The tooth underwent root canal therapy and the damaged incisal edge was repaired with a composite restoration. According to the patient, the tooth went dark shortly afterwards. The patient came to see me for correction of the discolouration and also closure of the patient s Diastema. The patient was going overseas shortly, so there was little time to spare. It was suggested to try non-vital tooth whitening in view of the difficulty on relying on composite resin alone, to mask this dark discolouration. (Figure 1) Figure 1. A periapical radiograph was taken to ensure a good root filling was present. The palatal composite was removed. Gutta Percha (GP) was found in the coronal areas and subsequently removed from the coronal space. A glass ionomer seal was placed over the GP before placement of the whitening mixture of sodium perborate/water. Two applications of the whitening mixture was carried out over 8 days, however the significant reduction in darkness occurred after the first whitening procedure over 6 days. (Figure 2) Figure 2. The patient returned on day 8 for the final restoration. The old composite was removed and the access cavity was sealed with glass ionomer. There was minimal labial tooth preparation. The enamel was maintained on the labial surface. The enamel surface was etched for 15 seconds, rinsed off before the application of one coat of Universal Scotchbond. This was light cured for 10 seconds. (Figure 3) Figure 3. The discoloured cervical area was covered with a composite flowable opaque mixture (Creative Colours Pink: A2.5 ) Filtek Supreme XTE was chosen as the composite to complete the case. A1Dentine shade was applied to the cervical area. White Enamel and XWBody shade combination to the remaining labial surface. The diastema was closed by the addition of shade A1D to the mesial surface of tooth 11. (Figure 4 & Figure 5) Figure 4. 10 3M ESPE MDI Mini Dental Implants Osseointegration of MDI A histology study conducted by Loma Linda University assessed osseointegration of MDI in miniature swine at three and six months. Histological evaluation was carried out blind by a board-certified oral pathologist, who examined the bone and soft tissues surrounding each implant for evidence of inflammation, abscess formation and necrosis, bone formation and resorption, and fibrous tissue formation next to the implant surface. In addition, samples were evaluated histomorphometrically for boneimplant contact area (BIC). The histological findings were comparable regardless of implant diameter, and the majority of histological changes seen were mild to moderate, with no evidence of infection detected. Histomorphometric evaluation showed bone integration in all implant samples at three months (average BIC of 82% [N=8]) and at six months (84.7% [N=5]). Figures 6 and 7 show representative photomicrographs of typical tissue response to a mini implant after three months. Compared with the 3-month groups, bone modeling and remodeling increased in the 6-month samples. The implants have been used by clinicians for these indications for more than 10 years, and several clinical studies have investigated their shortand long-term performance in the support of removable dentures. The patient population for the fixation of dentures is notable due to the comparatively high average age and the associated co-morbidities. Recent articles (listed below) document that success rates exceed 90% even after several years of observation.
