understand that I will be contacted in case of any problem with this. I understand that if my child behaves in a way which detracts from the enjoyment or safety of the other young people, I will be available to collect them, alternatively they could be stopped from attending activities and remain at the centre with an adult. These may be used for our own (Dukes Barn or The Ear Foundation) publicity purposes, for example on static displays, website, information leaflets or annual reports.
