1 cvetko.qxd :07 Page 124 Zobozdrav Vestn 2009; 64: RAZISKAVA POLOŽAJA ODPRTINE SPODNJE ČELJUSTNICE NA SKELETNEM GRADIVU A study of the position of the mandibular foramen in dry mandibles Ključne besede: odprtina spodnje čeljustnice, skeletno gradivo, položaj Key words: mandibular foramen, dry mandible, position Izvleček Izhodišča: Položaj odprtine spodnje čejustnice (OSČ) ocenimo pred dajanjem prevodne anestezije ob spodnji alveolni živec in pred nekaterimi kirurškimi posegi na spodnji čeljustnici. Namen raziskave je bil določiti vodoravni in navpični položaj OSČ na skeletnem gradivu. Gradivo in metode dela: Na 68 spodnjih čeljustnicah odraslih oseb smo izmerili razdalje med anatomskimi referenčnimi točkami na notranji ploskvi ramusa spodnje čeljustnice. Rezultati: Če smo za sprednji rob ramusa upoštevali zunanji, lateralni rob (linea obliqua), je bila OSČ na levi strani 2,5 mm in na desni strani 2,7 mm za središčnico ramusa. Če pa smo za sprednji rob ramusa upoštevali njegov medialni rob (crista temporalis), je bila OSČ na levi strani 1,0 mm in na desni strani 1,3 mm za središčnico ramusa. Navpični položaj OSČ je bil nekoliko nad sredino ramusa (1,0 mm na levi in 1,9 mm na desni strani) in pod ravnjo grizne ravnine (4,7 mm na levi in 4,9 na desni strani). Zaključek: Položaj OSČ na preučevanih spodnjih čeljustnicah je bil raznolik, v povprečju je bila OSČ za središčnico skozi ramus in nad sredino ramusa. Za oceno položaja OSČ je potrebno upoštevati sprednji medialni rob ramusa, ki ga tipamo pri dajanju lokalne anestezije ob spodnji alveolni živec. Abstract Introduction: The position of the mandibular foramen (MF) is estimated before performing an inferior alveolar nerve block and before some surgical procedures on the lower jaw. The purpose of the study was to determine the horizontal and vertical position of MF on dry mandibles. Material and Methods: Distances between anatomical landmarks on the inner surface of the mandibular ramus were measured on 68 adult human mandibles. Results: The horizontal position of MF was posterior to the midpoint of the ramus. The distance between MF and the midpoint of the ramus on the right and the left sides was respectively 2.5 mm and 2.7 mm if the external oblique line was taken as the anterior border of the ramus, and 1.0 mm and 1.3 mm if the internal oblique ridge (crista temporalis) was taken as the anterior border of the ramus. The vertical position of MF was slightly above the centre of the ramus (1.0 mm on the left and 1.9 mm on the right) and below the occlusal plane (4.7 mm on the left and 4.9 mm on the right). Conclusion: The position of MF on the studied mandibles was variable, but on average MF was situated behind the midpoint and slightly above the centre of the ramus. For estimation of the position of MF, the medial anterior border of the ramus is clinically relevant, since it is palpated when applying an inferior alveolar nerve block.

