1 Summit County Dental Task Force Minutes of the Meeting May 25, 2011 Mission: To assure that all people in Summit County have access to dental care PRESENT: Gene Nixon, Dar Theodus, Dr. Robert Fulton, Dr. Jim Lalumandier, Nancy Stanwick, Tom Quade, Sue Gerberich, Judy Burkett, Nancy Woolverton, Jennifer Film, Gillian Solem, Dr Jay Gershen, Dr Donald Raabe, Dr Diana Biordi, Dr. Adam Pollack, Dr. Kris Drummond, Greta Lax, Bob Howard, Alice Garst and Brenda Cox Gene Nixon called the meeting to order at 3:04 p.m. MINUTES: A motion was made to approve the minutes of the February 9 th meeting with corrections. OLD BUSINESS: Mary Agnes Kendra has a grad student/intern who has been instrumental in designing the DTF 2011 Report, which includes a website link with interactive modes. Project should be completed by June 20 th. GOALS FOR DENTAL TASK FORCE/DTF REPORT: This Dental Report is the third in eleven years to cover all oral health care in the county. The report offers recommendations. The dental task force checks to make that certain recommendations are being followed. Gene Nixon stated that the new purpose of the report should be to link oral health status with overall health. Before the final printing everyone will have an opportunity to review. Dar read the 2011 Dental Task Force Recommendations Recommendation 1: Provide a focused marketing public health message on oral health. Dr. Fulton stated that the message of dental/oral health, hygiene and oral practices need to step up and show how all this affects total health. Gillian Solem referenced a recent article about pregnant women not realizing the effects of oral health on pregnancy will go to other doctors. Also it was noted that new medical professionals are requesting more information about dental/oral health care during rotations. Gene wants to make certain there is a concurrence to move these recommendations forward. Dr. Gershen of NEOMED offered a business case for good oral health Days of work lost and the costs associated. 1

2 Recommendation 2: Adopt a community health model, which may include a component of a Community Health Dental Worker and/or mid-levels. Gene Nixon thinks this a worthy strategy. It was noted that Summit County takes 1/3 of the new refugees in Ohio. Currently there is a grant request out to address this issue. Recommendation 3: Encourage and support the establishment of M.P.H. oral health programs, which collaborate with local health districts and community health programs. Current programs: Kent State University, Case Western Reserve University and NEOMED. Recommendation 4: Work in partnership with local universities and colleges to implement an Education for Service initiative for community health models. NEW BUSINESS: GUEST SPEAKER PRESENTATIONS: Dr. Lalumandier, DDS, MPH DHAT/Midlevel models, MPH & DPH CWRU It is now time to open residency in Dental Public Health certification. The hope is that in August the program will run one year full-time and a two year part-time dental residency for individuals with a MPH. The program is only open to dental students/dds s. Midlevel Models In a recent article in Cleveland Plain Dealer the state s dental care for children gets a grade of B, primarily for the access to fluoridated water and tooth sealant programs. The Kellogg Foundation thinks Ohio needs another level of dentist: Dental Therapists, which can be a confusing term. Using the Alaska mode, Dental Health Aide Therapist as seen in the presentation at last meeting by David Maywhoor, Dr. Lalumandier doesn t feel this model provides enough education or requires much prior experience from students who will be caring for people in remote areas without supervision. Minnesota (MN) Metropolitan State model requires more education. The ADHP model requires a Dental Hygienist license, BA and additional course work. This can lead to a MS for Oral Health Case Practitioners, becoming licensed in both and by doing and addional 1,000 hours.. MN Model #2 - University of Minnesota also has a program whose admission criterion requires a BA in any field. In Dr. Lalumandier s opinion this program is not as impressive. The Advanced Dental Therapist works in collaboration with a dentist with an additional 2000 hours of work. 50% of the work is with an underserved population. This issue may be further addressed as Patrick Lloyd has been named dean of the College of Dentistry at Ohio State University. He most recently comes from the University of Minnesota. Point of Information: Within the next four or five years there will be an additional 20 more dental schools in the United States. Comparison of DH/DT vs DDS can be found in the Journal of Public Health Dentistry. 2

