1 SIT Pre-Enrolment Medical Examination Appointed Clinics Dear Students, SIT has appointed three healthcare providers for the following services at special rates for SIT students. These special student rates are valid during the matriculation period (15 May 2018 to 30 Aug 2018) ONLY. Students are required to bear their own expense and pay direct to the clinics for their services. There will be no reimbursement by SIT made to the student. 2. Please submit updated laboratory reports, do note that all reports must be dated after 1 February Students are advised to complete the medical check-up as soon as the matriculation package is received. The medical report will take approximately 1-2 weeks to be processed by the medical provider. After the completion of the full medical assessment. A longer processing time will be required if repeated test(s) are needed, due to abnormal results (if any). 4. Students are required to bring their past medical records when visiting any medical provider (eg. Health booklet, documented evidence of immunity, vaccination proof) or existing medications, if any. 5. Self-declaration is not accepted as proof of immunity. 6. Students are to ensure that they and their physician completes all the relevant sections of the SIT Medical Examination Form. An incomplete form will not be accepted. For the incomplete forms the ownership lies on the student to return to the clinic to get the forms completed and submit the forms to SIT@Dover. 7. Female students are advised to complete the medical check-up before the next menstruation cycle. If students are menstruating on the day of appointment, please re-arrange the medical check-up 5 days after last day of menstruation. 8. For vaccinations, students will be required to return to the respective clinics for second and/or third dose, if any. If students have any drug allergy, always inform the medical provider before undergoing any type of vaccination. 9. It is compulsory for international students to complete the ICA Medical Examination Report for the issuance of a Student Pass, in addition to the SIT Medical Examination form. 10.