2 cvetko.qxd :07 Page Uvod Položaj odprtine spodnje čeljustnice (OSČ) (foramen mandibulae) ocenimo pred dajanjem prevodne anestezije ob spodnji alveolni živec (n. alveolaris inferior) in pred nekaterimi kirurškimi posegi na spodnji čeljustnici (Reitzik in sod., 1976; Rajchel in sod., 1986; Wolford in Henry, 1993). Prevodna anestezija spodnjega alveolnega živca je neuspešna v od 29 do 35 % primerov (Wong in Jacobson, 1992; Afsar in sod., 1998). Neuspeh raziskovalci pripisujejo več dejavnikom, med katerimi je pogost raznolik položaj OSČ in napačna klinična ocena njenega položaja. Prevodno anestezijo spodnjega alveolnega živca dosežemo z vbrizganjem lokalnega anestetika v bližino OSČ na notranji ploskvi ramusa spodnje čeljustnice (ramus mandibulae), skozi katero živec vstopi v mandibularni kanal (canalis mandibulae). Pri bolniku OSČ ne moremo tipati, zato njen položaj ocenimo glede na anatomske referenčne točke (Bennett, 1984). Anatomski učbeniki navajajo, da je OSČ približno v sredini ramusa spodnje čeljustnice (Williams in Warwick, 1995), ali pa njenega položaja ne navajajo. Antropometričnih študij, ki bi ugotavljale položaj OSČ, je malo, njihovi podatki pa se razlikujejo. Avtorji opisujejo položaj OSČ glede na različne referenčne točke na notranji ploskvi ramusa. Navajajo, da je OSČ tik za središčnico skozi ramus (Hetson in sod., 1988), v srednji tretjini ramusa (Mwaniki in Hassanali, 1992), v sredini ramusa (Weiss, 1972; Nicholson, 1985; Keros- Naglič in sod., 1997; Jerolimov in sod., 1998), na polovici razdalje med zarezo spodnje čeljustnice (incisura mandibulae) in spodnjim robom ramusa (Nicholson, 1985; Mwaniki in Hassanali, 1992) ter nekoliko nad polovico ramusa (Bremer, 1952; Hayward in sod., 1977; Oguz in Bozkir, 2000). Navpični položaj OSČ avtorji opisujejo tudi glede na grizne ploskve spodnjih kočnikov. Medtem ko so nekateri avtorji OSČ ugotovili na vseh pregledanih spodnjih čeljustnicah nad ravnino griznih ploskev kočnikov (Hamilton, 1976; Basmaijan, 1980), drugi pa v višini griznih ploskev spodnjih kočnikov (Last, 1998), so tretji ugotovili, da je položaj OSČ raznolik glede na ravnino griznih ploskev kočnikov: v njihovi ravnini, pod in nad njo (Nicholson, 1985; Mbajiorgu, 2000; Oguz in Bozkir, 2002). Glede na klinični pomen položaja OSČ in na dejstvo, da njen položaj pri našem prebivalstvu še ni bil določen, je bil namen raziskave ugotoviti vodoravni in navpični položaj OSČ glede na anatomske referenčne točke na notranji ploskvi ramusa spodnje čeljustnice. Gradivo in metode dela Skeletno gradivo Položaj OSČ smo ugotavljali na 68 spodnjih čeljustnicah odraslih oseb iz učnih zbirk Inštituta za anatomijo in Inštituta za sodno medicino Medicinske fakultete v Ljubljani. Pri 25 spodnjih čeljustnicah je število zob zadostovalo za določitev ravnine griznih poskev kočnikov (na vsaki strani sta bila prisotna najmanj dva kočnika), 17 spodnjih čeljustnic je imelo premajhno število kočnikov za določitev ravnine griznih ploskev kočnikov, 26 pa je bilo brezzobih. Pri večini spodnjih čeljustnic sta bila spol in starost neznana, zato ju v raziskavi nismo upoštevali. Meritve na skeletnem gradivu Položaj OSČ smo določili na levi in desni strani spodnje čeljustnice z merjenjem navpičnih in vodoravnih razdalj med anatomskimi referenčnimi točkami na notranji ploskvi ramusa. Za določitev vodoravnega položaja OSČ smo izmerili (Slika 1 a, b): -- širino ramusa na najožjem mestu (ŠR) -- najkrajša razdalja