3 Comments: Dr. Gershen noted the research compares to Nurse Practitioners in The Netherlands vs US DHAT. The scope of the article was commissioned by the Kellogg Foundation for the 5 states under study. Dr. Pollock stated the Akron Dental Society is more concerned with territory vs location and price. If you put a DH/DT into the area what would happen? What about the perceived image of clients? Gene Nixon suggests better created models to continue the review of models. He stated there are enough dentists in NE Ohio; they are just not in the right areas. He suggests a formal letter to show support of the review of options and bring together those gathering information on new midlevel workers. Dr. Kris Drummond, DDs, MPH Community Dental Health Model, Community Health Coordinator A handout template was provided of the model. This program is currently in PA and CA. This model would be other than DHAT. This is a basic level not providing care just education since it would have to get passed by ODH. The Community Dental Healthcare Coordinator (CDHC) would work with/in community settings to collect information, like photographs, radiographs, to assist dentists triage patients. This was meant to be discussion of what we want our program to look like. It would address the barriers we are currently facing. Greta Lax, MS Interdisciplinary approach for Health Education/dental-medical model Akron-Region Interprofessional Area Health Education Center (ARI-AHEC). Akron-Region Interprofessional AHEC is a new center which replaced Summit Portage AHEC last year (for those who may have been familiar with the previous center). ARI-AHEC's service area is still Summit and Portage counties. Greta s current program works with students, middle through high school, interested in working with underserved populations. The focus of the program is on interdisciplinary collaboration. The organization is hoping to help make this experience available throughout the area. AHECs are a federal program that works with individuals at many levels, not just elementary and secondary students. The overall goal is improving health care access to underserved populations. "Through community-based interdisciplinary training programs, AHECs identify, motivate, recruit, train, and retain a health care workforce committed to underserved populations" (National AHEC Organization). Activities to meet this goal include: 1. Pipeline development: introducing health careers to elementary/secondary students from underserved populations and providing programming to help them to be successful in pursuing their goal careers 3

4 2. Clinical experiences: facilitating clinical experiences for health professions students, specifically working with underserved populations 3. Continuing education: for health professionals currently serving the target populations 4. Interprofessional education: initiatives to improve understanding of roles and contributions of different health disciplines for students and practitioners Additional information on AHEC: National AHEC Organization Comments: Gene thinks this program fits well with what the task force wants to do. Dr. Robert Fulton, MA, DDS, FADI Oral Health how do we incorporate it into medical education? Dr. Fulton provided a handout of his presentation. He feels this is fixing the problem after the fact. This is a worldwide pandemic and we are looking at how to cure it. He wants to get involved in some research and get another dental residency program in the Summit County area. He is very concerned those in the medical profession have a good understanding of the whole body: Doctors should look in mouths and dentists should understand the total body health. Comments: Both Gene and Greta agree and call for all medical professionals to provide a message that is consistent so people can learn how to be healthy. Dr. Jay Gershen NEOMED & Janine Janosky Medical Corridor, Austen Bio Innovation Institute Education for service how we can incorporation dental into the scholarship programs? How can we collaboratively move forward to reach our goal? Dr. Janosky was unable to attend this meeting. Dr. Gershen provided a handout and gave an overview of NEOMED, formerly NEOUCOM which has been in existence for over 30 years, since Upcoming plans development of a middle school to health professions practice pipeline - getting a five year commitment from students by providing free tuition. This National Health Service would keep trained populations in the community. NEOMED has received foundation and grant support for the planning strategies. It was reported that Dr. Janosky is working with the county looking at how health access is currently working in this area. Questions and comments Sue Gerberich announced that there will be an Accountable Care Community Conference presented by Austen Bio Innovation Institute of Akron (ABIA) on June 22 at JS Knight Center. She will be forwarding information to the task force. 4

5 Dr. Fulton suggested that everyone should provide 2 paragraphs on their thoughts from today s meeting. Dar would like everyone to give her their thoughts on a symposium. Dr. Gershen offered NEOMED as an alternate meeting place. The meeting was adjourned at 4:45pm Next meetings are July 13, :00 pm for DTF Report Review Committee and tentatively set for August 24, 2011 for the committee for Symposium.