med sprednjim lateralnim in zadajšnjim robom ramusa -- razdaljo med OSČ in sprednjim lateralnim robom ramusa (linea obliqua) (OSČ--SLR) -- razdaljo med OSČ in sprednjim medialnim robom ramusa (crista temporalis) (OSČ--SMR) -- razdaljo med OSČ in zadajšnjim robom ramusa (OSČ-ZadR) Ramus ima spredaj dva ločena robova (Slika 1). Lateralni oziroma zunanji rob je oster in se nadaljuje navzdol ter navzpred na telo spodnje čeljustnice (corpus mandibulae) kot poševna črta (linea obliqua). Medialni oziroma notranji rob je zaobljen in poteka od koronoidnega odrastka (processus coronoideus) navzdol ter se konča za zadnjim kočnikom v retromolarnem trikotniku kot temporalni greben (crista

3 cvetko.qxd :07 Page 126 Raziskava položaja odprtine spodnje čeljustnice na skeletnem gradivu 126 temporalis). Lateralni rob je bolj oddaljen od OSČ kot medialni. Da bi natančno določili položaj OSČ, smo merili razdaljo od OSČ do obeh sprednjih robov ramusa. Razdaljo med OSČ in sprednjim oziroma zadajšnjim robom ramusa smo merili od sprednjega oziroma zadajšnjega roba OSČ. Izmerjenim razdaljam smo prišteli polovico širine (notranjega premera) OSČ in dobljene vrednosti upoštevali v izračunih. Za določitev navpičnega položaja OSČ smo izmerili (Slika 1 b): -- višino ramusa (VR) -- razdalja med najnižjo točko zareze spodnje čeljustnice (incisura mandibulae) in spodnjim robom spodnje čeljustnice -- razdaljo med OSČ in najnižjo točko zareze spodnje čeljustnice (OSČ--ZSČ) -- razdaljo med OSČ in spodnjim robom ramusa (OSČ--SpR) -- razdaljo med OSČ in ravnino griznih ploskev spodnjih kočnikov (OSČ--GR). Ravnino griznih ploskev spodnjih kočnikov smo določili z merilom, ki smo ga položili na grizne ploskve spodnjih kočnikov. Navpične razdalje smo merili vzporedno s središčnico skozi ramus in upoštevali najnižjo točko OSČ, kjer je stik z lingulo. Izmerili smo tudi: -- širino OSČ (ŠOSČ) -- razdalja med sprednjim in zadajšnjim robom OSČ (notranji premer) -- višino lingule (VL) -- razdalja med najnižjo točko OSČ in vrhom lingule -- kot spodnje čeljustnice (KSČ) -- kot med tangento na zadajšnji rob ramusa in spodnjim robom spodnje čeljustnice Razdalje smo merili z digitalnim kljunastim merilom in meritve zaokrožili na 0,1 mm. Kot spodnje čeljustnice smo merili s pomičnim kotomerom v stopinjah. Meritve smo ponovili na 20 naključno izbranih spodnjih čeljustnicah. Razlike med povprečnimi vrednostmi prvega in ponovljenega merjenja, ki smo jih ugotavljali s parnim testom t, niso bile statistično značilne. Statistična obdelava podatkov Za določitev vodoravnega položaja OSČ smo izračunali povprečno razdaljo med OSČ in sprednjim oziroma zadajšnjim robom ramusa, odmik OSČ od središčnice skozi ramus in razmerje med odmikom OSČ od zadajšnjega roba ramusa in širino ramusa. Za določitev navpičnega položaja smo izračunali odmik OSČ od sredine

4 cvetko.qxd :07 Page ramusa in razmerje med odmikom OSČ od spodnjega roba ramusa in višino ramusa. Za ugotavljanje levo-desnih razlik med merjenimi spremenljivkami smo uporabili test t. Medsebojno povezanost merjenih spremenljivk smo ugotavljali s koeficientom korelacije po Pearsonu. Statistično značilnost korelacije smo ugotavljali s testom hi-kvadrat. Pri vseh testih smo razliko kot statistično značilno obravnavali pri p 0,05. Podatke smo statistično obdelali s programskim paketom SYSTAT 5.0 za Okna. Rezultati Izmerjene vrednosti kažejo raznolikost velikosti in oblike preučevanih spodnjih čeljustnic (Razpredelnica 1, Slika 2) in se ne razlikujejo značilno med levo in desno stranjo. Širina ramusa na najožjem mestu je bila v povprečju 28,5 mm na levi in 28,2 mm na desni strani, povprečna višina ramusa pa 46,8 mm na levi in 46,4 mm na desni strani. Vodoravni položaj OSČ Razdalja med OSČ in sprednjim lateralnim robom ramusa (v povprečju 17,6 mm na levi in 17,8 mm na desni) je bila na vseh spodnjih čeljustnicah večja kot razdalja med OSČ in sprednjim medialnim robom ramusa (v povprečju 14,6 mm na levi in 15,1 mm na desni) (Preglednica 1). Povprečna razdalja med sprednjim medialnim in sprednjim lateralnim robom ramusa je bila 3,0 mm na levi in 2,7 mm na desni strani. Povprečna razdalja med OSČ in zadajšnjim robom ramusa (12,6 mm na levi in 12,3 mm na desni) je bila manjša od povprečne razdalje med OSČ in sprednjim lateralnim robom in prav tako manjša od razdalje med OSČ in sprednjim medialnim robom. Razlike niso bile statistično značilne. Razdaljo med OSČ in zadajšnjim robom ramusa smo izrazili v odstotkih širine ramusa. Če smo upoštevali sprednji lateralni rob ramusa, je bila OSČ v povprečju za 41,9 % širine ramusa oddaljena od zadajšnjega roba ramusa na levi in za 41,0 % širine ramusa na desni. Če smo upoštevali sprednji medialni rob ramusa, je bila OSČ v povprečju za 46,4 % širine ramusa oddaljena od zadajšnjega roba ramusa na levi in za 45,1 % širine ramusa na desni. (Preglednici 2 in 3). Izračun položaja OSČ glede na središčnico je pokazal, da leži OSČ v povprečju 2,5 mm na levi in 2,7 mm na desni za središčnico skozi ramus, če pri merjenju širine ramusa upoštevamo njegov lateralni sprednji rob, in 1,0 mm na levi ter 1,3 mm na desni za središčnico, če smo za sprednji rob ramusa upoštevali njegov medialni rob (Preglednici 2 in 3). Navpični položaj OSČ V povprečju je bila OSČ nad sredino navpične razdalje med spodnjim robom ramusa in zarezo spodnje čeljustnice (1,4 mm na levi in 1,9 mm na desni strani) (Preglednica 4). Od spodnjega roba ramusa je bila OSČ v povprečju odmaknjena 24,7 mm na levi in 25,1 mm na desni strani, od zareze spodnje čeljustnice pa 22,1 na levi in 21,3 mm na desni strani. Povprečen odmik OSČ od spodnjega roba ramusa izražen v odstotkih višine ramusa je bil 53,0 % na levi in 54,2 % na desni (Preglednica 4). Razdaljo med OSČ in ravnino griznih ploskev kočnikov je bilo mogoče izmeriti pri 25 spodnjih čeljustnicah. Pri 23 spodnjih čeljustnicah (92 %) je bila OSČ pod ravnino griznih poskev kočnikov 1 do 11 mm (x = 4,4 ± 2,9 mm na levi in 4,9 ± 2,1 mm na desni). Le pri dveh spodnjih čeljustnicah (8 %) je bila OSČ nad ravnino griznih poskev spodnjih kočnikov (od 2 do 3 mm). Najpogosteje (v 67 %) je bila OSČ nad sredino ramusa in za navpično središčnico skozi ramus, t. j. v zgornjem zadajšnjem kvadrantu ramusa (Slika 2), kar je statistično značilno pogosteje kot v vsakem od preostalih kvadrantov: v 17 % je bila OSČ v spodnjem zadajšnjem kvadrantu, v 10 % v sprednjem zgornjem in v 6 % v sprednjem spodnjem kvadrantu. Širina OSČ, višina lingule in kot spodnje čeljustnice Širina OSČ, oblika in višina lingule ter velikost kota spodnje čeljustnice kažejo značilno raznolikost (Preglednica 1, Slika 3). Povprečna širina OSČ je bila 4,5 mm na levi in 4,8 mm na desni. Povprečna višina lingule je bila 8,0 mm na levi in 8,0 mm na desni. Kot spodnje čeljustnice je bil v povprečju 126,6 na levi in 126,4 na desni. Med velikostjo kota spodnje čeljustnice in položajem OSČ nismo ugotovili povezanosti.

V odstotkih je prikazan delež OSČ v posameznih kvadrantih ramusa spodnje čeljustnice. Razprava Raziskava medialne ploskve ramusov 68 spodnjih čeljustnic je pokazala raznolikost njene velikosti in oblike ter položaja OSČ, ki je v povprečju za središčnico skozi ramus, nad sredino ramusa in pod ravnino griznih ploskev spodnjih kočnikov. Spodnjih čeljustnic, vključenih v raziskavo, nismo analizirali po spolu. Afsar in sod. (1998) poročajo, da v položaju OSČ ni značilnih razlik med spoloma. Raziskane spodnje čeljustnice niso imele znakov skeletnih nepravilnosti, zato izmerjene vrednosti v takšnih primerih ne veljajo. Vodoravni položaj OSČ na raziskanih spodnjih čeljustnicah se je razlikoval glede na to, ali smo pri določanju širine ramusa upoštevali njegov medialni ali njegov lateralni sprednji rob. Če smo za sprednji rob ramusa upoštevali zunanji, lateralni rob (linea obliqua), je bila OSČ 2,5 mm na levi in 2,7 mm na desni za središčnico ramusa oziroma oddaljena od zadajšnjega roba ramusa za 41,9 % širine ramusa na levi in za 40,1 % širine ramusa na desni. Če pa smo za sprednji rob ramusa upoštevali njegov medialni rob (crista temporalis), je bila OSČ 1,0 mm za središčnico ramusa na levi in 1,3 mm na desni oziroma oddaljena od zadajšnjega roba ramusa za 46,4 % širine ramusa na levi in za 45,1 % širine ramusa na desni. Sprednji medialni rob je bil v povprečju za 2,5 mm bliže zadajšnjemu robu ramusa kot sprednji lateralni rob. Nicholson (1985) je edini meril razdaljo od OSČ do sprednjega lateralnega in sprednjega medialnega roba ramusa. V vseh drugih raziskavah so avtorji % 17 % Z A D A J upoštevali sprednji lateralni rob ali pa niso navedli, kateri sprednji rob ramusa so upoštevali pri meritvah, kar otežuje primerjavo rezultatov. Nicholson (1985) poroča, da je OSČ približno v sredini ramusa, in sicer tik pred središčnico, če za sprednji rob ramusa upoštevamo njegov medialni rob, ob upoštevanju sprednjega lateralnega roba pa je OSČ nekoliko za središčnico ramusa. Nasprotno je bila v naši raziskavi OSČ za središčnico ramusa, tudi če smo pri širini ramusa upoštevali njegov sprednji medialni rob. Ker pri dajanju lokalne anestezije ob OSČ za oceno širine ramusa tipamo njegov sprednji medialni rob, je pomembna določitev položaja OSČ glede na ta rob. Naši rezultati se ujemajo z raziskavami, ki navajajo, da je OSČ za središčnico ramusa (Miller, 1952; Hayward in sod., 1977; Hetson in sod., 1988; Mwaniki in Hassanali, 1992; Kaffe in sod., 1994; Afsar in sod., 1998; Mbajiorgu, 2000). Oguz in Bozkir (2002) sta ugotovila, da je na 34 spodnjih čeljustnicah Turkov OSČ 0,5 mm za središčnico na desni in 0,7 mm za središčnico na levi. Mbajiorgu (2000) poroča, da je na 38 spodnjih čeljustnicah Zimbabvejcev OSČ 2,6 mm za središčnico ramusa na desni in 2,1 mm na levi. V obeh raziskavah so avtorji za sprednji rob ramusa upoštevali njegov lateralni rob. V nasprotju z izsledki naše raziskave pa nekateri avtorji poročajo, da je OSČ v sredini ramusa (Weiss, 1972; Hamilton, 1976; Basmajian, 1980; Williams in Warwick, 1995).

Raziskava navpičnega položaja OSČ je pokazala, da je OSČ 1,0 mm na levi in 1,8 mm na desni nad sredino ramusa. Povprečen odmik OSČ od spodnjega roba ramusa, izražen v odstotkih višine ramusa, je bil 52,9 % na levi in 54,2 % na desni. Naši rezultati se ujemajo z raziskavami, ki poročajo o krajši razdalji med zarezo spodnje čeljustnice in OSČ kot med OSČ in spodnjim robom ramusa. Mbajiorgu (2000) poroča, da je OSČ 3 mm nad sredino ramusa. Oguz in Bozkir (2002) navajata najkrajšo razdaljo med zarezo spodnje čeljustnice in OSČ 22,4 mm na desni in 22,2 mm na desni, razdaljo od OSČ do spodnjega roba spodnje čeljustnice pa 31,0 mm na desni in 29,8 mm na levi. Nicholson (1985) ter Mwaniki in Hassanali (1992) navajajo, da je OSČ v sredini med zarezo spodnje čeljustnice in spodnjim robom ramusa. Od 25 spodnjih čeljustnic, pri katerih je bilo mogoče ugotoviti položaj OSČ glede na ravnino griznih ploskev spodnjih kočnikov, je bila OSČ pri 23 spodnjih čeljustnicah (92 %) pod ravnino griznih ploskev spodnjih kočnikov. Povprečna razdalja med OSČ in ravnino griznih ploskev spodnjih kočnikov je bila 4,4 mm. Glede na grizne ploskve spodnjih kočnikov avtorji navajajo različen položaj OSČ. Opisujejo jo nad ravnino griznih ploskev (Hamilton, 1976; Roberts in Sowray, 1979; Basmajian, 1980), v ravni griznih ploskev (Last, 1998) in v različni višini glede na grizne ploskve kočnikov (Nicholson, 1985; Mwaniki in Hassanali, 1992; Mbajiorgu, 2000; Oguz in Bozkir, 2002). Izmed vseh študij, ki navajajo raznolik položaj OSČ glede na grizno ploskve spodnjih kočnikov, so naši rezultati primerljivi z Nicholsonovimi (1985). Poroča, da je

8 cvetko.qxd :07 Page na 80 spodnjih čeljustnicah vzhodnih Indijcev v 22,5 % primerov OSČ v višini griznih ploskev spodnjih kočnikov, v 75 % primerov pa pod njo, ne navaja pa odmika OSČ od ravnine griznih ploskev spodnjih kočnikov. Mbajiorgu (2000) navaja, da je na spodnjih čeljustnicah Zimbabvejcev OSČ v 47,1 % primerov v višini grizne ravnine, v 23,5 % pod njo in v 29,4 % nad njo. Viravudth in Plakornkul (1989) so na spodnjih čeljustnicah Tajcev ugotovili OSČ 10 mm nad grizno ravnino. Domnevamo, da se položaj OSČ glede na grizno ravnino razlikuje pri različnih narodih. Gabriel (1958) in Hetson s sodelavci (1988) poročajo, da sta položaj OSČ in kot spodnje čeljustnice medsebojno povezana. V naši raziskavi nismo ugotovili značilnih povezav med položajem OSČ in velikostjo kota spodnje čeljustnice. Prav tako povezave nista ugotovila Oguz in Bozkir (2002). Mbajiorgu (2000) navaja, da se kot spodnje čeljustnice razlikuje med etničnimi skupinami. Položaj OSČ se v naši raziskavi ni razlikoval med levo in desno stranjo, kar so ugotovili tudi drugi avtorji (Hayward in sod., 1977; Mwaniki in Hassanali, 1992). To se ujema z domnevo, da poteka razvoj obeh strani spodnje čeljustnice pod vplivom enakih dednih dejavnikov (Avis Virginia, 1961). Razlike med levo in desno stranjo posamezne spodnje čeljustnice pa bi bile lahko odraz delovanja okoljskih dejavnikov. Širina OSČ ter oblikovanost in višina lingule na raziskanih spodnjih čeljustnicah kažejo značilno raznolikost. Širina OSČ je bila v povprečju 4,5 mm na levi in 4,8 na desni. Lingula je bolj ali manj izražena kostna letvica, ki s sprednje strani omejuje vhod v odprtino spodnje čeljustnice. Njena povprečna višina je bila 8,0 mm. Podobno višino lingule (8,4 mm) navajata Oguz in Bozkir (2002). Kositboworchai in sod. (2007) poročajo o veliki variabilnosti oblike in višine lingule. Med višino lingule ter položajem in širino MO nismo ugotovili povezave. Razhajanja med raziskavami glede položaja in oblikovanosti OSČ bi bila lahko posledica razlik v natančnosti merjenja razdalj, izboru anatomskih referenčnih točk, velikosti skeletnega gradiva, verjetno pa vsaj delno odražajo dejanske razlike med različnimi etničnimi skupinami oziroma narodi. Zaključek OSČ je bila na preučevanih spodnjih čeljustnicah v povprečju za središčnico skozi ramus, nad sredino ramusa in pod grizno ravnino. Za oceno položaja OSČ je klinično relevanten medialni sprednji rob ramusa, ki ga tipamo pri dajanju lokalne anestezije ob spodnji alveolni živec. Na uspešnost anestezije spodnjega alveolnega živca vplivajo poleg raznolikosti položaja OSČ tudi njena širina ter višina in izraženost lingule. Zahvala Avtorja se zahvaljujeva prof. dr.
